True posterior communicating artery (PCoA) aneurysms are a rare subset of intracranial aneurysms, accounting for approximately 1.3% of all such cases. This paper reviews the anatomy, epidemiology, and treatment options for true PCoA aneurysms, emphasizing the complications associated with their surgical management due to their unique anatomical position. It discusses the predominant presentation of these aneurysms with rupture, potential treatment challenges, and the importance of understanding their surgical anatomy for effective clipping and anatomical identification. The findings underscore the need for careful monitoring and tailored surgical approaches when addressing true PCoA aneurysms.
