Diabetic Foot Ulcer (DFU) is one of the complications that cause big problems in diabetics. One of the management of DFU management is by utilizing hyperbaric oxygen therapy (HBOT) which is an additional therapy that has been proven to be used in the treatment of DFU. However, the results of its benefits and use are still being debated, so the purpose of this review is to evaluate the effectiveness of HBOT on healing diabetic foot wounds. The method of this research is to use a literature review design. By using three international databases, namely PubMed, Science Direct, and Ebscohost, six articles were included. Diabetic foot ulcer (DFU) prevalence tends to increase every year. Within 30 seconds it is predicted that one leg will be amputated due to DFU. New blood formation triggered by adjuvant extracorporeal shock wave therapy (ESWT) on increasing VEGF level is expected to promote DFU healing. This study was aimed to prove whether ESWT as an adjuvant therapy could stimulate DFU healing by increasing VEGF serum level. This was a quasi-experimental study using two groups, the ESWT and the control groups. Two repeated measurements of VEGF levels and PEDIS scores were performed. The ESWT group (17 patients) received the adjuvant ESWT and the control group (7 patients) received conventional wound care therapy. Both groups received 8-time treatment for 4 weeks. Measurements of VEGF levels and PEDIS scores were performed at baseline and after 4 weeks. The results showed that the PEDIS score of the ESWT group decreased, albeit, not in the control group. The VEGF level of the ESWT group significantly increased (mean rank=13.24) compared to the control group (mean rank=7.33). The Wilcoxon signed-ranks test indicated that the PEDIS score in ESWT group had a significant decrease compared to the control group (mean rank=7.50), Z=-3.372, p=<.001. The Pearson correlation test showed a significant relationship between VEGF value and the PEDIS score reduction (81.4%). In conclusion, the adjuvant ESWT therapy could promote DFU healing (reduced PEDIS score) and increase VEGF levels in DFU patients.

Diabetic foot ulcer (DFU) is still a health problem related to the extent amount of time in wound healing process hence increasing the treatment cost. Given any methods that are not satisfying, encourage the search of other methods that would increase the wound healing rate, in which hyperbaric oxygen therapy (HBOT) is amongst them. The DFU patient that receives HBOT shows a significant increase in angiogenesis process markedly by faster epithelialization and granulation process. This study was aimed to prove that HBOT could accelerate the wound healing process among DFU patients measured by wound size and depth of PEDIS score. This was a randomized controlled trial study, conducted at Surgery Department of Prof. Dr. R. D. Kandou General Hospital from September 2018 to March 2019. There were 20 DFU patients divided into two groups, each of 10 patients. The control group received a holistic treatment, meanwhile the HBOT group received a holistic treatment added with the inclusion of HBOT. PEDIS score assessment of DFU in HBOT group was measured before and after the third HBOT session, meanwhile the control group was measured on the first and the third day. The results showed that the difference in PEDIS score value was markedly found in HBOT group compared to the control group (2 vs. 0, P=0.001). Conclusion: HBOT could enhance the wound healing process in DFU patients based on the declined PEDIS score.

Diabetic Foot Ulcer yaitu suatu kondisi kronis yang berkaitan dengan diabetes mellitus dan memengaruhi bagian bawah tubuh, umumnya manifestasi berupa kerusakan pada jaringan yang berhubungan dengan gangguan neurologis dan penyakit pembuluh darah perifer. Hal ini disebabkan oleh berbagai faktor (Kusumaningrum et al., 2020). Masalah utama dalam perawatan pasien Ny. S dan Ny. N ialah gangguan integritas kulit yang dapat dilihat melalui data subjektif dan objektif, termasuk gejala dan tanda utama seperti adanya luka diabetic foot ulcer. Kedua pasien ini menjalani terapi tambahan, yakni terapi ozone bagging sebagai bagian dari perawatan luka mereka. Terapi ozone bagging telah terbukti efektif dalam mendukung proses penyembuhan luka, berkurangnya risiko infeksi, dan menghilangkan bau pada luka. Terapi ozon dilakukan setiap pertemuan perawatan luka, dengan frekuensi tiga kali seminggu, dan durasi 15 menit per sesi untuk Ny. S dan Ny. N. Hal itu bisa kita lihat pada penurunan skor luka memakai winner scale score serta penurunan parameter bau odor menggunakan odor assesment tools.

Diabetes mellitus is a lifelong condition which is one of the leading causes of death in the world. Approximately 15% of patients with diabetes mellitus will suffer from diabetic foot ulcers. The incidence of lower extremity amputation associated with diabetic foot ulcers was significantly inversely correlated with the provision of foot care services. The purpose of this study was to determine the relationship between foot care and the occurrence of foot ulcers in patients with Type 2 Diabetes Mellitus based on literature studies in the last 5 years. Literature review is carried out based on issues, methodologies, similarities and further research proposals. Of the 11 articles, it shows that one researcher conducted a design using case control, one researcher used the cohort study method, while three researchers used a quasi experiment (pre-post test with control design), one researcher used a questionnaire design and five researchers used a cross sectional study (documentation studies). The population is all patients with diabetes mellitus. Based on 11 studies, it was found that there was a significant relationship between foot care and the occurrence of foot ulcers in patients with Type 2 Diabetes Mellitus. people with type 2 DM. Given the high prevalence of UKD in patients with type 2 diabetes, it is recommended that they perform routine foot care.

A diabetic foot ulcer is a common and fearful chronic complication of diabetes mellitus often resulting in amputation, and even death. A diabetic foot ulcer can be prevented by early screening and education in high risk individuals, and the management of underlying conditions such as neuropathy, peripheral arterial disease, and deformity. The prevalence of diabetic foot ulcer patients is 4-10% of the general population, with a higher prevalence in elderly people. Around 14-24 % of diabetic foot ulcer patients need amputations with a recurrence rate of 50% after three years. The main pathogenesis of diabetic foot ulcer is neuropathy and peripheral arterial disease (PAD). PAD contributes to diabetic foot ulcers in 50% of cases; however, it rarely stands alone. Other factors such as smoking, hypertension, and hyperlipidemia may contribute, too. In addition, PAD reduces the access of oxygen and antibiotics to the ulcers. Management of diabetic foot ulcers includes treatment of ischemia by promoting tissue perfusion, debridement for removing necrotic tissues, wound treatment for creating moist wound healing, off-loading the affected foot, surgery intervention, management of the co-morbidities and infections, and prevention of wound recurrences. Other adjuvant modalities include hyperbaric oxygen treatment, GCSF, growth factors, and bioengineered tissues.

Ulkus kaki diabetikum (UKD), salah satu komplikasi terberat dari diabetes melitus (DM) dengan angka amputasi dan mortalitas tinggi. Penyembuhan luka tidak hanya mengandalkan perfusi jaringan, tetapi juga dari perspektif imunologi. Beberapa studi telah menunjukkan asosiasi vitamin D dan UKD serta peran vitamin D dalam menurunkan inflamasi. Studi ini bertujuan untuk menelaah bukti terkait efektivitas suplementasi vitamin D per oral dalam penyembuhan luka pasien UKD. Penelusuran literatur menggunakan 4 database: PubMed, Scopus, Cochrane, and EBSCO Host. Dari 645 studi yang teridentifikasi pada awal penelusuran, 3 artikel dipilih untuk telaah kritis. Studi Kamble menunjukkan pengurangan luas permukaan luka lebih signifikan di kelompok vitamin D (8,06±6,82 cm2, p=0,02) dibandingkan di kelompok kontrol (3,76±1,73 cm2, P=0,28).
