Influence of ICG-technology on the results of surgical treatment of early forms of breast cancer

Breast cancer (BC) is one of the most common causes of death in women up to 50 years old. For today the choice of an adequate methods of surgical intervention and the need for an adequate surgical adjuvant therapy, quality of life of patients after surgeryis are important. The aim of our study was to select the volume of surgery for breast cancer using the technique of intraoperative identification of the sentinel lymph node (intraoperative ICG technology) and to evaluate the results of its urgent histological examination. It was shown that the most common method of surgery in patients who underwent intraoperative ICG technology was quadrantectomy, which was performed in 27 cases (54%), rarely subcutaneous mastectomy was performed (30%) and radical mastectomy (by Madden) – 16%. Diagnostic Accuracy of Mammography and Mammoscintigraphy with 99M TC-Mibi in Detection of Minimal Breast Cancer

In 437 primary patients with breast cancer (BC) we compared diagnostic accuracy of mammography (MG) and mammoscintigraphy (MCG) with 99mTc-MIBI. All women, included in the study, had lesions suspicious for BC. In 108 suspicious lesions were less than 10 mm in diameter. On histology 33 (30,5%) patients had invasive BC, 20 (18,5%) - noninvasive BC, 6 (5,6%) - lobular carcinoma, 2 (1,9%) - mucinous BC, 8 (7,4%) - fibroadenoma and in remained 39 cases (36,1%) - localised fibrosis. In the group of women with lesions below 10 mm sensitivity, specificity and accuracy of MG in diagnosis of BC were 72%, 74%, 73%, for MCG - 62%, 51%, 57%. Combination of MG and MCG helped to increase sensitivity to 93,4%. In 53 women with dense breasts sensitivity, specificity and accuracy of MG were 40%, 71,4%, 64%; for mCg two times higher (82%) with specificity of 55% and 60% accuracy. Combination of MG and MCG allowed increasing the sensitivity of diagnosis of minimal BC 2 times more (up to 91%).

Grayscale Color Mapping with the Mathematical Analysis of an Ultrasound Image in the Differential Diagnosis of Cystic and Solid Breast Masses

Objective. Atypical breast cysts are often quite a serious problem in noninvasive ultrasound differential diagnosis. To develop a system for automated analysis of grayscale ultrasound images, which on the principles of mathematical processing would make it possible to increase the specificity of diagnosis in this situation.Material and methods. The authors developed the CystChecker 1.0 software package. To test this system, they used a set of 217 ultrasound images: 107 cystic (including 53 atypical lesions that were hardly differentially diagnosed by standard methods) and 110 solid (both benign and malignant) breast masses. Статья посвящена поиску ответа на вопрос: что означает когнитивный подход к жанру? Автор полагает, что цель когнитивного изучения жанра состоит в построении его ментальной модели, отражающей репрезентацию жанра в сознании писателя и читателя. Для решения этой задачи необходимо соотнести жанр с различными форматами знания-не только концептуально-простыми (концепт), но и концептуально-сложными (фрейм, сценарий, матрица и др.). Обзор работ зарубежных и отечественных исследователей позволяет определить доминанты когнитивного подхода к жанру: • рассмотрение жанровой формы как текучей, нестабильной структуры; • выявление жанровых прототипов; • внимание к жанровой динамике, гибридным образованиям. Перспективным направлением когнитивных исследований автор считает установление корреляций между традиционными понятиями жанрологии (память жанра, жанровый архетип, жанровый канон) и терминами когнитивной поэтики. The article is devoted to the search for the answer to the question what the cognitive approach to the genre means. The author believes that the goal of the cognitive studies of the genre is to build its mental model, reflecting its representation in the minds of the reader and writer. To solve this problem, it is necessary to correlate genres with different formats of knowledge, not only conceptually simple (such as concept), but also conceptually-complex ones (such as frame, script, matrix, etc.). The review of the works of foreign and Russian researchers makes it possible to determine the dominants of the cognitive approach to the genre. They are: • consideration of the genre form as a fluid, unstable structure; • identification of the genre prototypes; • attention to genre dynamics and hybrid formations. The author assumes that a perspective direction of cognitive studies is to establish the correlations between the traditional concepts of genre (genre memory, genre archetype, and genre canon) and the terms of cognitive poetics.

Three-Dimensional Planning of Brachytherapy for Locally Advanced Cervical Cancer by CT/Mri Images

We present our results of 3D CT/MRI brachytherapy (BT) planning in 115 patients with locally advanced cervical cancer T2b-3bN0-1M0. The aim of this study was to assess the differences in the visualization of tumor target volumes and risk organs during the 3D CT/MRI BT. The results of the study revealed that the use of MRI imaging for dosimetric planning of dose distribution for a given volume of a cervical tumor target was the best method of visualization of the soft tissue component of the tumor process in comparison with CT images, it allowed to differentially visualize the cervix and uterine body, directly the tumor volume. Mean D90 HR-CTV for MRI was 32.9 cm3 versus 45.9 cm3 for CT at the time of first BT, p = 0.0002, which is important for local control of the tumor process. Evaluation changes in indicators of oncological service in breast cancer in Kazakhstan

According to estimates from the International Agency for Research on Cancer (IARC) by 2040 the global burden is expected to grow to 3 million new breast cancer cases and 1 million cancer deaths of this cause.
