Effects of Three Calcium Silicate Cements on Inflammatory Response and Mineralization-Inducing Potentials in a Dog Pulpotomy Model

This beagle pulpotomy study compared the inflammatory response and mineralization-inducing potential of three calcium silicate cements: ProRoot mineral trioxide aggregate (MTA) (Dentsply, Tulsa, OK, USA), OrthoMTA (BioMTA, Seoul, Korea), and Endocem MTA (Maruchi, Wonju, Korea). Exposed pulp tissues were capped with ProRoot MTA, OrthoMTA, or Endocem MTA. After 8 weeks, we extracted the teeth, then performed hematoxylin-eosin and immunohistochemical staining with osteocalcin and dentin sialoprotein. Histological evaluation comprised a scoring system with eight broad categories and analysis of calcific barrier areas. We evaluated 44 teeth capped with ProRoot MTA ( = 15), OrthoMTA ( = 18), or Endocem MTA ( = 11). Most ProRoot MTA specimens formed continuous calcific barriers; these pulps contained inflammation-free palisading patterns in the odontoblastic layer. Background : Perforation in normal pulp could be happened anytime. In sterile condition, direct pulp capping was the right therapy to maintain the vitality and pulp function. Histological evaluation of human pulp capped with light-cured calcium based cements: a randomized controlled clinical trial

Background: Calcium hydroxide has traditionally been used as the pulp capping material for pulpal exposures in permanent teeth. The tunnel defects in the barrier and the tendency for dissolution, however, fails to provide permanent protection to the pulp. Light curable resin based cements have been introduced to enable a better marginal seal and lesser dissolution. The purpose of this study was to compare and evaluate the response of human pulp following direct pulp capping with the new resin based Calcium silicate (TheraCal LC) and Calcium hydroxide with hydroxyapatite (Septocal LC) cements compared with calcium hydroxide (Dycal). Methods: 72 intact human premolars scheduled for orthodontic extractions were exposed to direct pulp capping procedures using three different pulp capping agents. Teeth were randomly divided into 3 groups, Group A: Dycal, Group B: TheraCal LC, Group C: Septocal LC. The teeth were extracted at the end of 15 and 40 days' and were evaluated histologically. They were scored for reparative dentin formation and inflammatory response. Inferential statistics was done using Chi square test. Results: Majority of the specimens in all three groups at the end of 15 days' showed partial to lateral deposition of hard tissue. There was continuous deposition of hard tissue and severe inflammatory response at the end of 40 days' in Dycal. There was partial deposition of hard tissue and reduced inflammatory response at the end of 40 days' in TheraCal LC and Septocal LC. However, the results were not statistically significant between the three groups at two different time periods. Conclusions: Light cured, Calcium silicate (TheraCal LC) and Calcium hydroxide with hydroxyapatite (Septocal LC) cements were as effective as calcium hydroxide (Dycal) in inducing the formation of reparative dentin and evoking inflammatory response.

Evaluation of mineral trioxide aggregate and calcium hydroxide cement as pulp-capping agents in human teeth

This study evaluated the histomorphologic response of human dental pulps capped with mineral trioxide aggregate (MTA) and Ca(OH) 2 cement (CH). Pulp exposures were performed on the occlusal floor of 40 human permanent premolars. After that, the pulp was capped either with CH or MTA and restored with composite resin. After 30 and 60 days, teeth were extracted and processed for histologic exam and categorized in a histologic score system. The data were subjected to Kruskal-Wallis and Conover tests (␣ ϭ .05). All groups performed well in terms of hard tissue bridge formation, inflammatory response, and other pulpal findings. However, a lower response of CH30 was observed for the dentin bridge formation, when compared with MTA30 and MTA60 groups. Although the pulp healing with calcium hydroxide was slower than that of MTA, both materials were successful for pulp capping in human teeth. (J Endod 2008;34:1-6)

A comparative evaluation of ProRoot mineral trioxide aggregate and Portland cement as a pulpotomy medicament

