Introduction: Neurotology is a rapidly expanding field of knowledge. The study of the vestibular system has advanced so much that even basic definitions, such as the meaning of vestibular symptoms, have only recently been standardized. Objective: To present a review of the main subjects of neurotology, including concepts, diagnosis and treatment of Neurotology, defining current scientific evidence to facilitate decision-making and to point out the most evidence-lacking areas to stimulate further new research. Methods: This text is the result of the I Brazilian Forum of Neurotology, which brought together the foremost Brazilian researchers in this area for a literature review. In all, there will be three review papers to be published. This first review will address definitions and therapies, the second one will address diagnostic tools, and the third will define the main diseases diagnoses. Each author performed a bibliographic search in the LILACS, SciELO, PubMed and MEDLINE databases on a given subject. The text was then submitted to the other Forum participants for a period of 30 days for analysis. A special chapter, on the definition of vestibular symptoms, was translated by an official translation service, and equally submitted to the other stages of the process. There was then a in-person meeting in which all the texts were orally presented, and there was a discussion among the participants to define a consensual text for each chapter. The consensual texts were then submitted to a final review by four professors of neurotology disciplines from three Brazilian universities and finally concluded. Based on the full text, available on the website of the Brazilian Association of Otorhinolaryngology and Cervical-Facial Surgery, this summary version was written as a review article. Result: The text presents the official translation into Portuguese of the definition of vestibular symptoms proposed by the Bárány Society and brings together the main scientific evidence for each of the main existing therapies for neurotological diseases. Conclusion: This text rationally grouped the main topics of knowledge regarding the definitions and therapies of Neurotology, allowing the reader a broad view of the approach of neurotological patients based on scientific evidence and national experience, which should assist them in clinical decision-making, and show the most evidence-lacking topics to stimulate further study.

Brazil is a heterogeneous country with continental dimensions. The different characteristics of cultural, socioeconomic, and demographic status of the population drive different strategies for neurological care. This knowledge helps the understanding of the current scenario with the consequent possibility of preparing for future challenges. We used data from annual internal forms of the Brazilian Academy of Neurology (BAN) since 2006 and the survey for all BAN members (3,240) in 2016. The geographic distribution of BAN members in Brazil follows the demographic concentration of the population. Participation of members from big cities was the most prevalent, 18.7% of participants were young neurologists, and 36.7% of neurologists had more than of 20 years of neurological practice. The improvement of knowledge of neurological practice in Brazil will be useful for BAN leadership in planning future actions. The 50th anniversary of the Brazilian Academy Neurology (BAN) is being celebrated. BAN was founded on May 5th, in 1962. The BAN initial leaders and members are treated with reverence, as well as the honorable place where its foundation took place: the Institute of Neurology, first in Brazil, at the backyards of the ancient National Hospice for the Insane, which is an important and fruitful birthplace of the Brazilian Neurology.

BACKGROUND Neurological disorders are significant causes of morbidity and mortality worldwide. However, data about general neurological inpatient admissions in Brazil is limited. Objective: To investigate the prevalence of neurological disorders according to disease group and lesion site among patients admitted to a general Neurology ward. METHODS This was an observational and descriptive study. The hospital discharge database for the Neurology ward was surveyed in accordance with the International Classification of Diseases, 10th edition (ICD-10), from September 2008 to October 2019. The final diagnosis was classified into neurological disorder groups and site. RESULTS Overall, 2,606 clinical neurological patient files were included, with mean length of hospitalization of 16.7 days and a total of 325 readmissions (12.5%). According to the Department of Informatics of the Unified Health System (DATASUS), the group of diseases of the circulatory system is the main cause of death in the state of Espírito Santo in 2021, with a total of 7208 deaths. Of these, 1820 (25.24%) are related to cerebrovascular diseases. Furthermore, neurological diseases cause a subsequent impact resulting from possible sequelae, increasing public health costs, length of stay and complexity in monitoring these patients. Understanding the impact of the epidemiological data mentioned above and associated with the current level of complexity of the neurology service at Hospital Santa Casa de Misericórdia de Vitória, the location used as a reference in this work, a study is necessary with the objective of collecting data related to inter-consultations neurological, in order to demonstrate the impact of hospital neurology in multiple medical areas. The objective was based on tracing an epidemiological profile based on the collection, recording and analysis of data from neurological consultations requested at the Hospital Santa Casa de Misericórdia de Vitória (HSCMV) over a period of one year. This is a primary, retrospective, observational, cross-sectional, descriptive study, carried out in a single center. It is based on data collected from the HSCMV neurology service's own database combined with information from patients' electronic medical records checked during hospital consultations at that service. It was concluded that the predominance of cerebrovascular diseases is in accordance with national and global literature and can be attributed to the contribution of modifiable risk factors such as physical inactivity, dyslipidemia, high blood pressure, diabetes mellitus and smoking. Furthermore, knowledge of the prevalence of such conditions related to hospitalization must be used in order to develop strategies to prevent complications and treatment associated with hospital stays.

Escola Paulista de Medicina (EPM) was founded in 1933 and the first Professor of Neurology was Fausto Guerner, who could not effectively assume the teaching activities due to his premature death in 1938. Professor Guerner had had his neurological training at Paris. Professor Longo was his successor. Longo was one of the founders of Arquivos de Neuro-Psiquiatria the foremost journal of neurosciences in Latin American. Longo died in 1967 and Professor Paulo Pupo succeeded him. Pupo introduced electroencephalography in Brazil. After his death in 1970, Professor Dante Giorgi succeeded him until 1974. Professor José Geraldo Camargo Lima took over the position after Giorgi's death. He created the Neurological Emergency unit, initiated the Post-Graduation in Neurology and divided the Discipline in specialized units. During the 1980's and until his retirement in 1995, EPM had become one of most important centers of Brazil training neurologists and researchers in neurological sciences.
