Challenges of Clinical Nursing Training Through Internship Approach: A Qualitative Study

Background: Nursing education programs are designed to decrease the gap between theory and practice. It is believed that internship programs can decrease this gap. An internship program has newly been implemented in Kermanshah Nursing School. However, the efficiency of this program has not been assessed. Therefore, the aim of the present study was to explore the challenges related to the internship education of nursing students. Methods: The study was conducted as qualitative research. The participants were eight nursing students who had at least one semester of experience in internship. They were selected through purposive sampling. Data gathering was done through semi-structured interviews, which were recorded and analyzed through a three-stage content analysis approach. For data management, MAXQDA-10 was used. Results: Out of the eight participants, three were male, and all were in the final year of their education. In total, 423 primary codes emerged after analyses, which were placed in seven categories, including: education before internship, lack of support, planning difficulties, interaction with staff, invisible evaluation, welfare defects, and professional identity. Conclusion: The study revealed the challenges of the internship program for nursing students in Iran. Some of the problems were related to educational background, others were associated with the shortcomings of the cultural aspects of the context. Therefore, it is recommended to optimize the clinical training of nursing students in the internship program by addressing these challenges.

Background: An internship project can be considered as a useful step for nursing interns in the transition period and bridge the theory-practice gap in nursing. Since the acquisition of professional competence and skills by students is essential, the present study attempted to provide a deep understanding of nursing students’ experiences. Methods: This study was conducted using a qualitative content analysis approach in the School of Nursing of Kerman University of Medical Sciences in 2020. The participants in the research sample were senior undergraduate nursing students who were selected using purposive sampling. The data were collected through semi-structured interviews with the participants and each interview lasted 60 to 90 minutes. A total of 27 interviews were conducted with 25 students. Background: On completing the graduate study, nurses require to be posted for a one-year internship program (IP). In other words, it is a form of 'transition or a change in a role.' As health care professionals, role transition is an inevitable, constant and dynamic process. The journey from student to a registered nurse is exciting as well as complex phenomena. The study was conducted to 'explore the views of nurse internee, who have completed their one year internship period, about their experiences related to that important phase of transition. Methodology: A qualitative descriptive study with purposive and convenience sampling method was conducted. Data was collected through interviews and analyzed through a method proposed by Colorafi KJ & Evans B (2016). Results: Eight interviews were conducted. The main theme identified was "Internship as a professional endeavor". Four subthemes as ambiguity in understanding objectives of the internship, administrative and communication problems, supportive environment and ethical practice environment were also identified. Conclusion: It was concluded that the internship period is a very essential component for achieving professional growth and skill acquisition, but it was also stressed that it can only be made effective, if objectives of internship be made clear to interns and communication and administrative problems solved on priority basis as to create an ethically sound and professionally favourable conditions for interns.
Perceived Level of Preparation for Nursing Internship

Background: Internship programs have increased in number and popularity in the past decade. It was designed to assist in the transition period of nursing graduates and providing the necessary specialized training to work in various specialty. Nursing programs assure the basics of a comprehensive knowledge that support novice students to get ready to practice in different workplace. Purpose: The study aimed to identify perceived level of preparation for nursing internship. Method: A descriptive exploratory research design was employed with the sample of 121 nurse interns and Clinical preparation requirements Electronic survey utilized for data collection. Result: Majority of the respondents gave their highest percentage for educational preparation requirements with Teaching & Information Giving, Psychomotor Skills and Communications Skills (79.70%, 78.82% and 76.92% respectively). Also, regarding nursing process the results revealed that the respondents indicated their highest percentage with evaluation and intervention steps (76.03 and 75.7% respectively) Conclusion: The respondents perceived that all educational preparation requirement described in the current study were essential/very important for their preparation for practice during internship training and for developing their clinical competencies.

