Objective: To compare the frequency of recurrence after surgical excision versus aspiration and injection of Steroids of wrist ganglion Study Design: Comparative cross-sectional study. Place and Duration of Study: Department of General Surgery at Combined Military Hospital, Kohat Pakistan, from Sep 2018 to Aug 2019. Methodology: Seventy patients reported to the Outpatient Department, Combined Military Hospital, Kohat Pakistan, with complaints of ganglion and wanted treatment. Patients were randomly allocated into two Study Groups 1 and 2, by lottery method. Group-1 was treated by aspiration of the cyst contents followed by Steroid injection, while Group-2 was by excisionof the ganglion surgically under a local anaesthetic agent. The patient was followed up after three months to look for recurrence in both groups. Results: The frequency of recurrence was 34.29% in Group-1 and 5.71% in Group-2, (p-value was less than 0.001). Ganglion cysts are smooth, fluctuant, tense cystic and transilluminant swellings commonly found at the dorsal aspect of the wrist, at the scapholunate articulation. It may involve volar aspect of wrist, tendon sheaths and even interphalangeal joints. This study compares the recurrence rates and complications between two methods of treatment widely accepted for ganglion cysts (e.g. Inj. Sclrotherapy vs Threading). This prospective observational study was conducted on patients who presented to the general surgery outpatient department of our institute with ganglion cysts of wrist. A total of 50 patients were included in the study and the study was conducted from January 2016 to June 2017.

Objective: To compare the efficacy of resolution, frequency of complications and recurrence of managing dorsal wrist ganglions with aspiration followed by intra-cystic injection of triamcinolone acetonide versus surgical excision. Study Design: Randomized controlled trial. Place and Duration: This study was carried out in the Department of Plastic and Reconstructive Surgery, Pakistan Institute of Medical Sciences (PIMS), Islamabad during the period from August 01, 2005 to August 31, 2011. Materials and Methods: All adult patients of either gender and all ages with clinical diagnosis of dorsal wrist ganglion were included. Patients with diabetes, malignancy, or bleeding diathesis were excluded. Half of patients were randomly assigned to the aspiration/ injection group and half to the surgical excision group. In the aspiration/ injection group, the ganglion was first aspirated and then 40mg triamcinolone acetonide was injected. In the surgical excision group, ganglion was excised under local anesthesia. The primary outcome measure was successful resolution of ganglion while the secondary outcome measures were complications and recurrence within two years. Results: Out of 60 patients, 78.33% (n=47) were females while 21.66% (n=13) were males. The mean age was 21.88±6.79 years. The duration of the ganglions was 15.28±4.85 months. Successful resolution of the ganglion was achieved in all the patients in both the groups following intervention. In triamcinolone group, mild pain was reported by 23.33%(n=7) patients while recurrence over two years was seen in two patients. (recurrence rate of 6.66%). In the surgical excision group, all the patients experienced moderate pain postoperatively which necessitated oral analgesics for five days, and restricted wrist mobility for 02 week. Recurrence over two years was seen in five patients. (recurrence rate of 16.66%) Conclusion: Aspiration followed by intra-cystic injection of triamcinolone is superior to surgical excision in managing dorsal wrist ganglion. As the aspiration/ injection is associated with a lower recurrence rate and less post procedure morbidity, it should be employed as the first line treatment.
A new method in the management of wrist ganglion in comparison with other traditional methods
A new method in the management of wrist ganglion (Silk thread passed through the ganglion); in comparison with other traditional methods Abstract Objectives: To compare a new method of ganglion management, efficacy of resolution, frequency of complications and recurrence of managing dorsal wrist ganglions with aspiration followed by intra-cystic injection of Methylprednisolone and surgical excision and our new procedure Silk thread passed through the ganglion. Patients and Methods: A total of 785 patients were included in this study,120 cases were excluded from the study because they were lost to follow-up. All patients were thoroughly examined to exclude the other causes of wrist swelling. The patients were divided into three groups A, B &C according to their choice of treatment. Group A were treated by surgical excision, Group B were treated by aspiration followed by steroid injection and group C by Silk thread passed through the ganglion. Baseline Data were recorded preoperatively and postoperative data were obtained at the intervals of 2 weeks, 6 weeks, 3 months, and 6 months. At the end of 6 months the data form was completed and results analyzed. Results: Patient's satisfaction was higher in Group C after Silk thread passed through the ganglion followed by surgical excision even if the ganglions recurred. The recurrence rate was 4 % in Silk thread passed and 24% surgical excision group and 43% in aspiration with steroid injection in group B. Conclusion: In this study Silk thread passed through the ganglion had better results compared to surgical excision and to aspiration and injection of Methylprednisolone.
Management of wrist Ganglia by Aspiration Combined with Intralesional Steroid: A Prospective Study

