Lung separation is the basis of thoracic anesthesia, which is performed by different instruments. Checking probable malpositioning of tracheal tube needs fiberoptic bronchoscopy. The aim of this study was to compare respirator suggested compliance with fiberoptic findings in detecting major tracheal tube malpositioning. Methods: A total of 256 patients undergoing thoracic surgery with double-lumen tracheal tube insertion in Imam Khomeini Hospital, Tehran, Iran, during 2010-11 were divided into three groups (n=86). We used left-sided double-lumen tube (DLT) for left or right-sided surgeries (groups 1 and 2), and right-sided DLT for left-sided surgeries (group 3). The position of the tubes was evaluated and compared using bag compliance versus fiberoptic bronchoscopy. Results: The mean age of the study population was 44.7±13.4 (16-73) years, while 155 (59.9%) were male. The sensitivity, specificity, positive and negative predictive values, and the accuracy of bag compliance test for left-sided DLT in supine position were 40% (95% CI: 20-60%), 99% (95% CI: 96-99%), 84% (95% CI: 54-94%) 92% (95% CI: 88-95%) and 92% (95% CI: 87-95%), respectively. The above-mentioned variables for lateral decubitus position respectively were 27%, 98%, 76%, 89%, and 88%. Malpositioning was more prevalent in right-sided DLTs (P=0.02). Conclusion: Based on the results of this study, and the high specificity, positive predictive value, and accuracy of bag compliance test, its use is encouraged as an alternative to fiberoptic bronchoscopy for checking DLT position, specially, in emergent surgeries or when fiberoptic bronchoscopy is unreachable due to lack of expertise or personnel.

Relationship Between the Mesiodistal Width of Maxillary Central Incisor and Interpupillary Distance

2010

Introduction: Estimating the width of maxillary central incisor may be difficult when an artificial tooth is selected for edentulous patients. One of the ways to choose a good size for this tooth is using extra-oral landmarks in face. This study examined the relationship between mesiodistal (MD) width of maxillary central incisor and interpupillary distance (IPD) which is a stable landmark in lifetime. Materials & Methods: In this descriptive cross sectional study, 100 Iranian subjects (50 males and 50 females, mean age 23±6.3 years) without ocular and dental deformities were selected. The MD width of upper central incisor and IPD were measured three times for each subject using Boley Gauge with 0.1 mm accuracy and then, the mean values were calculated. A mathematic equation to evaluate correlation between MD width of upper central incisor and IPD was calculated and then examined in the other 20 subjects (10 males and 10 females, mean age 24.8±1.56 years) by statistically comparing ...

Prevalence of Cesarean Section and Its Causes in Governmental Obstetric Hospitals of KASHAN-2014

2016

Background & objectives: Cesarean section has important role in reducing mortality rate and complications of delivery in both mothers and fetus. However, high rate of cesarean section is a major issue in the modern obstetrics. According to the World Health Organization, the expected rate of cesarean section should be 10-15 percent in different countries. In Iran, cesarean rate is about 50-65 percent. Given the high prevalence of cesarean section, this study was conducted to determine the prevalence of cesarean and its causes in governmental obstetric hospitals of Kashan in 2014. Methods: This cross-sectional study was conducted on 5751 births in two obstetric centers of Kashan in 2014. Required information was obtained from the data recorded in the mentioned two delivery centers. Cesarean rates were obtained in relation to the total number of births in the two centers, and reasons for cesarean section were extracted from patient’s records and obtained information was entered in a ch...

Evaluating the Effect of Dill (Anethum graveolens L.) SeedEssence on Pain and Vital Sign of Cesarean Delivery with SpinalAnesthesia

Journal of Ardabil University of Medical Sciences, 2017

Background & objectives: Pain is one of the most common post-operative complications of cesarean section, which is very important for mother in nursing a baby and breastfeeding. Finding ways to overcome this pain has always been a concern for researchers. Considering the application of plants in traditional medicine as sedatives, this study evaluated the effect of Dill seed oil on post-operative pain in patients with spinal anesthesia. Methods: This double-blind, clinical trial was conducted on pregnant women who met the inclusion criteria and referred to Hajar hospital in Shahrekord, Iran during 2015-2016. By simple random sampling, the patients were divided into two groups. In the first group, 10 cc Dill (Anethum graveolens L.) seed oil was prescribed at intervals of half an hour and one hour before spinal anesthesia and half an hour , one hour and two hours after spinal anesthesia. In the second group, placebos (standard treatment) were prescribed at the same intervals. Pain and ...
