In medical education research, self-efficacy beliefs have mostly been studied in relation to their impact on the mastery of communication competencies and clinical skills. Few studies are available – in the medical domain – that centre on a broader range of medical curriculum competencies, the way self-efficacy improves self-regulated learning, how self-efficacy affects motivation, provides study support, how self-efficacy boosts the career development of students and, how self-efficacy influences social and emotional support of students. © 2012 Published by Elsevier Ltd. Self-Efficacy in Medical Education: A Systematic Review of Literature

ABSTRACT: Self-efficacy is described as an important influencing factor of human behavior, linked to motivation and performance. Thus, its analysis in the educational context is relevant. The study aims to carry out a systematic review of self-efficacy in medical education, nationally and internationally, to analyze the main factors that impact the self-efficacy beliefs of medical professors and students. Therefore, we researched four databases: Virtual Health Library (BVS), Public Medline (PubMed), Brazilian Digital Library of Theses and Dissertations (BDTD), and CAPES Portal, from 2015 to 2020, in Portuguese, Spanish, and English. Development and Initial Validation of a Survey to Assess Students' Self-Efficacy in Medical School

Self-efficacy is a personal belief in one's capability to successfully execute the behaviors necessary to attain designated types of performances. Sometimes described as task-specific self-confidence, self-efficacy is a key component in many contemporary theories of motivation and learning. The purpose of this study was to develop a survey for measuring students' medical skills self-efficacy and to collect reliability and validity evidence for the instrument. A secondary purpose was to explore differences in students' self-efficacy from year 1 of medical school to year 4. We created 19 survey items based on the 6 core competencies of the Accreditation Council for Graduate Medical Education, and we collected data from 304 medical students. Results from an exploratory factor analysis suggested three interpretable factors: patient care self-efficacy (eight items, Cronbach's a = 0.92), interpersonal skills self-efficacy (three items, Cronbach's a = 0.76), and evidence-based medicine self-efficacy (three items, Cronbach's a = 0.79). We then compared students' self-efficacy at different stages of training using a one-way multivariate analysis of variance. Consistent with our expectations, we found several statistically significant differences, suggesting students' self-efficacy increased considerably from year 1 of medical school to year 4, F(9, 725) = 30.58, p 0.001, Wilks' λ = 0.46. Using this survey, medical educators and researchers have a psychometrically sound tool for measuring students' medical skills self-efficacy during undergraduate medical education. Practical implications and future directions are discussed.

Assessment of self-efficacy and academic performance of medical students

Comparison of the medical students’ perceived self-efficacy and the evaluation of the observers and patients

The accuracy of self-assessment has been questioned in studies comparing physicians' self-assessments to observed assessments; however, none of these studies used self-efficacy as a method for self-assessment. The aim of the study was to investigate how medical students' perceived self-efficacy of specific communication skills corresponds to the evaluation of simulated patients and observers. Methods: All of the medical students who signed up for an Objective Structured Clinical Examination (OSCE) were included. As a part of the OSCE, the student performance in the "parent-physician interaction" was evaluated by a simulated patient and an observer at one of the stations. After the examination the students were asked to assess their self-efficacy according to the same specific communication skills. The Calgary Cambridge Observation Guide formed the basis for the outcome measures used in the questionnaires. A total of 12 items was rated on a Likert scale from 1-5 (strongly disagree to strongly agree). We used extended Rasch models for comparisons between the groups of responses of the questionnaires. Comparisons of groups were conducted on dichotomized responses. Results: Eighty-four students participated in the examination, 87% (73/84) of whom responded to the questionnaire. The response rate for the simulated patients and the observers was 100%. Significantly more items were scored in the highest categories (4 and 5) by the observers and simulated patients compared to the students (observers versus students: -0.23; SE:0.112; p=0.002 and patients versus students:0.177; SE:0.109; p=0.037). When analysing the items individually, a statistically significant difference only existed for two items. Conclusion: This study showed that students scored their communication skills lower compared to observers or simulated patients. The differences were driven by only 2 of 12 items. The results in this study indicate that self-efficacy based on the Calgary Cambridge Observation guide seems to be a reliable tool.