Background: Ganglion is the most common be- nign soft tissue tumour of wrist and it represents about 60-70% of all such soft tissue tumours. There are various types of treatment options such as observation, aspiration, aspiration combined with intralesional steroids and surgical and arthroscopic excision. Many studies showed that surgical excision has the highest success rate but it has lots of dangers. So this study aimed that Aspiration with intralesional steroid, with its many advantages and success rate, may be the best choice for its treatment. Materials and methods: We conducted the prospective study among 367 patients over a period of January 2017 to December 2018 in Surgery Outpatient Department, Chittagong Medical College Hospital, Bangladesh. Aim and Objective: To compare the effect of aspiration withIntralesional Triamcinolone Acetonide Injection and aspiration alone in dorsal wrist ganglions. Material and Method: This was a prospective study done from Aug 2014 to Aug 2016 in department of general surgery Dr. SusheelaTiwari Government Medical College and Hospital Haldwani, Nainital, Uttarakhand. a total number of 88 Patients with dorsal wrist ganglions were offered treatment in two groups: 1-aspiration alone in 48 patients, 2-aspiration withIntralesional Triamcinolone Acetonide Injectioninto the cyst in 40 patients. Follow up was done at 1, 3, 6 and 12 months. Results: Successful treatment in group 1 was seen in 18 patients (37.5%), and in group 2 in 22 patients(55.%). Conclusion: Injection ofIntralesional Triamcinolone Acetonidein dorsal wrist ganglion along with aspiration was associated with higher rate ofsuccessas compared to that of aspiration alone.

Ganglion cysts are one of the most commonly encountered benign soft tissue swellings around the wrist. Ganglion is also one of the frequently seen lesions in minor surgical practice but still there is little agreement regarding their aetiology and management. Mostly these cysts are asymptomatic. The two main concerns bothering the patients are the cosmetic appearance of the cysts and the apprehension regarding malignant transformation in future. 1 Ganglions actually are benign soft tissue lesions consisting of cyst that is filled with mucin. These cysts are in turn connected to a tendon, tendon sheath or joint capsule. 2 Wrist ganglia cysts usually develop as a result of fluid leak-out which is contained within the surrounding sheath of tendons around the wrist. It becomes a cystic structure that consists of fluid identical to the normal fluid found within ABSTRACT Background: Ganglion is the one of the common soft tissue swellings in the hand and wrist. Treatment options available include reassurance, nonsurgical means like aspiration with or without steroid or hyaluronidase injection and surgical or arthroscopic excision. All current treatment options produce suboptimal results. This study endeavours to compare the outcome and recurrence rate and other complications after treatment of wrist ganglion by aspiration and methylprednisolone injection by double Intravenous cannula versus single needle technique. Methods: A prospective comparative clinical study was done with a total of 72 patients. 36 patients in group 1 were treated with double intravenous cannula technique and 36 patients in group 2 were treated with single needle technique. All the patients were followed up for 6 months to look for recurrence and any complication. The patient's age, sex and various characteristics of the ganglion cyst like side, site, size, etc. were recorded. Results: In group 1, recurrence was seen in 6 patients (15.7%) while in group 2, recurrence were seen in 14 (38.8%) patients. There was statistical significant difference between the two groups with p value of 0.04. No complication was seen in both groups due to methylprednisolone injection. Conclusions: Inspired by the results we advise to use aspiration and methylprednisolone injection by double intravenous cannula technique in the treatment of wrist ganglion before any surgical intervention. It is safe, simple, cost effective method and is less invasive and less time consuming with recurrence rate comparable to surgical and arthroscopic interventions.
Outcome of Dorsal Wrist Ganglion Surgical Excision

Objective: To investigate the clinical outcome of dorsal wrist ganglion treated with surgical excision. Methodology: It was a cross-sectional study performed at Kazi Hospital Lahore. Non-probability purposive sampling technique was used to enroll 50 patients of dorsal wrist ganglion who responded through a questionnaire / research proforma. Surgical excision was done under local anesthesia as day case. Results: We collected data of 55 patients. The mean age of the patients having dorsal wrist ganglion cyst was 31. Female patients were in a larger proportion than male patients. Right wrist was affected in n= 29 (58 %) of the individuals. At the end of our follow up only 2(45) patients had residual pain. Hypertrophic scar has been reported to be associated with the lower patient satisfaction (p = 0.001). Luckily in our study only two patients (4%) had this problem.
