Treatment of Oral Submucous Fibrosis: A Case Report
Background: Oral submucous fibrosis (OSF) is a progressive and inflammatory mucosal fibrosis disease with cancerous tendency. Even though the exact etiology and pathogenesis of OSF have not yet been elucidated totally, the currently recognized pathogenic factor is areca nut. The typical symptoms of OSF patients with limited mouth opening directly affect life quality. Treatments focus on reducing inflammation and improving mouth opening to improve a patient's quality of life. Steroids and corticosteroids are used to treat OSF for a long time because of preventing inflammatory response, reducing fibroblast proliferation, and alleviating collagen deposition. It was also reported that Chinese medicine formulas such as salvia/miltiorrhiza have certain effects in OSF treatment by means of activating blood circulation, removing blood stasis and softening the stiffen oral mucosa. Case Presentation: This case report describes the patient with limited month opening and the 3 years history of chewing areca nuts. The diagnosis of oral submucous fibrosis was confirmed with histopathological examination. With the combined treatment of sublingual compound Danshen dropping pills and topical triamcinolone acetonide for mouth rinse, the patient achieved curative effects of obviously improved mouth opening and soften oral mucosa. And the mechanism of the treatment was discussed. Conclusions: The combined treatment of topical triamcinolone acetonide and sublingual compound Danshen dropping pills may get good effects for OSF patients but need further study.

OSMF is a chronic disease of the oral cavity, which is more commonly found in the patients of south East Asia and is characterized by the build up of constricting bands of collagen in the buccal mucosa and adjacent structures. The precise cause is unknown but chewing of areca nut and its products, excess use of chilies and spices, poor nutrition, vitamin and iron deficiency have been suggested. Treatment holds option of both non-surgical and surgical approach. Non-surgical management includes multivitamin supplements including lycopene and range of medicines (e.g. intra-lesional injections of steroids, hyaluronidase, human placental extracts, chymotrypsin, pentoxyfilline, collagenase). Surgical intervention includes cutting of fibrous bands, which is used in more extreme cases. Heat therapy (thermodynamics) can also be used for resolving sub-mucous fibrosis.

The Oral Submucous Fibrosis is a chronic fibrotic disease that involves the oral mucosa and occasionally it even affects the upper third of the oesophagus. OSMF is characterized by a juxta epithelial inflammatory reaction followed by fibroelastic changes in the submucosa and epithelial atrophy that leads to stiffness of the oral mucosa causing trismus and inability to eat [1]. The etiological factors are excessive consumption of spicy foods nutritional deficiencies like chronic iron and Vitamin B complex deficiency, areca nut chewing habits.
ORAL SUBMUCOUS FIBROSIS – A SHORT REVIEW
Oral Submucous fibrosis (OSMF) is a well-known debilitating precancerous condition of oral cavity which results due to chewing tobacco, areca nuts and its related products. It is an irreversible condition with very high malignant potential. There is a dire need to curb this common but avoidable condition that is increasingly observed in the younger age groups which leads to early development of oral cancer. The purpose of this article is to comprehend OSMF in totality and help the health professionals to get acquainted with the possible etiology, clinical features, differential diagnosis and current treatment modalities of OSMF along with the planning of promotive and preventive research to elucidate the solutions to the problem.

Oral submucous fibrosis (OSMF) is a chronic incidious disease of the oral mucosa characterized by inflammation and a progressive fibrosis of the lamina propria and deeper connective tissues. It is a condition predominantly seen among people of Indian subcontinent and areca nut chewing has been strongly associated with the onset and progression of the disease. Oral submucous fibrosis (OSMF) is a persistent, progressive, pre-cancerous condition of the oral mucosa, which is related with betel quid chewing habit. OSMF is strongly connected with the risk of oral cancer. Studies suggest that the dysplasia is seen in about 25% of biopsied OSMF cases. Hence, it is very important to treat and manage OSMF in early stages. The purpose of this review was to highlight various treatment modalities for OSMF with little emphasis on clinical and histopathological features of OSMF. The literature was searched using "PubMed" and electronic databases from 1994 to 2013. Key words such as OSMF and treatment, treatment modalities for OSMF and management of OSMF were used. A total of 80 articles were retrieved from 1994 to 2014. Among the retrieved articles, only 20 articles were related to medical treatment procedures and four articles reviewed the entire management of OSMF. 10 papers discussed the surgical interventions and the remaining dealt with etiopathogenesis and prevalence of the OSMF. Various types of treatment modalities include conservative methods, medical management, and invasive methods like surgical elimination of the fibrotic bands and combined therapy. Our review of the literature for treatment of OSMF yielded a spectrum of treatment modalities to manage OSMF. Recent literature proves that the combination of drugs produce effective results in the management of OSMF. However, a more extensive clinical trial is necessary to come to a final conclusion about a particular treatment modality in managing OSMF.

Oral submucous fibrosis (OSF) is a well-recognized chronic insidious disease, precancerous condition, autoimmune and collagen related disorder which is multifactorial in origin mainly associated with the practice of chewing betel quid containing areca nut, a habit common among lower socio-economic strata of society. It is characterized by inflammation, increased deposition of submucosal collagen and formation of fibrotic bands in the oral and paraoral tissues, which increasingly limit mouth opening. In the myriad of literature available, numerous etiology and management techniques are put forward by various authors in the past. In this paper, an attempt is made to analyze critically and update the knowledge of the recent developments that enhances the understanding of the etiology of this premalignant condition and its medicinal & surgical management which improves the life expectancy.

Oral sub mucous fibrosis (OSMF) is a chronic, progressive, debilitating, scarring and crippling disorder of the oral cavity. It is characterized by inflammation, increased deposition of sub mucosal collagen and formation of fibrotic bands in the oral tissues, which increasingly limitmouth opening. Many OSMF treatment protocols have been proposed to alleviate the signs and symptoms of the disorder and there is overwhelming evidence that as areca nut is primary cause, stopping its use may have a considerable effect on symptoms rather than reversing pre-existing fibrosis. Even though several treatment modalities have been recommended, only a few are evidence based and can be considered for the optimal management of oral sub mucous fibrosis. The objective of this study was to propose and review treatment protocol to be followed for the management of oral sub mucous fibrosis. We reviewed several evidence-based studies and through this review we recommend topical interventions as the first-line of treatment since they are associated with low risk of systemic side effects.

Oral submucous fibrosis (OSMF) is a premalignant condition mainly associated with the practice of chewing betel quid containing areca nut, a habit common among South Asian people. Data from recent epidemiological studies provide overwhelming evidence that areca nut is the main etiological factor for OSMF. Over the years, many classification systems have been documented in medical literature based on clinical, histopathological, or functional aspects. Each classification has its own merits and demerits. An attempt is made to provide and update the knowledge of classification system of OSMF so that it can assist the clinicians, be beneficial in researches and academics in categorizing this potentially malignant disease for early detection, prompt management, and reducing the mortality.
