Quality of nursing care and nurses’ working environment in Ethiopia: Nurses’ and physicians’ perception

Background: Nurses play critical roles as patient advocate, regulating quality of care and improve health care values. Data are scarce regarding quality of nursing care in Ethiopia. Objective: To assess quality of nursing care and convenience of the nurse working environment to provide quality of nursing care in a tertiary hospital in Ethiopia. Methods: Data was collected prospectively using Quality of Nursing Care Questionnaires of Safford & Schlotfeldt and Practice Environment Scale of Nursing Work Index (PES-NWI). One hundred seventeen nurses and 51 physicians were included and data was analyzed using SPSS for windows version-20. Result: Four of the five PES-NWI variables had mean score less than 2.5, suggesting the nursing environment and management was unfavorable to assure quality care. All nursing care performances were low. The highest score was in nurse carryout orders (66.7%) and lowest in nurse-physician collaboration (24.8%). Conclusion: The quality of nursing care is substandard. Favorable environment and nurse physician relationship must be established to provide quality care.

Patients’ perception of quality of nursing care; a tertiary center experience from Ethiopia

Background: Nursing care closely influences patients' satisfaction with the overall quality of care, and the importance of measuring patient satisfaction with nursing care cannot be emphasized enough. Data are however scarce regarding patients' perception of quality of nursing care in Ethiopia. We performed this study to assess patient's perception of the quality of nursing care in a tertiary center in Ethiopia. Methods: Data were collected prospectively using Quality of Nursing Care Questionnaires-patient of Safford & Schlotfeldt. A total of 340 patients were included using systematic random sampling and data were analyzed using SPSS for windows version-20. Result: The nursing care performance was highest for nurse-physician relation (mean = 3.95) and low for education and home care preparation and physical care (mean score of 2.79 and 2.89 respectively). The emotional care and nurse administration mean score were 3.5 and 3.83 respectively. The overall nursing quality was neither satisfying nor dissatisfying (mean of 3.39). While only 36% of the respondents were satisfied with the nursing care, patient education has the strongest (AOR of 7.4) association with satisfaction. Conclusion: Patients perceived low quality of physical care, education and preparation for home care but better nurse-physician relation and nursing administration. However the overall quality measure was neither satisfying nor dissatisfying. This calls for an action from the health care administrators, educators and other stakeholders to improve the patient perception of quality nursing care.

Nursing Care Practices at Jimma University Medical Center: A Retrospective Cross - Sectional Study in Ethiopia

Nursing care is the gathering of information and prioritizing interventions before providing an appropriate cares using a specified assessment format to intervene the most common problems in Hospitals. Most of the nursing care activities were left undone or poorly assessed in Europe with further disrespect & abuse of women during institutional childbirth services in low-middle-income countries including Ethiopia. The current study focused to determine the status of nursing care practices in the obstetric ward of Jimma University medical center. Wherefore, a retrospective cross-sectional study was conducted from April 16, 2019 to May 15, 2019 with documentary analysis of 344 randomly selected medical records. A semistructured and pretested questionnaire containing 16 items of nursing diagnoses were used to collect data, which was then analyzed by SPSS version 20 under formal consent of participants. During the process of management, frequencies & percentages were calculated and used to describe the data by tables & figures for ranking among the identified nursing cares. A total of 344 records were reviewed and analyzed by the study making 100% review rate. The study revealed all the nursing cares indicated in the domains were provided to labouring mothers with variable degrees. The most prominent nursing cares in the ward were recognized to be cares related to nausea (95.9%), fatigue (95.1%), acute pain relieve related to uterine contractions (94.8%), avoidance of imbalance nutrition due to poor intake (38.1%), bathing/toileting self-care deficit related to pain (27.6%) and constipation due hemorrhoids (24.4%). The remaining elements were practiced rarely. The care practice of labouring mothers in the study unit was not comprehensive. Therefore, the researchers recommend a compressive care approach as much as possible to the study unit and further study on the determinants of variation among the care domains.

Patient experience of nursing care is the perception of the patient about the real existing nursing service. Addressing patient experience of nursing care is very important to improve nursing service quality because it identifies the factors that affect the nursing care quality better than patient satisfaction. Therefore, this study aimed to assess patient experience in nursing care and associated factors among adult admitted patients in Debre Markos and Dessie referral hospitals. An institution-based cross-sectional study was conducted from March 1, 2019, to March 30, 2019, among 528 consecutively selected adult admitted patients. Patients’ Satisfaction and Associated Factors with Nursing Care Services in Selected Hospitals, Northwest Ethiopia

