"Once Mad always mad": Mental Health Service Users‘ Experience in the Nigeria Labour Market

Background: The dearth of literature on the labour market experience of Mental Health Service Users in Nigeria signifies the dire need to explore service users' perspectives on issues of employment. Objective: This study explores the Mental Health Service User's experience on access to the Nigeria labour market. Method: The study adopts an interpretive phenomenological analysis and conducts an unstructured interview method to investigate participants' experiences and the sense they make out of their daily struggles in seeking employment. Data were thematically coded and supported by participant's narratives. Results: The findings generated two main themes with six additional subthemes: (1) The hindrance to the MHSU's economic participation is linked to hostility and discrimination, and institutional factors (the lack of credible response in policy and practice). Also, (2). close networks remain significant for the group's economic and social adjustment, but it is often not available for all in the labour market. Conclusion: The paper increases our understanding of informal networks' crucial role when navigating the labour market and shows that it may have severe implications in how policy functions. A more comprehensive study is needed to examine the nature of the informal support system that facilitates access to some and ostracizes others' needs. Consideration should be given to the policy environment, the role and efficacy of the non-governmental agencies, and the family networks system to understand hindrances to the engagement of MHSU.

BACKGROUND: There is limited research available regarding the coping skills of individuals living with schizophrenia and the strategies used in the open labor market, particularly from a South African context. OBJECTIVE: The aim of the study was to explore and describe the barriers, facilitators and coping strategies that individuals with schizophrenia use when returning to the open labor market after participating in a vocational rehabilitation program to improve work skills. PARTICIPANTS: Four individuals diagnosed with schizophrenia and two key informants (occupational therapists) participated in the study. METHODS: The research study was positioned within the interpretive paradigm, specifically utilizing an exploratory and descriptive design. Semi-structured interviews were used in order to gather data from the participants. RESULTS: Theme one reflects the barriers related to returning to work (i.e. society's acceptance of an individual's work potential). The second theme relates to the enabling factors related to returning to work (i.e. the usefulness of work preparation programs to enhance open labor market employability). Theme three relates to the coping skills that individuals with schizophrenia utilize (i.e. holistic support enhances participation in the worker role). CONCLUSION: In conclusion, the findings indicate individuals with schizophrenia experience barriers that prevent them from being able to adapt to their work environments. However, with the support of the occupational therapist, family, and employer, together with improvements to the vocational rehabilitation program, these barriers may be alleviated, and there could be an improvement in the reintegration of individuals with schizophrenia into the open labor market. The findings suggest that the disclosure of an individual's medical diagnosis, in order to facilitate the return to work of an individual diagnosed with schizophrenia, could be viewed as a barrier and a facilitator.

Background: Despite the importance of inclusive employment, described in Goal 8 of the Sustainable Development Goals (SDGs), employment of persons with psychiatric disabilities in Africa is lower than among the general population. Objective: The aim of this scoping review is to explore evidence related to the barriers to and facilitators of employment of persons with psychiatric disabilities in Africa. Methods: A literature search was conducted using six relevant electronic databases of articles published between 1990 and 2017. Results: Eight studies were identified and analysed regarding barriers and facilitators of employment of persons with psychiatric disabilities. The dynamic adaptation of the biopsycho-social model was used as an analytical framework. Identified barriers include ill health, (anticipated) psychiatric illness, social stigma and discrimination, negative attitudes among employers and the lack of social support and government welfare. Facilitators of employment include stability of mental illness, heightened self-esteem, a personal decision to work despite stigma, competitive and supported employment, reduction in social barriers/stigma and workplace accommodations. Conclusion: Employment of persons with psychiatric disabilities is essential, yet there is dearth of scientific evidence to identify contextual models that might be useful in African countries and other low-and middle countries (LMICs). This gap in information would benefit from further research to improve the employment rates of persons with psychiatric disabilities in Africa.

