Portuguese recommendations for the use of biological therapies in children and adolescents with juvenile idiopathic arthritis--December 2011 update

To update the Portuguese recommendations in order to assist the rational and safe prescribing of biological therapies in children and adolescents with Juvenile Idiopathic Arthritis (JIA) as more evidence and experience with these drugs are available. The recommendations were formulated by Rheumatologists and Pediatricians, with experience in Pediatric Rheumatology, based on literature evidence and consensus opinion. The evidence was sought through a MEDLINE search. The retrieved results were discussed and a set of recommendations proposed. All propositions were extensively debated and the final recommendations formulated. A consensus was achieved regarding the eligibility, response criteria, maintenance of biologic thera py, and procedures in case of non-response. Also, specific recommendations concerning safety procedures before and while on biologic therapies were formulated. Ankylosing spondylitis (AS) is a chronic inflammatory immune-mediated arthritis belongs to the so called group of axial spondylarthritis (SPA) (1).Considerably, the onset of the disease occurs in patients between the third and the fourth decade of life, and if not treated effectively it can lead to disability in about third of the patients (2-6). The pathogenesis of AS is multifactorial and results from a complex interplay between genetic predisposition and environmental triggers (7)HLA B27, from its discovery in 1973,(8) represents the central genetic factor related to disease etiopathogenesis. However, about 10-20% of defined AS patients were HLA B27 negative(9).We aimed in this review to assess the role of different factors contribute to the outcome of the disease.

ESPONDILITIS ANQUILOSANTE

RESUMEN. La espondilitis anquilosante (EA) es una enfermedad autoinmune, progresiva, sistémica, infla-matoria e incapacitante. Su prevalencia en México es del 0.9% en la población general. Afecta más a hombres en una relación de 5:1; la edad promedio de aparición es de los 15 a los 40 años de edad. Su etiología es multi-factorial; entre los agentes que llevan a su desarrollo se han implicado componentes genéticos y ambientales, principalmente. Palabras clave: Espondilitis anquilosante, complejo mayor de histocompatibilidad HLA-B27. ABSTRACT. Ankylosing spondylitis (AS) is an autoimmune, progressive, systemic, inflammatory and disabling disease. Its prevalence in Mexico is of 0.9% in the general population. It affects men the most, in a 5:1 ratio; the average age of onset is 15 to 40 years old. Its etiology is multifactorial; among the main agents leading to its development are genetic and environmental components.
