Socio-Economic Status and Life Expectancy in the United States, 1990-2010: Are We Reaching the Limits of Life Expectancy? (original) (raw)
This study updates earlier findings regarding changes in the relationship between socio-economic status (SES) and life expectancy (at birth) between 1970 and 1990. In a sample randomly drawn from each of eight of the nine census divisions of the United States, the earlier study found that High SES populations in seven of the eight states gained additional life expectancy over Low SES populations between 1970 and 1990. In the remaining state, the gap between High and Low SES populations found in 1970 narrowed by 1990, but did not disappear. Thus, High SES populations had higher average life expectancy than Low SES populations both in 1970 and in 1990. In the process of conducting this research, it became apparent that the life expectancy gains observed between 1970 and 1990 for both low and High SES populations did not continue from 1990 to 2010. This finding was neither wholly unexpected nor an active focus of the research. However, because the finding fits within the debate on the limits to human longevity, it seemed worthwhile to report it. Thus, the current study offers two major findings. First, the average life expectancy gap between high and Low SES populations found in 1990 has persisted to 2010; second, average life expectancy gains are far less for the Low SES population between 1990 and 2010 than was found for 1970 to 1990; and third, average life expectancy gains for the High SES populations effectively came to a halt between 1990 and 2010. These results have implications for the relationship between social inequality and health outcomes. They also confirm that the United States did not meet one of two key national health policy goals: the elimination of health disparities by 2010.

Widening socioeconomic inequalities in US life expectancy, 1980-2000

Background This study examines changes in the extent of inequalities in life expectancy at birth and other ages in the United States between 1980 and 2000 by gender and socioeconomic deprivation levels. Methods A factor-based deprivation index consisting of 11 education, occupation, wealth, income distribution, unemployment, poverty, and housing quality indicators was used to define deprivation deciles, which were then linked to the US mortality data at the county-level. Life expectancy estimates were developed by age, gender, and deprivation levels for three 3 year time periods: 1980-82, 1989-91, and 1998-2000. Inequalities in life expectancy were measured by the absolute difference between the least-deprived group and each of the other deprivation deciles. Slope indices of inequality for each gender and time period were calculated by regressing life expectancy estimates on deprivation levels using weighted least squares models. Results Those in less-deprived groups experienced a longer life expectancy at each age than their counterparts in more-deprived groups. In 1980-82, the overall life expectancy at birth was 2.8 years longer for the least-deprived group than for the most-deprived group (75.8 vs 73.0 years). By 1998-2000, the absolute difference in life expectancy at birth had increased to 4.5 years (79.2 vs 74.7 years). The inequality indices also showed a substantial widening of the deprivation gradient in life expectancy during the study period for both males and females. Conclusions Between 1980 and 2000, those in higher socioeconomic groups experienced larger gains in life expectancy than those in more-deprived groups, contributing to the widening gap.

Reducing health disparities in the United States has been targeted by numerous policies. We examine educational disparities in mortality and life expectancy among non-Hispanic blacks and whites in the 1980s and 1990s. Despite increased attention and substantial dollars directed to groups with low socioeconomic status, within race and sex groups, the educational gap in life expectancy is rising, mainly due to rising differentials among the elderly. With the exception of black males all recent gains in life expectancy at age 25 occurred among better educated groups, raising educational differentials in life expectancy by 30 percent. Differential trends in smoking-related diseases explain at least 20 percent of this trend.
Left behind: widening disparities for males and females in US county life expectancy, 1985–2010

Background: The United States spends more than any other country on health care. The poor relative performance of the US compared to other high-income countries has attracted attention and raised questions about the performance of the US health system. An important dimension to poor national performance is the large disparities in life expectancy. Methods: We applied a mixed effects Poisson statistical model and Gaussian Process Regression to estimate age-specific mortality rates for US counties from 1985 to 2010. We generated uncertainty distributions for life expectancy at each age using standard simulation methods. Results: Female life expectancy in the United States increased from 78.0 years in 1985 to 80.9 years in 2010, while male life expectancy increased from 71.0 years in 1985 to 76.3 years in 2010. The gap between female and male life expectancy in the United States was 7.0 years in 1985, narrowing to 4.6 years in 2010. For males at the county level, the highest life expectancy steadily increased from 75.5 in 1985 to 81.7 in 2010, while the lowest life expectancy remained under 65. For females at the county level, the highest life expectancy increased from 81.1 to 85.0, and the lowest life expectancy remained around 73. For male life expectancy at the county level, there have been three phases in the evolution of inequality: a period of rising inequality from 1985 to 1993, a period of stable inequality from 1993 to 2002, and rising inequality from 2002 to 2010. For females, in contrast, inequality has steadily increased during the 25-year period. Compared to only 154 counties where male life expectancy remained stagnant or declined, 1,405 out of 3,143 counties (45%) have seen no significant change or a significant decline in female life expectancy from 1985 to 2010. In all time periods, the lowest county-level life expectancies are seen in the South, the Mississippi basin, West Virginia, Kentucky, and selected counties with large Native American populations.
Widening of Socioeconomic Inequalities in U.S. Death Rates, 1993–2001

Background: Socioeconomic inequalities in death rates from all causes combined widened from 1960 until 1990 in the U.S., largely because cardiovascular death rates decreased more slowly in lower than in higher socioeconomic groups. However, no studies have examined trends in inequalities using recent US national data. Methodology/Principal Findings: We calculated annual age-standardized death rates from 1993-2001 for 25-64 year old non-Hispanic whites and blacks by level of education for all causes and for the seven most common causes of death using death certificate information from 43 states and Washington, D.C. Regression analysis was used to estimate annual percent change. The inequalities in all cause death rates between Americans with less than high school education and college graduates increased rapidly from 1993 to 2001 due to both significant decreases in mortality from all causes, heart disease, cancer, stroke, and other conditions in the most educated and lack of change or increases among the least educated. For white women, the all cause death rate increased significantly by 3.2 percent per year in the least educated and by 0.7 percent per year in high school graduates. The rate ratio (RR) comparing the least versus most educated increased from 2.9 (95% CI, 2.8-3.1) in 1993 to 4.4 (4.1-4.6) in 2001 among white men, from 2.1 (1.8-2.5) to 3.4 (2.9-3-9) in black men, and from 2.6 (2.4-2.7) to 3.8 (3.6-4.0) in white women. Conclusion: Socioeconomic inequalities in mortality are increasing rapidly due to continued progress by educated white and black men and white women, and stable or worsening trends among the least educated.
