Integrating a pharmacist into the general practice environment: opinions of pharmacist’s, general practitioner’s, health care consumer’s, and practice manager’s

Background: Pharmacists are viewed as highly trained yet underutilised and there is growing support to extend the role of the pharmacist within the primary health care sector. The integration of a pharmacist into a general practice medical centre is not a new concept however is a novel approach in Australia and evidence supporting this role is currently limited. This study aimed to describe the opinions of local stakeholders in South-East Queensland on the integration of a pharmacist into the Australian general practice environment. Methods: A sample of general practitioners, health care consumers, pharmacists and practice managers in South-East Queensland were invited to participate in focus groups or semi-structured interviews. Seeding questions common to all sessions were used to facilitate discussion. Sessions were audio recorded and transcribed verbatim. Leximancer software was used to qualitatively analyse responses. Results: A total of 58 participants took part in five focus groups and eighteen semi-structured interviews. Concepts relating to six themes based on the seeding questions were identified. These included positively viewed roles such as medication reviews and prescribing, negatively viewed roles such as dispensing and diagnosing, barriers to pharmacist integration such as medical culture and remuneration, facilitators to pharmacist integration such as remuneration and training, benefits of integration such as access to the patient's medical file, and potential funding models. Conclusions: These findings and future research may aid the development of a new model of integrated primary health care services involving pharmacist practitioners.

US Pharmacists' Effect as Team Members on Patient Care

Background: One approach postulated to improve the provision of health care is effective utilization of team-based care including pharmacists. Objective: The objective of this study was to conduct a comprehensive systematic review with focused meta-analyses to examine the effects of pharmacist-provided direct patient care on therapeutic, safety, and humanistic outcomes. Methods: The following databases were searched from inception to January 2009: NLM PubMed; Ovid/MEDLINE; ABI/IN-FORM; Health Business Fulltext Elite; Academic Search Complete; International Pharmaceutical Abstracts; PsycINFO; Cochrane Database of Systematic Reviews; National Guideline Clearinghouse; Database of Abstracts of Reviews of Effects; ClinicalTrials.gov; LexisNexis Academic Universe; and Google Scholar. Studies selected included those reporting pharmacistprovided care, comparison groups, and patient-related outcomes. Of these, 56,573 citations were considered. Data were extracted by multidisciplinary study review teams. Variables examined included study characteristics, pharmacists' interventions/services, patient characteristics, and study outcomes. Data for meta-analyses were extracted from randomized controlled trials meeting meta-analysis criteria. Results: A total of 298 studies were included. Favorable results were found in therapeutic and safety outcomes, and meta-analyses conducted for hemoglobin A1c, LDL cholesterol, blood pressure, and adverse drug events were significant (P Ͻ 0.05), favoring pharmacists' direct patient care over comparative services. Results for humanistic outcomes were favorable with variability. Medication adherence, patient knowledge, and quality of life-general health meta-analyses were significant (P Ͻ 0.05), favoring pharmacists' direct patient care. Conclusions: Pharmacist-provided direct patient care has favorable effects across various patient outcomes, health care settings, and disease states. Incorporating pharmacists as health care team members in direct patient care is a viable solution to help improve US health care.

Improving Health in an Integrated Health Care System: Empowering Pharmacists to Function as Key Agents of Change

With passage of the Affordable Care Act (ACA) in 2010, the integrated health care model has become standard in the United States. Implementing integrated healthcare teams has been empirically shown to improve patient health outcomes and reduce health care costs system-wide. Pharmacists are positioned to play a pivotal role, especially as health educators, in this new model. However, several systemic and policy-related barriers may impede pharmacists from realizing their full potential in the new integrated healthcare world. This paper describes the evolving integrated health care system and pharmacists' potential role in it. Through the use of real-world case examples, we elucidate how promoting pharmacists' full participation is likely to positively affect patient health outcomes. We conclude with some policy considerations related to pharmacists' abilities to function as key agents of change within the integrated health care system.