Background Job loss in patient with chronic illness like schizophrenia is the most serious public concern in the clinical and socio-economic terms worldwide. Background Mental disorders affect employment and the ability to work, and mental healthcare providers are important in the promotion of health and employment for affected individuals. The objective of this study is to explore the perspectives of mental healthcare providers on pathways to improved employment for persons with mental disorders in two lower middle-income countries. Methods Our study participants included mental healthcare providers (psychiatrists, occupational physicians, psychologists, and social care workers) from Kenya and Nigeria. Working with schizophrenia: Experts' views on barriers and pathways to employment and job retention
BACKGROUND: Unemployment among people with schizophrenia remains high despite slight improvements in vocational rehabilitation services and attitudes towards people with mental health disorders over the years. Experts are in a good position to increase our understanding on why this group still experiences such significant barriers to employment. OBJECTIVE: Interviews explored experts' views on schizophrenia and employment; with a particular focus on individual, attitudinal and structural barriers, as well as available interventions and their outcomes.

Background: Little is known about the extent to which work contributes to the recovery of people with schizophrenia. There is increasing interest in the subject because of new service models and the economic cost of unemployment in people with severe mental illness. Methods: A literature search was carried out with the aim of investigating: a) employment rates in schizophrenia and first-episode psychosis and the extent to which they have changed over time; b) the barriers to work; c) the factors associated with being employed among people with schizophrenia; and d) whether employment influences other outcomes in schizophrenia. Results: There are wide variations in reported employment rates in schizophrenia. Most recent European studies report rates between 10 % and 20%, while the rate in the US is less clear. There is a higher level of employment among first-episode patients. The employment rate in schizophrenia appears to have declined over the last 50 years in the UK. Barriers to getting employment include stigma,discrimination, fear of loss of benefits and a lack of appropriate professional help. The most consistent predictor of employment is previous work history. Working is correlated with positive outcomes in social functioning, symptom levels, quality of life and self esteem, but a clear causal relationship has not been established. Conclusions: Very low employment rates are not intrinsic to schizophrenia, but appear to reflect an interplay between the social and economic pressures that patients face, the labour market and psychological and social barriers to working.

Much of the focus on human rights and mental health in low- and middle-income countries (LMICs) has been on protection from coercion and abuse and on expanding access to mental health services, rather than on promoting and protecting social and economic rights. Despite the importance of work for mental health, there has been very limited consideration of the relationship between work and mental health in LMICs. This paper draws on ethnographic and participatory research in urban and rural sites in Ghana to illustrate the meanings and value of work, as well as experiences of support, exclusion, and discrimination, among people with lived experience of mental illness in Ghana. The paper outlines the policy context of mental health and human rights in Ghana and evaluates the challenges of implementing mental health, disability, and labor legislation to protect the rights of persons with mental illness—particularly the poorest and most vulnerable—in both formal and informal employment. ...

Introduction: Evidence suggests that supported employment strategies improve the rates of employment for individuals with schizophrenia. The Model of Occupational Self-Efficacy (MOOSE) is such a strategy aimed at enhancing work skills and facilitating return to work (RTW). The aim of the current study was to explore the experiences of individuals with schizophrenia about the use of the MOOSE in enhancing work skills and facilitating RTW in the open labour market. Method: A qualitative research project with an interpretivist paradigm explored the experiences and perceptions of nine participants diagnosed with Schizophrenia. Two occupational therapists participated as key informants. Data were collected by means of semistructured interviews and analysed using inductive thematic analysis. Results: Barriers and facilitators to the use of the MOOSE were revealed. More importantly, the MOOSE enhanced the development of worker identities and contributed to the successful adaptation of worker roles in the RTW process of individuals with schizophrenia in the open labour market. Conclusion: The MOOSE is an effective supported employment strategy for persons with schizophrenia. Factors such as client-centred practice, the presence of social support, and ongoing therapeutic support contributed to the participants' successful RTW experience.
