Fracture of The Sustentaculum Tali: Features and Surgical Management for Fracture Reduction (original) (raw)
International Journal of Life Science and Pharma Research
Sustentaculum tali is a horizontal shelf-like projection on the medial part of the calcaneum. The medial facet articulates to the talus and supports the medial column of the foot. The spring ligament originates at the plantar aspect of sustentaculum tali, and flexor hallucis longus passes below it and indirectly supports the longitudinal arch of the foot. Sustentaculum tali is a strong cortical bone, so its isolated fracture is unusual. Therefore, the fracture of the sustentaculum tali is easily misdiagnosed. The patients complain of pain below the medial malleolus and also on passive movement of the flexor hallucis longus tendon. The patient has a history of falls from height with the foot supinated or associated talus fracture. Sustentaculum tali fractures are sometimes considered to be extra-articular fractures. CT scan helps diagnose the intraarticular medial facet involvement in this fracture and is now regarded as intra-articular. Need of the study is to make the clinicians aw...

Fractures of the sustentaculum tali
Operative Orthopädie und Traumatologie, 2013
Anatomical reconstruction of displaced sustentaculum tali fractures via a direct medial approach. Displaced fractures of the sustentaculum tali with incongruity or depression of the medial facet of the subtalar joint, entrapment of the flexor hallucis longus or flexor digitorum longus tendons, fracture line extending into the posterior facet of the subtalar joint. Infected or grossly contaminated soft tissue, severely restricted vascular supply to the foot, high perioperative risk. Direct medial approach over the sustentaculum tali, retraction of the tendons, joint exploration, fracture reduction using the medial facet and cortical outline as guidelines, fracture fixation with two small fragment screws from medial to lateral directed slightly plantarly and posteriorly. Fractures with depression of the medial facet as a whole can alternatively be reduced and fixed percutaneously. Lower leg splint for 5-7 days, partial weight-bearing with 20 kg for 6-8 weeks (until radiographic signs ...
Place of Orthopedic Treatment in the Talus Fractures: About Two Cases
International Journal of Innovative Research in Medical Science
Fractures of the talus are rare and serious injuries. Their prognosis is bad since they are complicated in most cases of osteonecrosis of the talus; they require adequate treatment either surgical or orthopedic whose aim is to minimize the risk of osteonecrosis and osteoarthritis; the authors report two cases of fracture of the talus treated conservatively with good clinical and radiological outcome two years of decline.
