Background Chronic musculoskeletal pain (CMP) affects daily life function and is the most prevalent disorder in primary health care. The primary objective was to examine demographic factors and pain characteristics associated with reduced health-related quality of life (HRQoL) among patients in primary care reporting CMP. Our secondary objective was to compare HRQoL in patients with and without CMP. Method This cross-sectional study was conducted in Trondheim, Norway. Twenty randomly selected GPs, and their listed patients aged 21–58 were invited to participate. Self-reported CMP data was collected using online questionnaires. HRQoL was measured by the 15D questionnaire, total score of 0.9 was used as cut-off for clinical reduced HRQoL. To estimate the prevalence of chronic regional and widespread musculoskeletal pain in a sample of the general adult population and study the association to age, sex, socioeconomic class, immigration, and housing area. A cross sectional survey with a postal questionnaire to 3928 inhabitants on the west coast of Sweden. The age and sex adjusted prevalence of chronic regional pain (CRP) was 23.9% and chronic widespread pain (CWP) 11.4% among 2425 subjects who responded to the complete questionnaire. Odds ratio (OR) for CWP showed a systematic increasing gradient with age and was highest in the age group 59-74 yrs (OR 6.36, 95% CI 3.85-10.50) vs age group 20-34 yrs. Background The long-term consequences of chronic pain and/or stiffness from the musculoskeletal system (musculoskeletal complaints: MSCs) have not been well explored. The aims of this study were to investigate whether MSCs reported at baseline influence all-cause and cause-specific mortality during 21 years follow-up of a general Northern Norwegian adult population. Methods A total of 26,977 men and women aged 25-97 years who participated in the 1994-1995 survey of the Tromsø study (response rate 77%) were included in the present prospective cohort study. Baseline data were collected from the 1994-1995 survey and information on death and emigration was taken from the National Register of Norway. Cox regression analyses were performed to examine if MSCs predicted risk of mortality. Results 5693 (21.1%) participants died during follow-up. Mean time between entry into the survey and death or emigration was 18.6 years (standard deviation 4.87) for all-cause mortality. There was an increased risk of death among those with MSCs at baseline in the crude Cox regression model. Female gender has been associated with musculoskeletal complaints (MSCs), but there are limited studies on how other factors may influence women and men differently. The aim of this prospective cohort study was to explore possible predictors of MSCs in women and men free of MSCs at baseline. The present study included participants from the population-based Tromsø study, with baseline data from 1994-1995 and follow-up data from 2007-2008. MSCs were defined as having pain and/or stiffness in muscles and joints for 3 consecutive months during the past year. Predictors of MSCs were examined through binary logistic regression analyses and presented as odds ratios with 95% confidence intervals. PURPOSE: The aims of this paper were to evaluate the relationship between chronic pain and global quality of life (GQOL) and to explore the effect of possible confounders, mediators, and moderators such as selected demographic variables, chronic illnesses, stress-related symptoms, fatigue, and subjective health of the relationship between chronic pain and GQOL. METHODS: We used a cross-sectional design, including 1,893 respondents from a population of 4,000 of Norwegian citizens, aged 19-81 years, who were randomly drawn from the National Register by Statistics Norway in November 2000 (48.5%). Pain duration of more than 3 months was categorized as having chronic pain. The Quality of Life Scale, the Fatigue Severity Scale, and the Posttraumatic Stress Scale were used as our main dependent and independent variables, respectively. A series of multiple regression analyses (GLM in SPSS) were applied using GQOL as the dependent variable, entering subsets of independent variables in a theoretically predefined sequence. RESULTS: In the total model, there was no significant relationship between chronic pain and GQOL. The model explained 39% of the variance in GQOL. For direct effect sizes, stress-related symptoms were related most strongly to GQOL, followed by subjective health, fatigue, chronic illnesses, and selected demographic variables. CONCLUSION: These findings support the assumption of a complex and indirect relationship between chronic pain and GQOL.

To identify and characterize persons in the population who do not report musculoskeletal pain. Methods. This was a population-based 4-year prospective longitudinal study by postal questionnaire. Population sample recruited from general practice registers in NorthWest England followed up at 15 months and 4 years. Results. Of respondents, 17.4% [95% confidence interval (CI) 16.1%-19.7%] reported no pain in the previous month at all 3 measurement intervals over 4 years. They were characterized by low levels of psychological distress [relative risk (RR) low vs high levels of psychological distress 2.3; 95% CI 1.7-2.9], low levels of depression (2.7; 95% CI 2.0-3.6), low levels of anxiety (2.1; 95% CI 1.6-2.7), low health anxiety (1.6; 95% CI 1.2-2.1), and low illness behavior scores (5.8; 95% CI 4.0-8.3), good quality sleep (3.4; 95% CI 2.6-4.4), no somatic symptoms (RR 0 vs 3 or more, 3.1; 95% CI 1.6-6.3) and no adverse life events in the 6 months prior to baseline data collection (RR 0 vs 3 or more, 3.2; 95% CI 1.6-6.2). On multivariable analysis, good quality sleep, low illness behavior, low psychological distress, and absence of recent adverse life events remained statistically independent predictors of musculoskeletal health. In total, 46% of persons who had all 4 of these characteristics consistently reported being free of pain, compared to only 5% of those who had none. Conclusion. In a general population sample, over a period of 4 years, only around 1 in 6 persons do not report musculoskeletal pain. These persons report low levels of psychological distress and high quality sleep, both of which are potentially modifiable risk factors for the targeting of interventional or preventive strategies.

Objectives Musculoskeletal (MSK) pain and hypersomnolence (HPS) are very disabling conditions that may share some pathophysiological factors. This study aimed to evaluate the interaction between MSK pain and HPS and its association with mood symptoms, fatigue, quality of life, and both objective and subjective sleep quality. Design Cross-sectional study. Setting General population based sample. Participants 510 individuals from EPISONO cohort, São Paulo (Brazil). Measurements All participants completed questionnaires, had clinical assessment and underwent a full-night polysomnography. The SF-36 is a well-validated health status instrument measuring eight different health concepts. One aim of this study was to compare health status as measured by SF-36 in subjects from the general population with no chronic pain (NCP), chronic regional pain (CRP), and chronic widespread pain (CWP). A second aim was to assess if SF-36 could reflect changes in pain status over time. A third aim was to study if health status at baseline, measured by SF-36, could predict pain status 3 years later. The study was designed as a 3-year follow up with a postal questionnaire, including the SF-36 health survey, to 2357 subjects from the general population aged 20 -74 years. The results were controlled for age, sex, co-morbidity, and socio-economic status. At baseline, all eight health concepts of SF-36 discriminated between subgroups with NCP, CRP and CWP. Changes in SF-36 over the 3-year follow up time coincided with improvement or deterioration of pain status. Baseline SF-36 scores predicted pain outcome 3 years later. These results support that both physical and mental aspects of health status as measured by SF-36 are affected by the burden of musculoskeletal pain, are sensitive to changes in pain status, and also predict the further development of pain. q

Background and aims The influence of pain as well as Quality of Life (QoL) varies in accordance with biological, social, psychological and existential factors. This study investigates the influence of such factors on the relationship between pain and QoL among older adults from a gender perspective. Methods The Swedish National Study on Aging and Care (SNAC-Blekinge) baseline sample comprised 1402 individuals aged 60–96 years, of whom 769 (55%) reported pain. The participants were invited by a letter to take part in the study, which was carried out by research staff in two sessions of three hour each. Participants gave informed consent and completed a questionnaire between the two sessions. The reason for non-participation was registered among subjects who declined the invitation.
