Inflammatory fibroid polyp (known also as Vanek's tumor) is a type of localized, non-neoplastic inflammatory pseudotumor or inflammatory myofibroblastic tumor that occurs most commonly in the stomach but also in the small and large bowel. It is a documented cause of intussusception in adults. We report a case of a 40-year-old woman who presented with severe, postprandial abdominal pain followed by projectile vomiting over a period of three days. Ultrasonography demonstrated a solid and echogenic mass surrounded by the typical mural layers of an invaginated jejunum. She underwent urgent laparotomy and resection of an 18 cm tumor from the distal jejunum. The immuno-histopathological diagnosis after segmental jejunal resection was a jejunal inflammatory fibroid polyp. Although inflammatory fibroid polyps are seen very rarely in adults, they are among the probable diagnoses that should be considered in obstructive tumors of the small bowel causing intussusceptions.

Its treatment is surgical. The debate focused on the issue of primary en bloc resection of intussuscepted bowel versus initial reduction followed by more limited resection. We report five cases of small bowel intussusception in adult caused by Vanek's tumor. Methods: The retrospective study reviewed epidemiological data, clinical features, diagnosis and management of adults with a diagnosis of intussusception admitted to our department from August 2010 to April 2014. Only intussusception caused by Vanek's tumor was included in our study. Results: There were 4 males and 1 female. Their average age was 53 years. All of them were admitted with abdominal pain. The abdominal exam revealed an abdominal distension in 4 patients and a palpable abdominal mass in 2 patients. One patient had a normal physical examination. CT demonstrated <>, a sign pathognomonic for ileo-ileal intussusception. All of the patients underwent surgical treatment by laparotomy for 4 patients and laparoscopy approach for 1 patient. 4 patients had an ileo-ileal intussusception; which was resected after partial reduction and followed by end-to-end anastomosis. In one case, we found an ileo-caeco-colic intussusception, which was resected and side-to-side ileo-colic anastomosis was performed. Diagnosis of Vanek's tumor was confirmed by immunohistochemical examination. Postoperative course was uneventful. Conclusions: The recommended treatment of adult intestinal invagination is surgical resection. Discussion continues as to whether or not a reduction has to be done before a resection.

Intussusception of the bowel in an adult is a rare surgical emergency. Diagnosis is quite simple as the patient presents with features of intestinal obstruction. The causes are varied with adults and infants. Vanek'stumor is an inflammatory fibroid polyp with high eosinophilic infiltration. Vanek's tumour is a rare cause of intussusception in an adult. Here we present a case of a 40-year-old male who presented with abdominal pain of 3 days onset was diagnosed to have sub-acute intestinal obstruction. On further imaging, he was diagnosed to have intussusception and was taken up for emergency laparotomy. Intraoperatively the intussusception was relieved and the cause was found to be a hard intra-luminal mass. Resection of the mass with 5 cm clearance was done with end to end anastomosis. Histo-pathology revealed an inflammatory fibroid polyp.

Macroscopically the tumours are either pedunculated or sessile in nature. Majority of them are in the form of pedunculated polyps (Figure 1). Their size may vary from 0.2 cm to 20 cms. Vanek's tumor is a rare solitary lesion that is non-neoplastic, and arises from the submucosa of the gut. The most commonly affected sites are the stomach and the ileum [1]. IFP causing intestinal obstruction is uncommon and that ileo-ileal intussusception has been reported in the literature rarely. We present a case of an ileo-ileal intessuception in a patient who presented with acute on chronic pain abdomen and anorexia. CT abdomen & pelvis revealed a 46 × 36 mm intraluminal mass in the left iliac fossa causing ileo-ileal intussusception. A 60 years old gentleman was brought to the A&E with a history of intestinal obstruction. He was examined and admitted to the surgical unit. CBC revealed unique feature of reactive thrombocytosis apart from a low hemoglobin and a raised TLC. His CT scan demonstrated findings of ileo-ileal intussusception. Inflammatory fibroid polyps (IFP) are benign mesenchymal tumors described first by Vanek, who reported a series of cases occurring in the gastric submucosa. Since then, there have been varying presentations of these benign tumors, many of them have shown to infiltrate into surrounding tissue and can cause significant symptoms including intestinal obstruction and intussusception. This can result in peritonitis and bowel necrosis, and can change the management of a typical bowel obstruction. Adult intussusceptions are rare, as this phenomenon occurs most often in infants and children. IFP can occur anywhere in the gastrointestinal tract but most commonly in stomach and small intestine. The largest IFP arising from the gastrointestinal tract described in English literature has been 15 cm at total length. Inflammatory fibroid polyps (Vanek’s tumor) are non-frequent benign lesions, described by Josef Vanek in 1949, that originate in the submucosa of the gastrointestinal tract and usually reach 1 to3 cm. They are generally seen in the stomach and the small intestines, and are less common in the colon and the esophagus. Inflammatory fibroid polyps (IFPs) are one of the rare conditions leading to intestinal obstruction in adults. A differential diagnosis includes gastrointestinal stromal tumor, small intestine lymphoma, and small intestine cancer. We presented here a case with a giant inflammatory fibroid polyp of the terminal ileum causing intussusception.
