Case report on nutrition management in liver transplant

Liver transplantation or hepatic transplantation is a procedure of replacing diseased liver with a healthy liver. It needs cautious post-operative care including nutritional intervention. We present a 65 year old patient underwent a liver transplant due to acute liver failure. The patient’s diet history revealed a daily intake of carbohydrates and fats but inadequate protein intake. The patient’s nutritional interventions were analyzed and evaluated through hospital recalls and proper follow up visits. In studies, hospital recall on 7 day of post-surgery showed a great improvement in calorie and protein intake. The discharge diet plan included a well-balanced diet with protein, carbohydrates and fat intake to ensure nutritional care for the patient’s recovery. Nutritional intervention plays a vital role in post-operative care of liver transplant patients. Patients with end-stage liver disease (ESLD) frequently have diverse abnormalities of carbohydrate, lipid, and protein metabolism that cause progressive deterioration of their clinical condition and lead to malnutrition. Malnutrition is almost universally present in patients with ESLD undergoing liver transplantation and has been associated with increased morbidity and mortality. It is essential to identify and correct nutritional deficiencies in this population and provide an adequate nutritional support during all phases of liver transplantation. In conclusion, this article reviews the etiologic factors, prevalence, assessment and management guidelines of nutritional disorders seen in patients with ESLD undergoing liver transplantation.

N utritional status has been shown to be an important prognostic factor in patients with end-stage liver disease undergoing liver transplantation (1-7). In these patients, malnutrition may be related to poor nutritional intake, malabsorption and liver disease itself. Muscle wasting, fat store depletion, impaired immunological function, and decreased vitamin and trace element serum levels may influence patient outcome by prolonging catabolic state, increasing risk of septic complications, and causing long term weaning and intensive care unit stay (8,9). However, there are no detailed clinical guidelines and recommendations with regard to the perioperative nutrition of the liver transplant patient. Only limited data from controlled studies are available, which may well be attributed to transplant units placing a
