Bradykinin and angiotensin-converting enzyme in serum of patients with diabetic retinopathy and the prognosis of diabetic macular edema development (pilot study)

BACKGROUND. Diabetic macular edema (DME) is a microvascular complication of diabetic retinopathy. One of the key roles in the pathogenesis of DME may belong to the components of rennin-angiotensin and kallikrein-kinin systems: bradykinin (Bk) and angiotensin-converting enzyme (ACE).PURPOSE. To determine the Bk and ACE concentration and ACE activity in serum of patients with proliferative diabetic retinopathy (PDR) and to estimate the significance of these parameters for the early diagnostic and prognosis of DMO.MATERIALS AND METHODS. Serum was collected from the 2 groups of patients with II type diabetes. Group I (n=9) had DME, group II (n=27) had PDR without DME. Control group (n=14) consisted of adult volonteers without diabetes and ophthalmic diseases. Background. Diabetic retinopathy, one of the major complications of diabetes, is one of the leading causes of low vision and blindness. One of the main causes of decreased central vision in patients with diabetic retinopathy and type 2 diabetes mellitus is diabetic maculopathy (DMP). DMP with possible development of diabetic maculopathy edema can be detected even in patients with initial changes in the fundus. The purpose was to investigate the safety profile and effectiveness of various modern vitreoretinal interventions in the treatment of diabetic maculopathy. Materials and methods. Эпидемиология ахарный диабет (СД)-одна из наиболее значимых медицинских проблем современного мира. В структуре эндокринных заболеваний СД является самой распространенной патологией. Количество больных с каждым годом неуклонно возрастает, что связано как с более ранним выявлением заболевания, так и с общим увеличением средней продолжительности жизни. На сегодняшний день в мире насчитывается почти 400 млн больных СД, а к 2035 г. ожидается увеличение численности до 592 млн чело-век [1]. В Российской Федерации, по данным Государственного регистра СД на 2013 год [2], количество больных (зарегистрированных по обращаемости) превышает 3,7 млн человек, а реальная численность превышает 10 млн человек [1, 2]. Наиболее тяжелыми осложнениями СД являются диабетическая ретинопатия и диабетический макулярный отек (ДМО). По данным Wisconsin Epidemiological Study of Diabetic Retinopathy (WESDR) при длительности заболевания СД более 20 лет ретинопатия выявляется в 80-100% случаев, при этом ДМО развивается в 29% случаев [3].

The choice of treatment for macular edema following retinal vein occlusion

A clinical efficacy assessment was performed for the therapy of post-occlusive cystoid macular edema using intravitreal anti-VEGF-ranibizumab injections at short and remote terms from the disease onset. It has been shown that ranibizumab intravitreal injections in retinal vein occlusion allow to increase visual acuity and to reduce retinal edema, and thus rehabilitate patients in maximally short terms. Intravitreal injections of vascular endothelial growth factor inhibitors at short terms from the disease onset allow achieving stabilization of the pathological process with preservation or improvement of visual functions, while reducing the number of repeated intravitreal injections.

ERG assessment of the functional activity of the retina in following the surgical closure of idiopathic macular holes

Twenty patients with stage 3 or 4 idiopathic macular holes were evaluated with standard cone ERG, photopic flicker ERG at 8,3-30-Hz, to study the function of cone photoreceptors, bipolar cells, and Muller cells after the surgical correction of their macular hole. The correlations between the ERG parameters and the microperimetry and optical coherent tomography data were evaluated. IMH has been associated with the general reduction in the function of the cone photoreceptors and bipolar cells and with the sharp increase in the activity of Muller cells and their functional relationships with some bipolar cells. The nature of the changes in photopic standard and flicker ERGs demonstrated the dependence of the central retinal function on the dynamics of cone bipolar cells’ recovery. Hypotensive therapy effectiveness evaluation in primary open-angle glaucoma evidenced by pattern electroretinogram data

Purpose. To evaluate the change in the functional activity of retinal ganglion cells (RGCs) in response to intraocular pressure (IOP) drop in patients with newly diagnosed early stage primary open-angle glaucoma (POAG) using a pattern electroretinogram (PERG). Material and methods. A prospective non-randomized cohort study included 12 patients (24 eyes) including 5 men, 7 women, aged 63.42 ± 2.96 years with newly diagnosed early stage POAG in at least one eye, who underwent PERG (Diopsys Nova, Diopsys, Inc.) in addition to an ophthalmologic examination. After being diagnosed with POAG, all patients were prescribed prostaglandin analogues as monotherapy. One and four months into the treatment, repeated examinations took place. Results. Хемиските изгореници претставуваат 7-10 % од повредите на очите. Повредите на окото со хемиски средства ги делиме на изгореници со алкалии и изгореници со киселини. Се јавуваат главно во индустријата, занаетчиството, домаќинствата, а инциденцијата на изгореници со гасена вар е многу честа кај градежните работници. Сепак тежината на изгореноста зависи од тоа која супстанција ја предизвикала, колку време супстанцијата имала контакт со окото и каков бил третманот на повредата.
