Neuropatía periférica tras el parto: Implicaciones de la anestesia epidural (original) (raw)
Revista de la Sociedad Española del Dolor
Postpartum neurological damage occurs in the obstetric practice with an incidence between 0.08-0.92%. Neurological damage secondary to epidural technique is about 2:10,000. In last years, epidural anesthesia has become a generalized practice in obstetrics. Because of it, sometimes, the postpartum neurological damage can be attributed to epidural analgesia, forgetting other mechanisms. We present a 21 years old patient, 139 cm height, who 24 hours after epidural labor analgesia suffered a "drop foot", whose evolution was toward a complete resolution. Drop foot can be consequence of peroneal nerve, sciatic or lumbar or radicular damage. Clinical evolution, and complementary studies, such as RNM or EMG are useful for differential diagnosis. It looks necessary to study peripartum peripheric neuropathies to discard its possible relation to anesthetic technique. These lesions should be minimized by our side with adequate punction level choose, avoiding direct local anesthetic injection, especially when a paresthesias occurs, and with a correct risk factors evaluation in the obstetric patient. Once the lesion has occurred, it is important a close follow up until the complete resolution. © Sociedad Española del Dolor. Published by SED.

Influencia de la analgesia epidural en el parto
2010
IntroductionEpidural analgesia (EA) is the most effective procedure for pain relief during labor but can cause adverse effects. The aim of this study was to analyze the influence of EA on the rate of instrumental deliveries, cesarean sections and intrapartum fever.
Son un grupo diverso de trastornos que afectan a los nervios periféricos. Tiene una prevalencia del 2% en la población general y aumenta a 8% en los mayores de 55 años.
Fallo de la anestesia epidural en mujeres de parto tras la indicación de cesárea

Objective: To demonstrate the incidence and causes responsible for the failure of epidural analgesia for urgent caesarean delivery when indicated. Method: We conducted a prospective observational study collecting data from emergency cesarean section during 19 months between 2010 and 2011 in a university hospital. There were performed 4,120 deliveries including 542 caesarean 502 of them were urgent and semi-urgent and 40 scheduled, these latter have been excluded from the study due to having spinal anesthesia (AR) directly applied. In the first group there were 12 cases of failure (EA) and not detected in 490 (failure rate 2.44%) . The control group was a random sample of 60 patients, extracted to obtain sufficient statistical power, they were obtained from the control group of 490 cases, in order to compare them with the group of the 12 cases where the failure was observed. Results: the variables observed that may be related to the failure are gestational age in weeks, technical dif...
Objetive: Recognize the major disabling aspects that are part of the environment in the newborn intensive care unit; and establish the principles of posture in maintaining psychomotor development during hospital stay and highlight the elements of communication and behavior of children in the NICU. Methods: A qualitative study based on the Grounded Theory method by which the clinical experiences of the author are described and contrasted with the bibliographic review has been performed. Results: The maintenance of posture is a strategy to allow psychomotor adequacy, whose purpose is to simulate the stay in the intrauterine environment. It is important to note irreversible repercussions of the “oversight” and “inattention” to the appropriate position and en-vironmental management - setting newborn, whose disabling neurological sequelae not only the child but to the family. Keywords: Posture, Environment, Psychomotor, Sinactiva.