Furthermore, over the last 34 years, educational researchers from diverse fields of inquiry have used the notion of self-efficacy to predict and explain a wide range of human functioning, from athletic skill to academic achievement. Perceived self-efficacy and academic performance of medical students at ataturk university, Turkey

Self-efficacy's influence on student academic achievement in the medical anatomy curriculum

Self-efficacy is defined as a person's beliefs in his or her own abilities to successfully complete a task and has been shown to influence student motivation and academic behaviors. More specifically, anatomical self-efficacy is defined as an individual's judgment of his or her ability to successfully complete tasks related to the anatomy curriculum; these include dissecting, learning anatomical concepts, and applying anatomical knowledge to clinical situations. The purpose of this study was to investigate the influence of anatomical selfefficacy on the academic performance of students enrolled in a medical gross anatomy course. To obtain students' anatomical self-efficacy ratings, surveys containing the same anatomical self-efficacy instrument were completed by first-year medical students at a southeastern United States allopathic medical school after each of four gross anatomy assessments. Additional data collected included student demographic information, Medical College Admission Test 1 (MCAT 1) scores, and anatomy assessment scores, both written examination and laboratory practical. To investigate the potential predictive nature of self-efficacy for academic performance on both the written examination and the laboratory practical components of medical anatomy assessments, hierarchical linear regression analyses were conducted. For these analyses, academic ability (defined as the sum of the physical sciences and biological sciences MCAT scores) was controlled. The results of the hierarchical linear regressions indicated that all four laboratory practical scores were predicted by the corresponding self-efficacy ratings, while two (i.e., thorax/ abdomen and pelvis/lower limb) of the four written examination scores were predicted by the corresponding self-efficacy ratings (P 0.05).

An Investigation of the Self-Efficacy of Medical Students for the Anatomy Curriculum: Role of Gender and Prior Experience, and Self-Efficacy's Influence on Academic Achievement

Anatomical self-efficacy is defined as an individual's judgment of his or her ability to successfully complete tasks such as dissecting, learning anatomical knowledge, and applying anatomical knowledge to clinical situations. This research investigates medical student self-efficacy for the anatomy curriculum. Five surveys containing the same embedded anatomical self-efficacy instrument were completed by first-year medical students at the University of North Carolina School of Medicine; one pre-survey administered prior to students beginning a medial gross anatomy course and four post-surveys administered after students completed examinations during the course. Impact of Self-Efficacy Beliefs on Short- and Long-Term Academic Improvements for Underperforming Medical Students

This study investigated the effectiveness of an innovative remediation program, based on promoting self-efficacy, a construct of self-regulation theory, which was developed to provide sustainable improvements in academic and clinical performance of underperforming medical students. Eighteen medical (eight 4 th and ten 5 th year) students participated in the remediation program. Interventions were multi-faceted and composed of a wide range of group activities. The efficacy of the intervention was evaluated using mixed-method approach consisting of interviews, questionnaire and quantitative analysis of OSCE outcomes (pre and post intervention). The in-depth interviews were used to explore the short-and long-term causal effects of the remediation program on improved performance. Students demonstrated significant improvements across 3 of the 5 domains measured in OSCE performance (management, diagnosis and communication). There was a 44% improvement in number of stations passed by participants post-remediation (95% CI = -52.25 to -35.64; r = 0.82). Large effect sizes were obtained, indicating the substantive significance of the results. Self-efficacy beliefs were markedly increased post-intervention with the total score increasing from 33.52 (SD=4.9) points pre-remediation to 45.71 (SD=5.1) points postremediation (95% CI = -14.52 to -9.83; p<0.001; r = 0.77). Subsequent qualitative data suggested that beyond summative exam results, gains were translated to the clinical context with enhanced confidence and self-belief, enabling perceived improved performance in the workplace. Multi-dimensional, group orientated remediation, which emphasises self-efficacy beliefs, may enhance outcomes for students in both high stakes assessment and in the clinical context, thus allowing translational and longitudinal benefits.
