Accident Risks in the Energy Sector: Comparative Evaluations

Severe accidents are considered one of the most controversial issues in current comparative studies of the environmental and health impact of energy systems. The present work focuses on severe accident scenarios relating to fossil energy chains (coal, oil and gas), nuclear power and hydro-power. The scope of the study is not limited to the power production (conversion) step of these energy chains, but, wherever applicable, also includes full energy chains. With the exception of the nuclear chain, the focus of the present work is on the evaluation of the historical experience of accidents. The basis for this evaluation is the comprehensive database ENSAD (Energy-Related Severe Accident Database), which has been established at PSI. For hypothetical nuclear accidents, a probabilistic technique has also been employed. Article Info Background: Risk assessment is an important tool for reducing casualties and financial damage in the oil and gas industry. This research aimed to identify and evaluate process hazards in the petrochemical industry in 2016. Material and Methods: In this case study, a team was organized and briefed on the process. Besides, hazard identification was performed using the Hazard and Operability Study. Next, causes were analyzed using the Fault Tree Analysis and occurrence probability of top events. Finally, events and subevents were ranked. The minimum cut sets were determined using Boolean algebra. Results: A total of 77 events were identified. Accordingly, unacceptable, tolerable, and acceptable risk levels were 41, 31, and 5 events, respectively. Fire was the most unacceptable risk level, with the final events of "human errors in correct gasket installation on the flange surface" and "flange defects" having had the shares of 51.2 and 21.55%, respectively. Conclusion: The combination of the two HAZOP and FTA techniques is useful in process industries in which incomplete performance of the system and control systems is the most effective factor in the potential occurrence of fire. Human errors and flange defects are the two main factors in this event, so occupational safety and health must be improved in this system. Thus, due to complex interactions between humans, machines, materials, and the environment in systems, such as the petrochemical industry, which lead to uncertainties in safety results of the process, risk assessment is recommended to be performed periodically using different techniques.
Analysis of Fuel Stations Hazards by Using Risk Assessment Criteria

Injury free work environment is an essential element for any successful project. Better safety performances can be achieved only if safety matters priority at all stages of project addressed adequately. Concrete efforts of owners, contractors, sub contractors and designers are equally important at different levels of project. It is necessary to develop such strategies that can help to reduce injury cases. Past safety studies investigated various methods to improve injury statistics but it still contains more room for improvement. This paper will present the application of risk assessment criteria (RAC) to identify critical, major, moderate and minor activities during fuel station operation. Health safety & environmental hazards related to fuel stations activities will be identified, estimate the associated risks and necessary control measures for mitigation of identified risks will be highlighted in this study. The RAC used in this study is widely applying in upstream oil and gas sector in Pakistan. Poor maintenance, carelessness, house keeping, mechanical fault and robbery found to be the major causes of occurrences of unsafe acts and unsafe conditions at fuel stations. RAC are methods of work system design and helpful to address associated risks in a project. During study it was felt that many resources are available within an organization to minimize accidents/injuries but due to not fully utilization of these resources accidents/incidents happen. After calculating risks, application of available resources can be done more appropriately. In case of unavailability, requirements can be highlighted and applied strategically to get better results.
STUDY OF HAZARD IDENTIFICATION TECHNIQUES ADOPTED BY OIL AND GAS INDUSTRIES FOR RISK ASSESSMENT
Risk management in oil and gas industry play vital role in preventing accidents. In oil and gas industry risk assessment is carried out in conceptual stage to end of life cycle of the plant. Accidents in oil and gas industry give catastrophic result to the industry and affect the country's economy. Fire, explosion and toxic gas release from the oil and gas industry kills huge number of employees, publics and damage assets and impact the environment. Major accidents such as Bhopal, Flexibrough, Pipher Alpha, Seveso, BP Blow out, Indian Oil Terminal fire, Texas City, Deep water horizon, Macondo were results in many people died; damaged assets; impacted the environment. Over the last four decades management of risk, from the operation of hazardous facilities are being increased focus. These major accidents emphasize the importance of process safety in oil and gas industries. Risk management is used by oil and gas industry to manage the threats & risks in their operation. Risk management has many steps. The first and key step of risk assessment process is hazard identification. In this research an attempt is made to study the various techniques used by oil and gas industries to identify the hazards and their advantages and limitations. HAZID, HAZOP, SAFOP, Fire and Explosion Index, Mond Index, FMCA, LOPA, FTA are used by various companies to identify the hazards or causes of major incidents.

Accident risks are compared across a broad range of energy technologies. Analysis of historical experience was based on the comprehensive database ENSAD. OECD and EU 27 performed significantly better than non-OECD countries. External costs of accidents are very small, but impacts can still be enormous. No technology performs best for all risk indicators; thus tradeoffs are inevitable.

In 1998 ENSAD, a highly comprehensive database on severe accidents with emphasis on the energy sector, was established by the Paul Scherrer Institute (PSI). The historical experience represented in this database was supplemented by probabilistic analyses for the nuclear energy, to carry out a detailed comparison of severe accident risks in the energy sector [1]. The database allows to perform comprehensive analyses of accident risks, which are not limited to power plants but cover full energy chains, including exploration, extraction, processing, storage, transports and waste management.
