Effect of Pharmacist Counseling on the Success of Therapy and the Quality of Life of Hypertensive Patients in a Hospital in East Nusa Tenggara

Hypertension is a chronic disease that requires an intensive monitoring from time to time to prevent complications leading to cardiovascular disease. In addition, its complex health needs have an effect on the treatment process and the motivation of the patient for treatment. A Study On Effect Of Pharmacist Mediated Patient Counseling In Hypertensive Patients In Terms Of Knowledge, Compliance And Quality Of Life

To create awareness about hypertension in order to improve their quality of life. Free health camps were conducted; screening of blood pressure was done. Data were collected based on medical history, family history, food and social habits and so on. Individual counselling was done in the respective languages in which they were comfortable. Feedback was collected from the patients regarding their satisfaction towards the camp and the services provided to them. Also, patient information leaflets containing the basic information about hypertension were provided to each patient to enhance their knowledge even further. There was a significant decrease in the blood pressure levels at the end of the study as compared to the beginning because they followed our advice and adhered to it, depicting that our counselling was beneficial in the improvement of their Quality Of Life. study conclude that providing individual counselling decrease in the blood pressure level

Improving Outpatient’s Quality of Life through Patient Adherence of Antihypertensive Therapy Using “Mobile Phone (SMS) and Brief Counseling‑5A” in Polyclinic of Internal Medicine at PKU Muhammadiyah Bantul Hospital, Yogyakarta

The Health-Related Quality of Life (HRQoL) is one of the important psycho-social characteristics that can affect patient's ability to manage therapy. Poor of knowledge of hypertension and the changing lifestyle can affect the quality of life of patients. One of the pharmacist's interventions in hypertension management is to conduct counseling. Motivational counseling helps health service to assess patient's understanding and patient's readiness to change patient's behavior. Some motivational counseling methods still need to be developed. Therefore, this study aims to find the influence of the "brief counseling-5A" and "motivational SMS" by a pharmacist on the quality of life and blood pressure control in hypertension patients in the internal disease polyclinic, PKU Muhammadiyah Bantul Hospital, Yogyakarta. The study has been done by using the quasi-experimental method with prospective data collection during the period of January until April 2013. Sixty patients have met inclusion criteria and were divided into two groups. Thirty patients (50%) received "brief counseling-5A" and "motivational SMS" as intervention group and the other thirty patients (50%) received usual care as a control group. The data collection was done by interviewing patients. Medication adherence and QoL were assessed by using Morisky Medication Adherence Scale (MMAS) and SF-36. The values of blood pressure are taken from patient's medical records. Patient's quality of life showed a good improvement during post study. It is shown in 8 different domains including pain, fatigue, physical function, emotional function, social function, role physical, mental health, and general health. In intervention group, physical function, emotional function, and pain showed highly significant improvement (p<0.05). Systolic and diastolic blood pressure in the intervention group decreased significantly (p<0.05) (systolic p=0.001 and diastolic p=0.018) in the post study. From this study, it is concluded that the "brief counseling-5A by pharmacist" and "motivational SMS" had a positive impact on patient's QoL and patient's blood pressure, hence, the therapy outcome has been achieved.

Pharmacists today are aware that the practice of pharmacy has evolved over the years to include not only preparation and dispensing of medication to patients but also interaction with patients and other health care providers throughout the provision of pharmaceutical care. Health related quality of life is increasingly viewed as a therapeutic outcome and is gradually gaining the same level of importance as clinical or physiological outcome parameters. (eg: blood pressure, blood sugar levels) this study aimed to assess impact of pharmacist provided patient education on treatment out comes, KAP& QOL of patients with coexisting diabetes and hypertension. Our study confirms that improvement in knowledge of the disease and its management had positive impact on treatment outcomes and quality of life (PCS). At the same time it is noticed that counseling had no effect on mental component summary of the patient’s quality of life. This study thus emphasis the impact of patient counseling on KAP & QOL in patient with diabetes and hypertension.

IMPACTS OF COUNSELING ON ADHERENCE TO PRESCRIBED MEDICATIONS AND BLOOD PRESSURE OF HYPERTENSIVE PATIENTS IN FOUR INDONESIAN PRIMARY HEALTH CENTERS Original Article

Objective: To analyze the impacts of pharmacist counseling on adherence to medications and reduction in systolic and diastolic blood pressures (SBP/DBP) of hypertensive outpatients. Methods: A retro-prospective cohort study was undertaken to evaluate the impacts of two-month period counseling on medication adherence and SBP/DBP of hypertensive patients (n=47) insured by Social Security Organizing Body in primary health centers (Medan Deli, Helvetia, Darussalam, and Teladan) in Medan. Inclusion criteria were patients diagnosed with hypertension, age ≥ 18 years, and under treatment of antihypertensive drugs. A questionnaire was used to assess characteristics of the patients, antihypertensive drugs provided, and BP. The eight-item Morisky Medication Adherence Scale (MMAS-8) of each patient was assessed. Characteristics of the patients and antihypertensive drugs provided were descriptively analyzed. Impacts of counseling on medication adherence and SBP/DBP of the patients were analyzed using Wilcoxon test. All analyses were performed using Statistical Package for the Social Sciences (SPSS, version 19, Chicago, IL, USA) (p value <0.05 was considered significant). Results: Characteristics of the patients: male, 23.4%, female, 76.6%, mean age, 61.22 ± 9.90 (years). MMAS-8 score improved significantly from 4.58 before counseling to 6.28 after counseling, p = 0.000. There was a reduction in SBP from 160.49±23.15 mmHg before counseling to 149.04 ± 21.02 mmHg after counseling, p = 0.001. DBP also reduced from 91.23 ± 12.82 mmHg before counseling to 87.14 ± 9.94 mmHg after counseling, p = 0.014. Conclusion: Counseling improves adherence to prescribed medications and BP in hypertensive patients.