Background: Patients satisfaction measures have developed tremendous attractions in the sector of health care delivery services for the purpose of ascertaining the quality of health care services in general and nursing care in particular. It is everyone's most common experience to witness significant patient/ client complaints and dissatisfactions with respect to hospital healthcare deliveries, particularly nursing care services. The objectives of this study was to determine adult patients' satisfaction and associated factors with nursing care at Felege Hiwot Referral and Finote Selam District hospitals, Amhara region, Northwest Ethiopia 2013. Methods: Institutional-based cross-sectional study was conducted from April 1-30, 2013. Systematic random sampling technique was employed and 374 patients were selected from the study wards of the selected hospitals. A 19-items questionnaire, which involved a 5-point Likert scale, was employed to collect the data. Data were collected by trained data collectors using pretested structured questionnaires. Data were checked for completeness, consistency, coded and entered into EPI Info (version 3.5.2) and transferred to SPSS version 16 for analysis. Bivariate and multivariate logistic regression analysis was computed to test the strength of association and level of significance. P-value <0.05 was considered as statistical significant. Results: The overall satisfaction was found to be 67.1%. Freedom, privacy given to patients and the capabilities of the nurses at their jobs were the top areas of patients have got satisfied on nursing care services delivered. Patients who were admitted to referral hospital were less likely to be satisfied (AOR= 0.047; CI: 0.02-0.14) than those admitted to district hospital. Patients who earned free hospital services were two times more likely to be satisfied in the overall satisfaction than those who paid for the hospital services (AOR=2.01;CI: 1.27-3.48) and those patients who were under acute conditions were less likely to report overall satisfaction with nursing care services (AOR = 0.406; CI 0.21-0.77) than those under chronic illness. Conclusions: A significant proportion of the study participants (67.1%) were satisfied with the overall nursing care services. Differences in the level hospitals where patients admitted to, their ward service type, and their condition of illness were found to be statistically significant predictors of patients' satisfaction. However, the hospitals service needs improvement. The Amhara national regional state (ANRS) Health Bureau is recommended to launch intervention programs to foster the information provision skills and the skills of making clients feel well of the nursing staff at the study hospitals.

The Practice of Nursing Process and Associated Factors Among Nurses Working in Public Hospitals of Harari People National Regional State, Eastern Ethiopia: A Cross Sectional Study

Background: The nursing process is the framework for providing professional and quality nursing care. The practice of nursing process in part of Africa has not reached the standard that is set by the profession’s regulatory body. Levels of Adult Patients' Satisfaction with Nursing Care in Selected Public Hospitals in Ethiopia

Objective: To assess level of adult patients' satisfaction and associated factors in nursing care provided in selected public hospitals in Ethiopia. Methods: A cross sectional institution based study was conducted on 582 randomly selected patients admitted for at least two nights in three wards of selected public hospitals in Eastern Ethiopia. Patients were interviewed face to face using the adapted Newcastle Satisfaction with Nursing Scales (NSNS) at the time of their discharge. Data was analyzed using SPSS V 16. Result: More than half of the respondents, 307(52.75%), were satisfied with the nursing care they received. The patient satisfaction was found to be 62.71%, 55.67%, 44.85% and 55.15% for nursing characteristics, the caring activities, the amount of information given and the entire caring environment respectively. Previous history of admission, patients' income level, and type of admission rooms have been found to significantly affect overall satisfaction of patients. Conclusion: The overall level of adult patients' satisfaction was moderate. The hospitals should consider mechanisms to improve the nurses' communication skills and interpersonal relationships beyond training on direct patient care.

Exploration of Patient's Experience with Quality of Care at Outpatient Department of Yekatit 12 Hospital Medical College, Addis Ababa Ethiopia

Background: The patient flow to outpatient service in any hospital of Ethiopia is increasing from time to time. It is a fact that meeting the growing demands of patients for health care service in the outpatient departments must ensure quality of care. Exploring the quality of care in terms of patient perspective is very vital. Therefore, the aim of this study was to explore patients' experience of quality of care at outpatient department of Yekatit 12 Hospital Medical College. Method: A qualitative phenomenological approach research design was used. Eighteen focused group discussions each contains eight patients were conducted. A total of 144 patients were recruited through purposive sampling technique. The interviews were audiotaped, transcribed and subjected to thematic analysis. Semi structured questioner was used to collect the data. Result: The study participants described their experiences about the quality care at the hospital. The finding obtained from the focused group discussion was grouped in to three themes: breach of ethical principles, lack of necessary infrastructure and unwelcoming physical environment. Conclusion: It was found that the quality of care at the hospital was unsatisfactory. Thus the hospital mangers should design and implement a strategy to improve the quality of care provided at the hospital so as to improve patients' satisfaction with the service.

Assessment of Nursing Care Experience and Associated Factors Among Adult Inpatients in Black-Lion Hospital, Addis Ababa, Ethiopia : An Institution-Based Cross-Sectional Study

Introduction: Patient experience has been used as an indicator to measure the quality of health care provided by nurses. However, this information is rarely available in countries like Ethiopia without which improving the quality of service and demonstrating the benefits of changes in nursing practice is usually difficult. The aim of this study was to assess nursing care experience and associated factors among adult inpatients in Black-Lion hospital, Addis Ababa, Ethiopia from March to April 2012. Method: An institution-based cross-sectional study was employed among 374 adult patients admitted to Medical, Surgical and Gynecologic wards in Black-Lion hospital, Addis Ababa. Patients admitted during the study period was considered and convenient sampling was used. A modified<Newcastle Experience with Nursing Scales (NENSs) was used as data collection tool. Data were entered with Epi Info version 3.5.1 and cleaning and analysis was done by using SPSS version 16. Frequencies distribution, binary and multiple logistic regressions were done. Odds Ratio and 95% confidence interval was computed. Result: A total of 374 adult patients from medical, surgical and gynecological wards were approached from the study hospital with 100% response rate. About 90% of patients had rated the experience above the mean. Number of nights the patient spent in the ward (AOR: 0.26) and occupation (AOR: 2.90) were found to be significant predictors of patients' better nursing experience. Conclusion and Recommendation: Nine out of ten patients had rated nursing experience above the mean which is relatively higher magnitude. Patients who should stay longer have to be given insight to better boost the nursing experience.
