To analyse trends in the rates of consumption of benzodiazepine (BZD) anxiolytics, BZD hypnotics and non-BZD hypnotics and the association with contextual factors. Descriptive time series study. Units of analysis were monthly dose per inhabitant per day (DID) and dose per medical card per day(DCD) of benzodiazepine(BZD anxiolytics(BZD-A), BZD hypnotics(BZD-H) and non-BZD hypnotics(Non-BZD-H) between January 2006-December 2015. We analysed 6 primary healthcare districts(PHD) and used defined daily doses (DDDs) to calculate the monthly DIDs(overall and by ATC group). Trends and monthly percentage change (MPC) were analysed through joinpoint regression. The annual DID increased by 26% overall, the trend was different across ATC groups. Consumption in BZD-A and BZD-H increased (27.1%,61.9%), consumption in Non-BZD-H decreased by 35%. There was high variability in DCD across the PHD, with an overall increase of 10.2%(5.7%-22.9%). IRRATIONAL DISPENSING AND SELF MEDICATION OF BENZODIAZEPINES BY GENERAL POPULATION OF A BIG CITY
A systematic observational study is performed in general population of age group 18-60 years to collect enough data on the irrational dispensing pattern and self medication of benzodiazepines. There were two separate specially designed Performa's, one for the OPD pharmacy or dispensary in different communities of Karachi and one for the general population of variable demographic status duly filled by 190 out of 200 OPD pharmacies approached, (n = 190 with response rate 95 %) and 478 out of 508 people approached (n = 478 with response rate 94.09 %) In this study 478 individual participated. Mean age of population was 43.62 ± 17 years. 44.6 % were male (n = 213) and 55.4 % were female (n = 265) the common Benzodiazepines reported to be used by study population are Alprazolam 54 %, Bromazepam 40 %, Clorazepate, Lorazepam and Azolam 2 % each. The three primary reasons reported lead to the use of benzodiazepines by general population are depression 64 %, sleep disturbance 19 % and pain 17 %. There were 68 people (29 %) confessed that they are using it without any prescription from GP. By the Performa filled by OPD community pharmacies findings are benzodiazepines are the 6 th largest selling drug class. Alprazolam and Bromezepam both are the top most selling benzodiazepines members 88.52 % followed by Lorazepam 6.39 %, Clorazepate 0.09 % and others 5.0 %. Among 190 pharmacies visited, around 79 pharmacies (41.57 %) reported to dispense benzodiazepines without prescription. Significant variable with respect to rate of use of benzodiazepines in the community were educational status, socio-economical background, occupation, marital status etc with p < 0.000 when Pearson chi-square test were applied.

Background: To determine the trend in the consumption prevalence of benzodiazepines (BZDs) and drugs related to BZDs (Z-drugs) in the population of the Health Region of Lleida to explore patterns of use and characteristics associated between 2002 and 2015. Methods: Retrospective community cohort study carried out between 2002 and 2015 consisting of individuals from a Health Region of 358,157 inhabitants in 2015. The consumption of BZDs was evaluated using prescription billing data from the Public Health System . All types of BZDs and their analogues that have been approved by the drug agency were included. Trends regarding age and sex were investigated. Results: A total of 249,471 individuals generated 338,148 dispensations. Overall, 53% were women, and the mean age was 60 years. The prevalence of BZD use in 2015 was 14.2%—18.8% in women and 9.6% in men—and was 36% in those over 65 years. DRUG UTILIZATION EVALUATION OF BENZODIAZEPINES IN A TERTIARY CARE HOSPITAL

Objective: To assess the rationale use of benzodiazepines among various departments in a multi-speciality hospital. Methods: A prospective study was conducted with a sample size of 200 for a period of six months. Data was collected from patients based on inclusion and exclusion criteria. Naranjo Adverse Drug Reaction Probability Scale and Drug Interaction Probability Scale (DIPS) were used as a study tool to measure the causality of adverse drug reactions and drug interactions. Based on the dosage of various benzodiazepines DDD was calculated and compared with WHO Anatomical Therapeutic Chemical (ATC) classification Defined Daily Dose (DDD). Results: BZD's were mostly prescribed in males (74.5%) and married patients (86.5%) were more exposed to benzodiazepines compared to others. Lorazepam (70.1%) was found to be the most commonly used drug, mainly prescribed for sedation, followed by anxiety. DDD was calculated and majority of patients had DDD in accordance with WHO standard. Based on cost analysis, Clobazam was found to be the high cost and Lorazepam being the low-cost drug. The results of drug utilization evaluation of benzodiazepines study were compiled and reported to the respected department physician and their feedback was collected. Conclusion: The study showed a rational utilization of benzodiazepines and the negative outcomes of BZDs can be reduced by providing drugrelated information to the prescribers and consumers.