Background: The clinical learning environment is an interactive network of forces within the clinical setting that influences learning outcomes. It includes everything that surrounds students and affects their professional development in the clinical setting. Aim: was to determine internship nursing students' opinions of factors affecting their clinical learning environment in Assiut University Hospitals. Study design: A descriptive design was used. Setting: Assiut University Hospitals-Assiut Governorate-Egypt. Subjects: The study sample consisted of 200 nursing internship students. A self-administered questionnaire to gather data about personal characteristics and clinical learning environment. Results: Displayed that the high mean scores and standard deviation of the internship nursing students opinions involved instructor, learning objective, and clinical environment factors respectively. In addition, there were negative correlations in scores of internship nursing students' opinions of their clinical learning environment (learning objectives, instructor, how to deal with students in clinical settings & clinical environment) and their residence with statistically significant differences respectively except for supervision and evaluation factor. Conclusions: The most influential factors affecting the students in the area of clinical practice are the instructor, learning objectives, and clinical environment. Recommendations: Developing "Clinical Practice Guide" based on the job description for internship nursing students training areas.

Background: Most of faculty members in nursing in Indonesia have no experience working as health care provider and majority of them did not provide nursing servicesfor a long time. Background: Nursing process is a deliberate problem solving approach for meeting client's health care needs has been important to nursing practice for a long time. Aim: To evaluate the effect of applying an educational Program regarding nursing process on Acquiring Advanced skills among internship nurse's students. Subject and methods :Quasi-experimental research design carried out on 69 purposive sample of internship student nurses in 2017-2018 internship year collected by using two validated reliable adopted tool from Qanowe , 2004 , the first tool contains sociodemographic data sheet related to internship students age , gender , department , hospitals , the second tool related to student's assessment skills sheet it contains 12 parts divided into 4 main parts related to personal & professional qualifications followed by 6 parted related to nursing process and 2 parts related to personal skills and abilities with totally 83 items. Results reflected that there are a statistically level of knowledge among the internship nursing students regarding steps of nursing processes after the educational program implementation compared with unsatisfactory level before educational program implementation. Also, there is an improvement of internship nursing students for nursing processes post of educational program implementation whereas the mean of total knowledge scores level before the program compared to after program implementation with a highly statistically significant difference. There was a strong positive correlation found among internship student nurses between leadership and management skills, learning and education , communication , security and safety, other skills and different steps of nursing processes with a highly statistically significant difference at post-implementation of educational program compared to pre-program implementation. Conclusion & recommendations: Based on study results the implementation of an educational program for internship nursing students on nursing process and its application benefits showed a remarkable increase and improvement of the internship students nurse 's satisfactory level of professionalism and personal qualifications and acquiring the ultimate positive and noticeable improvement in the nurse's advanced acquired skills towards Profession and personal attitude , learning , management and leadership , communication , security and safety , and other professional skills as computer using , critical thinking , problem solving .There are an obvious needs for routinely refreshment program on nursing processes offered on simple media to internship nursing students to minimize work hazards. Efforts should be carried out to design and implement interventions that suit the hospital rules and policies regarding application of nursing processes.

Back ground: Orientation programs are known to be a key element for the successful career of newly graduated nurses. An effective orientation program can offer the novel nurse with significant confidence in her clinical skills and decision-making, which results in job satisfaction. Result: All the participants in this study were females in age range from 22-26 yrs, with a mean age of 23.5 yrs. Comparison of pre and post test score knowledge of Nurse Interns has been done by using paired t test. The result stated that the difference in pre and post test is considered to be extremely statistically significant (p<0.0001) and programme was effective. Conclusion: The orientation programme is an important aspect in the transition from student to the nurse which gives them confidence, job satisfaction, and competence. Every institution is responsible for guiding the new nurse to provide such opportunities for learning.
[Communicative reactions of nursing students regarding their first curricular internship period]

The objective of the present study is to describe and analyze the reactions of nursing students, presented during their first curricular internship. It is a cross-section field study, with data analyzed according to the method of content analysis. The results describe the training field, the relationship with the nursing team, the patients and the professor in the period of training. In the conclusions, we can affirm that the stress and the contradictions experienced in the first period of internship are part of the student's growth and education. The professor is responsible for determining the type of interaction that the students will experience. He/She needs to be capable of listening, encouraging and for his/her own qualification, in order to support the students with the early experiences of the practice, so that the first internship a becomes motivating factor for the students.
