A medical record is a file containing records and documents about the patient's identity, examination, treatment, actions and other services that have been provided to the patient on health care facilities. The problems that occur in the Klinik Utama Meditama Semarang is the management of medical data that is less effective and efficient. This information system aims to manage medical data can run quickly and precisely. This system development method uses [10] SDLC by making [11][12] UML diagrams such as use case diagrams, class diagrams, activity diagrams, entity relationship diagrams as the design stage of the system. This information system is [8][13]WEB-based using PHP programming language supported by MySQL database. The design result of this system is expected to be one solution of the problems that occurred during this time.
