Subacute thyroiditis complicated by extra-thyroidal abscess: A very rare presentation of tuberculous thyroiditis

Tuberculosis of the thyroid gland is a very rare condition. We report a case of tuberculous thyroiditis in a young woman who presented with a right-side solitary thyroid nodule associated with features of subacute inflammation of a short duration. Diagnosis was confirmed by fine needle aspiration cytology (FNAC) with positive Ziehl Neelsen staining for acid-fast bacilli (AFB) and positive culture for Mycobacterium tuberculosis. The patient was treated medically with satisfactory resolution of the infection and regression of the nodule. Conclusion: FNAC provides a confident non-operative diagnosis of thyroid tuberculosis, obviating the need for unnecessary surgical removal of a thyroid nodule readily treated medically.

Tuberculous Osteomyelitis of Orbit- A Case Report

Background: Orbital tuberculosis is a rare manifestation of extrapulmonary tuberculosis. We report an interesting case of osteomyelitis of orbital bone in an Indian child. Case Report: A 9-year old girl presented with a gradually increasing painless swelling of left upper lid and blepharoptosis along with swelling of left side of forehead for last two months. The swelling was soft, fluctuating and non-tender. Diagnostic tap revealed thick pus which prompted us to do an incisional drainage of the abscess. The pus was sterile on microbiological examination. A diagnosis of osteomyelitis of orbit along with cold abscess of infratemporal fossa was made on the basis of clinical examination, positive Mantoux (MX) test, high erythrocyte sedimentation rate (ESR) and bony erosion of frontal and zygomatic bones in contrast enhanced computerized tomography (CECT). Patient showed marked improvement with antitubercular drug (ATD) but a sinus developed at the incision site. Six months later patient presented with occasional serous discharge from the sinus.The patient underwent excision of the sinus along with curettage and debridement of the left frontal bone and the tissue was sent for histopathological and microbiological examination. ATD was continued for 15 months. Her post-treatment three year follow up was without any recurrence. Conclusion: High index of suspicion should be kept for early diagnosis and timely treatment of the orbital tuberculosis. We want to report the case because of its rarity and clinical interest.

Abstract: The common midline neck masses include thyroglossal duct cyst, lipomas, thyroid tumor or cyst and rarely a midline lymph node, particularly the node just above the thyroid isthmus. Thyroid tuberculosis (TTB) is a rare cause of thyroid disease and therefore only occasionally included in the differential diagnosis. In this study we tried to explain the clinical and paraclinical manifestations of TTB with a case series of 26 patients with definite or suspicious diagnosis of TTB.

Tuberculosis of the Thyroid Gland: Review of the Literature

Objective Thyroid tuberculosis (TTB) is a very rare condition, even in countries with a high prevalence of tuberculosis. The aim of this article is to review and retrieve data about thyroid tuberculosis from the English-language literature in order to gain a better understanding of the clinical characteristics of TTB. Study Design We performed Medline, PubMed, and library searches using the key words “thyroid tuberculosis,” “throid disease,” “tuberculosis and thyroid.” Reference lists of the articles obtained and previous reviews were also examined. Results We retrieved 76 cases matching our selection criteria from the search. Review of the cases with TTB revealed a slight female preponderance. The patients reported in the literature ranged in age from 9 to 83 years, with a median age of 40 ± 16 years for men and 44 ± 17 years for women. In the articles surveyed, TTB presented with a broad spectrum of manifestations, ranging from an isolated nodule to thyrotoxicosis. It seems that diagnosis of thyroid tuberculosis has recently been increasing, perhaps because of the growing incidence of tuberculosis and the diagnostic use of fine-needle aspiration cytology in the diagnosis. Although, in the past, the diagnosis was genarally made by the examination of the specimens, at present, fine-needle aspiration cytology seems to be a useful method in diagnosis tuberculous thyroiditis.The role of surgery is limited after the diagnosis. The choice of treatment should be medical antituberculous therapy. Conclusiıons Preoperative diagnosis of thyroid tuberculosis is important because of the availability of medical treatment and the limited role of surgery. This condition should be kept in mind in evaluating patients with a thyroid nodule, in communities where the prevalence of tuberculosis is high.

Tuberculosis of the thyroid gland: two case reports

Tuberculosis of the thyroid gland is a rare entity even in countries like India where tuberculosis is endemic. The patients may present with thyroid swelling, inflammation and very rarely thyroid dysfunction. Caseous necrosis and epithelioid cell granulomas on fine-needle aspiration cytology and histopathological examination are diagnostic. We present two cases of thyroid gland tuberculosis. One patient had subclinical thyrotoxicosis with presentation mimicking acute bacterial thyroiditis. The other patient had a solitary thyroid nodule with normal thyroid function. Involvement of other organs was absent in both cases. Proper diagnosis may avoid unnecessary surgical interventions.

Methods: Design: Case Report Setting: Tertiary Government Hospital Patient: One Results: A 55-year-old farmer presented with an 8-month progressively enlarging anterior neck mass, and fine needle aspiration biopsy yielded grossly turbid straw-colored aspirate admixed with blood with microscopy showing scattered inflammatory cells and macrophages set against a colloid background. After total thyroidectomy, hispathology revealed parenchymal infiltration by multiple aggregates of plump spindled to epitheloid cells forming granulomas with interspersed multinucleated giant cells, central caseation necrosis and surrounding fibrosis with chronic inflammatory infiltrates. The nodal masses also showed prominent germinal centers with interspersed epitheloid cells and foamy macrophages. Fi...
