Chronic pelvic pain and quality of life in women with and without endometriosis

To determine the impact of chronic pelvic pain on quality of life in women with or without endometriosis Methods: A total of 162 women with chronic pelvic pain were enrolled in a cross-sectional study and divided into two groups: 84 with endome triosis, 78 without endometriosis. Assessment of dysmenorrhea, non-menstrual pain, dyspareunia and diffuse abdominal pain by a visual analog scale, assessment of quality of life by SF-36. Results: The endometriosis group showed more dysmenorrhea but less dyspareunia and nonmenstrual pain than women affected by other diseases. Pain had a negative influence on quality of life in both groups, but the endometriosis group showed a greater negative impact in all areas of SF-36, yet only in three (physical role, physical pain and mental role) was the difference statistically significant. Conclusions: Endometriosis impacts quality of life more than other forms of chronic pelvic pain. Interventions aimed at improving quality of life specifically designed for patients with chronic pelvic pain in general, and with endometriosis in particular, should be developed, tested, and integrated into the routine care regimen provided for these patients.

Application of the World Health Organization Quality of Life Instrument, Short Form (WHOQOL-BREF) to patients with endometriosis

Objective Endometriosis, a common hormone-dependent chronic inflammatory disease causes various symptoms including pelvic pain, which affect the physical and psychological quality of life in women of reproductive age. The present study aimed to assess the quality of life of Iranian women with endometriosis using the World Health Organization Quality of Life Instrument, Short Form (WHOQOL-BREF) questionnaire. Methods This cross-sectional study performed in Tehran between October 2014 and July 2016 included 70 Iranian women with endometriosis. The Iranian version of the WHOQOL-BREF questionnaire was used to measure quality of life in these women. The Pearson's correlation coefficient, the paired and the independent t-tests, and a linear regression model were used to statistically analyze the data using the SPSS software version 16.0. Results The total mean score of the WHOQOL-BREF questionnaire was 80.58. The highest and the lowest mean scores were observed in the environmental (28.15) and the physical health domains (10.59), respectively. A multiple linear regression model (backward method) showed that insurance coverage was associated with the total WHOQOL-BREF score, whereas age, insurance coverage, and income level were associated with domains 1 through 4, respectively (P<0.05). Conclusion Endometriosis affects various aspects of the quality of life of Iranian women who therefore require holistic management focused on both, the physical and psychological aspects of treatment.

Evaluation of Quality of Life and Sexual Satisfaction in Women Suffering from Chronic Pelvic Pain With or Without Endometriosis

Introduction. Chronic pelvic pain (CPP) is one of the most frequent symptoms in women of reproductive age. This is an enigmatic clinical condition that results from the complex interactions of physiological and psychological factors with direct impact on the social, marital, and professional lives of women. Aim. To evaluate the quality of life and sexual satisfaction of women who suffer from CPP with or without endometriosis. Method. Forty-nine patients who had been diagnosed with endometriosis and 35 patients with CPP diagnosed with another gynecological condition, all 84 of whom were treated at the Chronic Pelvic Pain and Endometriosis Clinic at Universidade Federal de São Paulo (UNIFESP) from January to July of 2008. The controls were 50 healthy women from the Family Planning Clinic at UNIFESP. Main Outcome Measures. World Health Organization Quality of Life Assessment-Bref (WHOQOL-BREF) quality of life questionnaire and the Golombok-Rust Inventory of Sexual Satisfaction (GRISS). Results. No statistically significant differences were observed between the groups with CPP symptoms, in either the results from the WHOQOL-BREF or in the GRISS questionnaire. In both questionnaires, differences were observed when the two groups of symptomatic women were compared with the group of healthy women. Conclusion. CPP caused by endometriosis or other gynecological conditions leads to a significant reduction of quality of life and sexual satisfaction. Psychological Approach to Endometriosis: Women’s Pain Experience and Quality of Life Improvement

Endometriosis is a chronic condition affecting 10 to15% of women in childbearing age. Understanding the impact of this disease on women's well-being is still a challenge, namely to intervene. Pain is the most current and troublesome symptom. Although medical treatments for pain relief are effective, recurrence rate remains significant, calling for a better understanding and development of new approaches for pain management. A group Cognitive Behavioral Therapy (CBT) for management of associated co-morbidities is suggested, paying special attention to Chronic Pelvic Pain (CPP). CBT design can be grounded on information collected from focus groups and a one-group exploratory trial. Evaluation of therapy effectiveness is possible to be performed by comparing group CBT to Usual Care (UC) and Support Group (SG) in a randomized controlled trial. Research in this area could represent an important step in providing a solution to the management of endometriosis and, to the best of our knowledge, the first national psychological approach for its understanding and treatment. Endometriosis is a chronic, inflammatory disease affecting more than 170 million women worldwide and up to 10% of women of reproductive age. Toward an improved assessment of quality of life in endometriosis: evaluation of the Spanish version of the Endometriosis Health Profile 30

Purpose: Our main objective is to evaluate the psychometric properties of the Spanish version of the EHP-30 questionnaire. The secondary aim is to evaluate the differences in the scores of the core EHP-30 scales between patients with either surgical treatment or conservative management of endometriosis. Methods: Cross-sectional study conducted into a tertiary hospital endometriosis reference unit. All patients (n ¼ 223) pre-surgically completed the core EHP-30 questionnaire, the EQ-5D questionnaire (n ¼ 184) and a visual analogue scale (n ¼ 210) for endometriosis-related pain. Demographical and clinical data were recorded. Results: Psychometric characteristics of the Spanish core EHP-30 questionnaire were investigated. Statistical analyses confirmed the five-structure factor, a high degree of internal consistency and of item-total correlation for all the assessed items. Convergent validity between EQ-5D and EHP-30 items and between VAS and EHP-30 subscale pain was observed. Additionally, patients with surgical management rendered significantly higher scores in the core EHP-30 subscales "pain" and "control and powerlessness". Conclusions: We present the reliability, validity and acceptability of the Spanish core EHP-30 questionnaire, providing clinicians and researchers with an improved tool to assess the endometriosis-related quality of life. Additionally, we show that patients subsidiaries of surgical treatment for endometriosis present with higher pain and powerlessness than those with conservative management.

A health-related quality-of-life instrument for symptomatic patients with endometriosis: A validation study

OBJECTIVE: This study was designed to evaluate the reliability, validity, and responsiveness of a newly developed, health-related quality-of-life measure. STUDY DESIGN: A total of 137 women (122 from a Phase III clinical trial and 15 from a private practice setting) with endometriosis completed the questionnaire several times. RESULTS: Reproducibility and internal-consistency reliability were acceptable with intraclass correlation coefficients ranging from 0.94 to 1.00 and Cronbach's α coefficients ranging from 0.84 to 0.97. Construct validity was demonstrated on the basis of correlations between items and scales. Health-related quality of life varied in a consistent manner according to clinician-rated measures of pelvic pain and dysmenorrhea and patient-reported levels of endometriosis pain, but no relationship emerged according to the revised American Fertility Society classification. In general, the questionnaire was moderately to highly responsive to change. CONCLUSION: This is the first comprehensive health-related quality-of-life questionnaire available for use with endometriosis patients that has demonstrated reliability, validity, and responsiveness.

Evaluation of Quality Of Life in Endometriosis Patients Before and After Surgical Treatment Using the EHP30 Questionnaire

Backround: Endometriosis is one of the most common gynaecological illnesses causing extensive psychological, physical and social impact on patient´s life and exerts negative effects on health-related quality of Life (HRQoL).
