Due to presence of Monosodium urate and Monohydrate crystals in body, certain chemical reaction occurs at joints or periarticular tissues in body characterized as Gout in modern science. Due to fault lifestyle including food habits, physical activity or work, prevalence of Gout is raising among population. Presence of deposition of uric acid and raise in level of uric acid is cardinal feature of Gout which is mostly managed by analgesic, glucocorticoids or other acid lowering therapies like xanthine oxidase inhibitor or uricosurics drugs. On correlating with the sign and symptoms of Gouty arthritis, Vata-Rakta is Vyadhi mentioned similarly in Ayurveda literature, in which Vitiated Vata and Rakta Dosha both localized in multiple joints of body. A female patient with raised uric acid and mild edematous swelling at joints of upper and lower limb was treated with Ayurveda oral medication for initial 15 days which showed a dramatic improvement in level of uric acid along with reduction of swelling on joints. Uric acid lowered from 7.8 mg/dl to 2.4mg/dl in 15 days with only conservative management. Relief in complaints like pain and burning sensation got also reduced. This case study provides us the guidelines to manage gouty arthritis as per Ayurveda Siddhant in effective and safer way

Background Vatarakta vis-à-vis gout is a common condition in both primary care and specialist practice. Gout is a systemic disease characterized by manifestations of chronic underlying hyperuricemia, resulting in the deposition of monosodium urate crystals in various tissues. Gout is the most common of the crystal induced arthritis due to disturbed uric acid metabolism and precipitation of urate crystals in extracellular space of joints, periarticular tissue, bones, and other organs. Even though Gout is generally grouped under Arthritis, it is a metabolic disorder of Uric Acid. Extensive description of Vatarakta in ayurvedic classics shows that it was prevalent widely in that era too. In present era changes in lifestyle & food habits leads to recognizable increase in incidence of Vatarakta in common people. Total 35 patients of Vatarakta (Gout), out of which 32 patients completed the course of treatment. Study was Open label, Interventional, Prospective and Single arm clinical trial for the duration of 42 days (6 weeks). Patients were given Kaishore Guggulu and Punarnavadi Guggulu 2 tablets (500 mg) each twice a day with lukewarm water. The required amount of medicines was supplied by the IMPCL (A Govt. of India Enterprise) Mohan ? Dist. Almora, Uttrakhand after ensuring safety and quality batch to batch. Patients were followed 2-week interval i.e. on 2nd,4th and 6thweek of treatment period. The effects of treatment on subjective and objective parameter evaluated. Results obtained shows encouraging results in the management of Vatarakta (Gout) with no side effects observed so far.

Hyperuricemia is considered to be one of the leading causes of joint stiffness and pain especially small joint most commonly affecting first metatarsophalangeal joint. This is an excess of uric acid in the blood. It is commonly associated with traditional risk factors such as dysglicemia, dyslipidemia, central obesity, gout (Nikras), gouty arthritis, tophi formation, abnormal blood pressure, etc. Concordantly, recent studies have revived the controversy over the role of circulating uric acid, hyperuricemia, and gout (Nikras) as an independent prognostic factor. In this regard, the role of Unani compound medicine including Habb-e-Asgandh, Habb-e-Suranjan, and Sharbat Bazoori Motadil in combination with and without Hijama Bil-Shart (Wet Cupping), evaluated on multiple patients with the possibility of lowering increased serum uric acid level. In this review, we will focus on controlling of hyperuricemia with the use of Unani Medicine and Hijama Bil Shart.

In the last few decades, both hyperuricemia and gout have increased markedly. Recent studies show new concept into the transporters that handle uric acid in the kidney as well as possible links between these transporters & hyperuricemia. There are changes in the treatment of established hyperuricemia. Febuxostat and PEGuricase are two novel treatments that have been evaluated and shown to be highly effective in the management of hyperuricemia. Monosodium urate (MSU) crystals are the inducers of inflammation. Within the joint, they trigger a local inflammatory reaction, neutrophil recruitment, and the production of proinflammatory cytokines as well as other inflammatory mediators. The uptake of MSU crystals by monocytes involves interactions with components of the innate immune system. The inflammatory effects of MSU are IL-1-dependent and can be blocked by IL-1 inhibitors. These advances in the understanding of hyperuricemia and gout provide new therapeutic targets for the future.

The Unani medicines have proven its own importance regarding its affectivity, cost and minimal side effects for many illnesses. The general treatment of Gout in Unani system of medicine (USM) comprises of moderating the alter humours (akhlat), excretion of excessive & altered humours, correction of digestion & regulation of metabolism and subside the local inflammation. For this purpose, the medicines having properties like, demulcent (mulattif), concoctive (munzij), diuretic (mudir-e-boul), digestive tonic (muqawwi-e-meda) and anti-inflammatory (muhallil-e-auram) are used. There are so many drugs to treat Gout without causing any side effect but most of these drugs have not been studied on modern scientific parameters, so keeping the fact in mind the formulation comprises of drugs Chobchini (Smilax china), Surinjan Shirin (Colchicum latium), Sibr (Aloe barbadenesis) has been tested for gout treatment. It is an open clinical trial; conducted on 60 patients for duration of 60 days. Findings were recorded on a specially designed chart and inference was made by appropriate statistical analysis (Paired 't' test). This result suggesting the effect of test drug is very effective in lowering the serum uric acid. Findings of trial were recorded on a specially designed chart and inference was made by appropriate statistical analysis.

Clinical Evaluation of 'Punarnava Amrita Guggulu' in the Management of Vata-Rakta (Gout

An unusual metabolic condition is Vata-Rakta. The chronic relapsing and intermittent nature of Vata-Rakta illness makes it extremely distressing. The overall prevalence of hyperuricemia is 25.8%, with a larger percentage of men than women having increased serum uric acid. Vatarakta is the name for the illness that is characterised by Raktadhatu aberration brought on by Vata Dosha morbidity. At the current study, Punarnava Amrita Guggulu was taken twice daily with lukewarm water in doses of 1gm (two 500mg tablets each). Results revealed that Vidaha (Burning sensation) (58.33%), Sandhi Shoola (reduced by 65.71%), Sandhi Shotha (reduced by 54.76%) were all statistically very significant outcomes of the current study. Raga (Redness in joints) (60.71%), Toda (Pricking sensation) (57.95%) and Twakvaivarnya (Discoloration) (61.25%). Sparsha Asahyata (Tenderness in joints) (52.47%) Visphota(58.60%) Sandhi Akunchana Prasarana Vedna(63.49%) Sandhi Vikriti (Deformity Status) (47.14%).
Evaluating the Role of Suranjan in Combination with Sharbat-e-Bazoori for the Management of Gout

Gout is metabolic disease that usually affects middle age and rarely elderly people. Clinically it is observed that this disease occurs due to intake of protein rich and high calorie diet. Pathologically deposition of mono-sodium urate crystals in joint space and other tissues results in the symptoms like pain and inflammation. Methods: in this study, patients of gout are treated with soranjan tablet in combination with sharbat-e-Bazoori for a period of one month who qualified the inclusion criteria. Results: we observed that severity of joint swelling, pain, inflammation, tenderness, stiffness and mobility resolved significantly after the treatment (p-value <0.0001*). All the objective parameters like; Sr. uric acid, ESR and CRP improved significantly after the treatment with a p-value (<0.0001*). Conclusion: owing to the painful and stressful condition of the gout, it has been demonstrated that a combination of two Unani drugs (Habi Suranjan and Sharbat-e-Bazoori) together effectively treats gout and the associated symptoms.