Satisfaction’s Level of Hypertensive Patients towards Pharmacy Counseling with Al-Quran Based at Karya Sehat Pharmacy in Purwokerto, Banyumas Regency

Hypertension is a major cause of death in some countries. World Health Organization (WHO) estimated there are 11% of undetected hypertension in which 50% of them are living in developing countries such as Indonesia, with the incidence of 25.8%. Pharmaceutical counseling is one of the most comprehensive pharmacist services. Therefore, pharmacists' participation through counseling is needed to provide pharmacological therapy by administration of antihypertensive medication and nonpharmacological therapy, for instance by recital of Al-Quran during pharmaceutical therapy which become the objective of this study. This research aimed to determine satisfaction's level of hypertensive patients with the intervention of pharmaceutical counseling with Al-Quran based as well as effectiveness of the therapy. This research was conducted from February until March 2017 at Karya Sehat Pharmacy in Banyumas regency. This research is an experimental with post test only design group methods. Data in this study is quantitative data with questionnaires as research instruments. Result of this research showed that the satisfaction's level of patients with usual care counseling and Al-Quran based pharmacy counseling had CSI values of 73.55% and 72.63%, respectively. There is no difference of level of satisfaction between the two groups. Both of pharmacy counseling methods can be applied in pharmacy services to improve patients's satisfaction.

IMPACT OF DRUG INFORMATION SERVICES ON OUTCOMES AND QUALITY OF LIFE OF HYPERTENSIVE PATIENTS

BACKGROUND: Hypertension is an important public health challenge associated with morbidity and mortality, and amenable to control through both non-pharmacological lifestyle factors and pharmacological treatments. In Nigeria, little is known as regards treatment outcomes, patient medication experience and satisfaction derived by patients from pharmaceutical services offered on antihypertensive medications. This study aimed investigating how pharmacist-provided drug information services to healthcare practitioners and patients affect the outcomes and quality of life in the management of hypertension. METHOD: A cross-sectional survey of 100 hypertensive patients and pharmacists in 20 community and hospital settings was conducted using self-structured questionnaires, to assess the quality of drug information and pharmaceutical care rendered to patients and to evaluate patients’ medication adherence and quality of life. A retrospective study was also carried out through review of patients’ case notes for measurement of treatment outcomes and adherence. RESULTS: 70% of the facilities in the hospital and community settings did not have a drug information center. Pharmacists provided 40% of required drug information to hypertensive patients. It was important for 87% of the patients to have their questions on drug therapy answered, but only 37% had their questions answered by the pharmacist, which influenced the drug taking behavior of 50%. 40% of the patients had low adherence, 50% had medium adherence and 10% had high adherence. 4.4% of the patients had well-controlled hypertension. The physical health measure for the patients was 69.04% (±7.84) and the mental health measure was 67.38% (±3.60). CONCLUSION: Adequate information on drugs and drug therapy, as well as proper counseling provided by pharmacists to hypertensive patients would greatly improve patients’ attitude towards medication use and adherence, and accordingly improve their clinical and humanistic outcomes, stimulating them to have a positive physical and mental state of health.

The Impact of Counseling on Drug Adherence in Hypertensive Patients at Tertiary Care Hospital in Larkana, Sindh

This study aimed to monitor the impact of counseling on adherence in hypertensive patients. Patient adherence was measured using the Hill-Bone medication adherence scale; non-adherent participants were counseled regarding antihypertensive medicines. The counseling impact was measured among enrolled participants. Most participants were male (n = 135), and only five were transgender. 52% of patients were from rural areas, and 48% were from urban areas. The 70 participants were intermediate and passed only 23. 38% of study participants were unemployed, while 26% were government employees. The 116 patients were 61-70 years old, while only 29 participants were 71-80. 33% of participants were prescribed 5 drugs, whereas no reported patient was prescribed one drug. Extremely low adherence was observed before counseling, whereas excellent results were achieved after patient counseling regarding antihypertensive medicines. To date, no study exists to measure adherence to antihypertensive medications. This study will help the doctors to counsel the patients so that patients will take the medications according to the prescription. The adherence rate of hypertensive patients can be improved if the patients are properly counseled regarding antihypertensive medicines.

Impact of pharmacist's intervention on disease related knowledge, medication adherence, HRQoL and control of blood pressure among hypertensive patients

The present study was aimed to evaluate the effect of educational intervention provided to the patients of hypertension through pharmacist with the goal to improve knowledge about hypertension, adherence to prescribed medicines, blood pressure control and HRQoL(Health Related Quality of Life).A total of 384 patients were assigned randomly into 2 groups including intervention and control groups each having 192 patients. Urdu versions of knowledge questionnaire regarding hypertension, medication adherence scale (MMAS-U) by Morisky and EuroQol scale (EQ-5D) were utilized. Each patient's blood pressure was measured. After educational intervention, an increase was found in mean knowledge score about hypertension (18.18 ± 4.00), adherence score (5.89 ± 1.90), HRQoL score (0.73 ± 0.12) and Visual Analogue Scale (VAS) score (69.39 ± 5.90) among the IG.
