Prevalence and Nature of Sexual Violence in Older Adults in Europe: A Critical Interpretive Synthesis of Evidence

ABSTRACTSexual violence (SV) is an important public health issue with a potential major impact on victims and their peers, offspring and community. However, SV in older adults is under-researched. This paper aims to establish the prevalence and nature of SV in older adults in Europe, link this with existing policies and health care workers’ response to sexual health needs in older age and critically revise the current used frameworks in public health research.We applied a Critical Interpretative Synthesis. After the first phase of purposive sampling we included 14 references. Another 14 references were included after the second phase of theoretical sampling. We ultimately included 16 peer-reviewed articles and 12 documents from the grey literature.0.0% to 3.1% of older adults in Europe were sexually victimised in the past year. The issue of domestic violence against the elderly emerges as a serious and growing public health problem and suggests the need for a greater field of intervention in this area, given the supposed risk to which this older population is subjected. This work aims to propose interventions in Primary Health Care that impact the reduction of violence and abandonment rates against the elderly, proposing actions through health education for the elderly and their family members/caregivers assisted by a family health team (ESF) in the municipality of Juazeiro-BA. Thus, it is an extension project developed by medical students at Faculdade Estácio de Juazeiro, which proposes to carry out meetings with the elderly and their families through workshops that bring relevant information about violence against the elderly. Finally, it is expected that this work can contribute to reducing the rates of violence in the intervention area, as well as sensitizing professionals and local managers on the subject.

Study design, sampling and assessment methods of the European study 'abuse of the elderly in the European region

Violence against and abuse of older persons (VAO) aged>60 years has become a prominent public health issue. From January 2009-July 2009, we conducted the cross-sectional European study 'Abuse of the elderly in the European region' (ABUEL) among community-dwelling elderly populations aged 60-84 years in Germany, Greece, Italy, Lithuania, Portugal, Spain and Sweden. We describe the cooperation, completion and response rates; the modes of recruitment and administration; and analyse differences in response rates between countries. We calculated the population fraction (respondents in each age/sex group divided by the population in the same age/sex group) and the population fraction ratio (PFR) to describe and analyse heterogeneity between countries. European report on preventing elder maltreatment. WHO

Elder maltreatment is pervasive in all countries in the WHO European Region, and estimates suggest that at least 4 million people in the Region experience elder maltreatment in any one year. Most countries in the Region have an ageing population, and one third of the population is forecast to be 60 years and older in 2050, putting more people at risk of elder maltreatment. Elder maltreatment has far-reaching consequences for the mental and physical well-being of tens of millions of older people, and if left unchecked will result in their premature death. Estimates suggest that about 2500 older people may lose their lives annually from elder maltreatment. The report highlights the numerous biological, social, cultural, economic and environmental factors that interact to influence the risk and protective factors of being a victim or perpetrator of elder maltreatment. There is some evidence of effectiveness, and examples include psychological programmes for perpetrators and programmes designed to change attitudes towards older people, improve the mental health of caregivers and, in earlier life, to promote nurturing relationships and social skills learning. The evidence base needs to be strengthened, but much can be done by implementing interventions using an evaluative framework. Prevention and social justice for older people can only be achieved by mainstreaming this response into health and social policy. Surveys show that the public and policy-makers are increasingly concerned about the problem, and the policy response needs to be strengthened to meet this demand.
