Primary meningococcal arthritis of the knee caused by Neisseria meningitidis group B: About a case

INTRODUCTION: Primary arthritis is a rare form of meningococccal disease. It occurs as an isolated acute purulent arthritis without meningitis, and presence of Neisseria meningitidis in articular fluid. PATIENT AND OBSERVATION: We report the case of an infant aged 45 days without any particular medical history presenting isolated primitive monoarthritis of the left knee to Neisseria meningitidis group B unhurt of meningeal infringement, at Children's Abderrahim El Harouchi hospital of Casablanca. The examination in the admission showed a feverish infant, with presence of a painful and inflammatory effusion of the left knee. The neurological examination was normal and did not reveal signs of meningeal infringement. The articular draining resembled a purulent liquid whose cytobacteriogical examination showed the presence of neutrophils in heap, the culture revealed the presence of Neisseria meningitidis. Contexte et objectifs. Les patients souffrant de syndrome fibromyalgique sont fréquemment rencontrés en pratique clinique et déconcertent bien souvent les médecins. Notre étude a pour but de déterminer la prévalence de la fibromyalgie chez les patients gonarthrosiques, son impact sur la présentation clinique et radiologique ainsi sur le cours évolutif de gonarthrose. Méthodes. Il s’agissait d’une étude transversale analytique portant sur les patients atteints de gonarthrose définie selon les critères ACR, suivis au service de rhumatologie de l’hôpital militaire Moulay Ismail à Meknès ; entre novembre 2018 et avril 2019. La fibromyalgie était retenue chez les patients remplissant les critères ACR 2010. Résultats. The floating knee: Aretrospective analysis of 172 cases

A retrospective multicentric observational study of 172 floating knee injuries is reviewed. According Fraser's classification 71.5% of the cases are type I and the repartition of the type II in the 3 subgroups is as follow: II A 8.2%, II B 11.6%, et II C 8.7%. At least one of the fractures was open in 69.2% of the patients. The average Injury Severity Score (ISS) was 19.5. The polytraumatized patients (ISS over 18) represented 37.7% of the cases. The surgical procedure started within the 6 first hours in 62% of the injured people. The Intramedullary nailing (IM) was the preferred method at the femur site (73%, 126 cases), at the tibia IM was performed in 54.4% of cases and external fixation was used in one every four patients. The mean operating time was 167 minutes (range: 40'-300') and the total time for the whole procedure was 216 minutes (range: 80'-410'). The mean follow up is 34.2 months. Total knee arthroplasty in a spinal cord-injured patient: A case report

Patients with spinal cord injuries are prone to knee hydrarthrosis (also known as ''water on the knee''), which can cause pain, functional impairment and the restriction of social activities. Total knee arthroplasty is a potentially appropriate treatment. Here, we report on a patient presenting partial T12 AIS grade C paraplegia who was able to walk with two forearm crutches, an ankle-foot orthosis on the right leg and a kneeankle-foot orthosis on the left leg. Thirteen years after the spinal cord injury, the patient presented with septic arthritis of the right knee, complicated by recurrent hydrarthrosis during standing and walking. Arthroscopy revealed advanced osteoarthritis. Total knee arthroplasty was performed, with very good functional and social outcomes two and half years after surgery.