Introduction: Recently, some studies have compared mineral trioxide aggregate (MTA) with portland cement (PC), concluding that the principle ingredients of PC are similar to those of MTA. The purpose of the present study was to evaluate the biocompatibility of PC as a pulpotomy medicament. Materials and Methods: Thirty premolars that scheduled for extraction for therapeutic reasons were randomly assigned to two experimental groups: ProRoot MTA (PMTA) and PC. After isolation and pulp exposure, pulpotomy was carried out and pulps were dressed with PMTA and PC. After 6 months, the teeth were extracted and prepared for histological analysis based on Cox et al. criteria. The data were analyzed by Z-test of proportion with 1% of allowed error. Results: No statistically significant difference was found between the two groups with respect to inflammatory response, soft tissue organization, and dentine bridge formation (P > 0.05). Conclusions: PC was associated with similar favorable biological response to pulpotomy treatment as PMTA. The findings of this study support the idea that PC can be considered a cheaper substitute to MTA.

Evaluation of Modified Portland Cement versus Mineral Trioxide Aggregate as a pulpotomy agent in Primary Molars: Histological Study Research Article

Background: The aim of this study was to evaluate the histological response of dental pulp tissue in primary molars to Modified Portland Cement (MPC) mixed with Zirconium Oxide and Calcium Chloride in comparison with Mineral Trioxide Aggregate (MTA). Methods: The study was conducted on children between 7.5-9 years of age, pulpotomy was performed on 60 Primary Molars (to be extracted as a part of Orthodontic treatment). The molars were divided randomly into two groups, each of them contains 30 teeth. After pulpotomy was conducted, the root Pulp was covered with MTA or MPC and a layer of Glass Ionomer Cement (GIC) and the tooth were filled with amalgam eventually. The teeth were extracted after one week, one month, and two months respectively and histological samples were collected and evaluated Based on the Inflammation response, soft tissue organization, and Dentinal bridge formation. Results: Modified Portland Cement has showed no significant differences in terms of inflammation response, soft tissue organization, and Dentinal bridge formation in comparison with MTA in the observation period. Conclusion: MPC can be a replacement material of MTA in terms of primary molars pulpotomy.

Histologic Assessment of Human Pulp Response to Capping with Mineral Trioxide Aggregate and a Novel Endodontic Cement

Introduction: This study was conducted to compare human pulp response to mineral trioxide aggregate (MTA) and a novel endodontic cement (NEC) when used as pulp capping materials after a time period of 2 and 8 weeks. Methods: Thirty-two premolar teeth that were scheduled for extraction for orthodontic reasons were exposed and capped with either MTA or NEC. Half of the specimens underwent extraction and histologic analysis after 2 weeks, and the remaining half were assessed after 8 weeks. Each slide was graded histologically according to the morphology of the dentinal bridge, thickness of the dentinal bridge, presence of odontoblast cells, and inflammation of the pulp. Results: Both MTA and NEC showed significantly better pulp response after 8 weeks compared with 2 weeks, with a thicker and more tubular pattern of the dentinal bridge, less pulp inflammation, and a palisade pattern of odontoblast cells. Although MTA and NEC groups had no significant difference in each measure in both time intervals, NEC induced a thicker dentinal bridge with less pulp inflammation at both 2 weeks and 8 weeks, compared with MTA. Conclusions: MTA and NEC showed similar biocompatibility and favorable response in pulp capping treatment and inducing the formation of the dentinal bridge. (J Endod 2010;36: 1778-1781

Evaluation of Two Different Types of Mineral Trioxide Aggregate Cements as Direct Pulp Capping Agents in Human Teeth

Traumatic human dental injuries involving the pulp might necessitate direct capping procedures. This clinical study aimed to analyse the histological outcomes using two different direct capping materials. Twenty patients with bilateral premolars, scheduled for orthodontic extraction, were selected. The teeth were treated either using ProRoot MTA or RetroMTA. All patients were recalled after 30 and 60 days for teeth extraction. The histopathologically stained specimens were blindly evaluated using hard tissue bridge formation, inflammatory reaction and pulpal findings criteria. Data were evaluated statistically. Results: After 60 days, only the parameter for hard tissue bridge formation showed significant difference in the ProRoot MTA group (p = 0.010), while both direct capping materials performed similarly regarding inflammatory pulp reaction and pulpal findings.
