When It Helps, When It Hurts: Preliminary Results of Relationship Enhancement Education and <i>The Hold Me Tight</i> Program

Relationship education such as self-help and group interventions may be helpful for couples seeking to improve their relationships. This outcome study sought to provide preliminary data on the effectiveness of two interventions for couples, comparing the self-help book Hold Me Tight: Seven Conversations for a Lifetime of Love, to the 8-week relationship course, The Hold Me Tight Program: Conversations for Connection, which includes reading the book at home. Results suggest that reading the book alone may lead to positive change, attending the course in addition to reading the book may not result in as much change, and that participation in the course may have a potentially negative effect for less happy couples in the area of dyadic consensus. Results signify the importance of further research to understand the potential benefits as well as iatrogenic effects of relationship education.

PREDICTING FOLLOW-UP OUTCOMES IN EMOTIONALLY FOCUSED COUPLE THERAPY: THE ROLE OF CHANGE IN TRUST, RELATIONSHIP-SPECIFIC ATTACHMENT, AND EMOTIONAL ENGAGEMENT

Emotionally Focused Couple Therapy (EFT), an evidence-based couple therapy , strives to foster lasting change through the creation of secure attachment bonds in distressed couples. Although studies have demonstrated lasting change in follow-up (Wiebe et al., in press), research is needed to investigate predictors of long-term outcomes. Our goal was to investigate predictors of long-term outcomes in relationship satisfaction. Relationship satisfaction was assessed across 24 months in a sample of 32 couples who received an average of 21 EFT sessions. Decreases in attachment avoidance were most predictive of higher relationship satisfaction across follow-up. These findings support the theoretical assumption that EFT helps couples foster lasting change in relationship satisfaction through the facilitation of secure attachment bonds.

TWO-YEAR FOLLOW-UP OUTCOMES IN EMOTIONALLY FOCUSED COUPLE THERAPY: AN INVESTIGATION OF RELATIONSHIP SATISFACTION AND ATTACHMENT TRAJECTORIES

Emotionally Focused Couple Therapy (EFT; The practice of emotionally focused couple therapy: Creating connection. New York, NY: Brunner-Routledge) is an evidence-based couple therapy that aims to create lasting change for couples (Clinical Psychology: Science and Practice, 6(1), 67-79). Although studies have demonstrated strong results in follow-up (Journal of Marital and Family Therapy, 28(4), 391-398), less is known about relationship functioning across time after therapy has ended. We modelled change in relationship satisfaction and attachment from pre-therapy through 24 months follow-up in 32 couples. HLM results confirmed a significant growth pattern demonstrating increases in relationship satisfaction and secure base behaviour and decreases in relationship specific attachment anxiety over the course of therapy and across follow-up at a decelerated rate. These findings support the theoretical assumption that EFT helps couples engaged in therapy create lasting relationship satisfaction and attachment change.

Four year effects of couple relationship education on low and high satisfaction couples: A randomized clinical trial

Objective: Relationship education (RE) usually is conceived of as relationship enhancement for currently satisfied couples, with a goal of helping couples sustain satisfaction. However, RE also might be useful as a brief, indicated intervention for couples with low satisfaction. The current study evaluated the effect of RE on couples with low and high relationship satisfaction. Method: The study was a randomized controlled trial in which 182 couples were randomly assigned to: a book reading control condition (control); RELATE online assessment with feedback and relationship goal setting (RELATE); or RELATE with CoupleCARE (RCC), a flexible delivery skill-based education program. Couples were assessed on relationship satisfaction and individual mental health before and after RE, and through to 4-year follow-up. Results: Couples with high initial satisfaction showed no effects of RE on satisfaction. RCC but not RELATE increased satisfaction in couples with low initial satisfaction, but effects dissipated between 6 and 12 months after RE. There were no effects of RE on mental health. Conclusion: Flexible delivery RE produces immediate effects as an indicated early intervention for couples with low relationship satisfaction, but the effects attenuate. Future research needs to seek methods to produce better maintained effects. What is the public health significance of this article? Flexible delivery relationship education (RE) has no demonstrable benefit for couples high in satisfaction, and universal offering is not justified at this time. RE enhances relationship satisfaction in couples with low relationship satisfaction, but that effect dissipates across 6 to 12 months. Indicated offering of RE might be warranted, but there needs to be further research on how best to enhance maintenance of effects.

CHANGES IN RELATIONSHIP-SPECIFIC ATTACHMENT IN EMOTIONALLY FOCUSED COUPLE THERAPY

Emotionally focused couple therapy (EFT; Johnson, The practice of emotionally focused couple therapy (1st/2nd edition). Brunner-Routledge, New York, 2004) is an effective treatment of relationship distress (Johnson et al., Clinical Psychology: Science and Practice, 1999; 6, 67). However, less is known about EFT's impact on couples' relationshipspecific attachment bond. Using hierarchical linear modeling with a sample of 32 couples, we examined session-by-session changes in couples' relationship-specific attachment anxiety and avoidance and pre-to posttherapy changes in their relationship-specific attachment behaviors. Couples significantly decreased in relationship-specific attachment avoidance, and those who completed a blamer softening significantly decreased in relationship-specific attachment anxiety. Couples' attachment behavior significantly increased toward security. Finally, session-by-session decreases in relationship-specific attachment anxiety and avoidance were significant associated with increases in relationship satisfaction across sessions. These results provide empirical support for the attachment-based assumptions of EFT.

The Role of Relational Instability on Individual and Partner Outcomes Following Couple Relationship Education Participation

Some scholars have suggested that distressed populations may benefit more from couple and relationship education (CRE) than do their nondistressed counterparts. We examined this hypothesis using actor-partner interdependence models to explore the relationship between baseline relational instability and change for individuals and their partners (379 couples; 758 individuals) who participated in a CRE program for 6 to 8 weeks. Findings indicated that a higher level of relational instability on the part of women was associated with greater positive change in depressive symptoms. Furthermore, respondents' and partners' baseline relational instability moderated the change in women's couple quality, such that women reported greater positive change in relationship quality when reporting higher instability and higher relationship quality before CRE participation, and when their partners reported higher instability and lower quality before CRE participation.

Predicting Change in Marital Satisfaction Throughout Emotionally Focused Couple Therapy

Emotionally focused couple therapy (EFT) is an empirically validated approach to couple therapy that uses attachment theory to understand the needs and emotions of romantic partners. EFT is recognized as one of the most effective approaches to couple therapy, but to guide therapists in their use of EFT, a theoretically based model to predict change is needed. This study tested such a model by recruiting 32 couples, and 14 therapists who provided approximately 21 sessions of EFT. Couples completed self-report measures of marital satisfaction, attachment security, relationship trust, and emotional control at pre- and posttherapy and after each therapy session. Couple relationship education: A randomized controlled trial of professional contact and self-directed tools

The aim of this randomized controlled trial was to examine the efficacy of an evidence-based relationship distress prevention program, the Couples Coping Enhancement Training (CCET), in dual well-earning couples and to investigate whether effects vary by (a) hours of professional contact and (b) mode of delivery (face to face vs. self-learning DVD). N = 159 couples were randomly assigned to 1 of 4 intervention conditions: (1) standard CCET (15 hours face to face), (2) compact CCET (12 hr face to face), (3) short CCET (self-learning DVD + 8 hr face to face), or (4) wait-list control group.
