Fetus in Fetu: A Rare Presentation in an Adult Female - Case Report

A case of fetus in fetu, a pathologic condition resulting from abnormal embryogenesis in a diamniotic, monochrionic pregnancy is reported. This condition usually presents in infancy and childhood. In this present case, a 30 year old patient misdiagnosed preoperatively as retroperitoneal dermoid is presented. This is the oldest patient reported with fetus in fetu in literature.

A 2-week-old girl was found to have an asymptomatic abdominal mass on routine examination. Surgical removal revealed the mass to have 4 limb buds situated relative to a palpable vertebral column. Karyotyping revealed the mass to be 46 XX. The mass was therefore diagnosed as a case of fetus-in-fetu. The literature on this rare entity is reviewed, and the diagnosis and pathogenesis of the disease are discussed.

Fetus- in- fetu (FIF), is a rare congenital anomaly that occurs due to malformed fetus growing inside the twin. It occurs due to the aberration in the diamniotic monochorionic monozygotic twinning with unequal division of inner cell mass of blastocyst leading to inclusion of small cell mass inside the developing sister fetus. The most common presentation will be with intra-abdominal mass located in the upper quadrant retroperitoneally. Complete excision with meticulous dissection is curative and allows confirmation of diagnosis.

Fetus in fetu is a rare developmental aberration, characterized by encasement of partially developed monozygotic, diamniotic, and monochorionic fetus into the normally developing host. A 4-month-old boy presented with abdominal mass. Radiological investigations gave the suspicion of fetus in fetu. At surgery a fetus enclosed in an amnion like membrane at upper retroperitoneal location was found and excised. The patient is doing well after the operation.

Fetus in Fetu (FIF) is a rare congenital anomaly in which a malformed parasitic fetus grows in the body of its twin. It is a rare pathological condition and about 200 cases have been reported in the literatures. Prevalence more in infant and children but may present in any age, the oldest reported case was 47 years old. Here we are reporting 2 cases of FIF, first one is 3 month 10 days old male child and second one is 7 month old female child. Our first case presented with an abdominal lump and the second case presented with huge abdominal distention with lump, respiratory distress and anaemia. After proper imaging studies elective surgery was performed and pathology confirmed the diagnosis in both cases. Here we discuss about FIF, difference between FIF and teratoma, chance of malignancy and post-operative follow-up.

Fetus in fetu (FIF) is a rare cause of abdominal mass in children. One of the malformed monozygotic diamniotic twins is located in the body of other twin. It is differentiated from teratoma by the presence of vertebral organization with limb buds and other organ systems. Diagnosis is based on radiologic findings. Surgical excision is the treatment of choice, leading to the complete removal of the mass. To our knowledge, less than 200 cases have been described in the literature. Herein, we report 2 cases of FIF, a newborn who was diagnosed antenatally and a three-and-half-year-old boy diagnosed with mediastinal FIF after admission for recurrent respiratory tract infections.

Fetus in fetu is a rare disorder. Its embryopathogensis and differentiation from teratoma has not been well established. It is a parasitic twin of a diamniotic monozygotic twin. Here we report, two cases of fetus in fetu with review of literature. In case report 1, a 2-year-old boy was referred for asymptomatic lump in abdomen since birth. X-ray showed the mass in the abdomen with some calcification and fluid inside. CT scan reported a heterogenous mass in the retroperitoneum with bony malformation. CT showed presence of three vertebrae in it. After surgically excising the mass and opening the sac it showed presence of trunk and two limbs with one of the limbs having a nail. Histopathology showed presence of GI tract. In case report 2, 4 month female was found to have lump in the abdomen by housemaid while bathing. X-ray showed a soft tissue shadow while ultrasonography revealed cystic mass arising from right kidney. CT suggested cystic mass with calcification not arising from kidney. During exploration whole mass was excised and there was frank fetus inside it. Histopathology confirmed presence of four vertebral bodies with germ layers. Although fetus in fetu is rare condition, correct diagnosis using imaging can be made before surgery. Complete excision is curative.

Fetus in fetu is a rare condition that has been defined as the presence of one of the twins in the body of the other. It is most frequently located in retroperitoneal area; however, it has been reported in other locations as well. This report presents two cases of "fetus in fetu" cases: one located in the retroperitoneal area and the other in the sacrococcygeal area.
