PURPOSE: To use anterior segment optical coherence tomography (AS‐OCT) to evaluate anterior chamber depth (ACD) and angle after phacoemulsification and intraocular lens (IOL) implantation. SETTING: Department of Ophthalmology, Yeditepe University, Istanbul, Turkey. METHODS: Forty‐seven eyes (37 patients) had uneventful phacoemulsification and IOL implantation through a clear corneal incision. Anterior segment OCT was performed preoperatively and 1 week and 1 month postoperatively. The angle‐referenced (ACD1), pupil‐referenced (ACD2), and lens‐referenced (ACD3) ACDs, crystalline lens rise (CLR), pseudophakic posterior chamber depth (PPCD), and nasal and temporal iridocorneal angles were measured. This study aimed to evaluate the effect of uneventful phacoemulsification on central macular thickness (CMT) using spectral-domain optical coherence tomography (SD-OCT) of macula and its correlation with effective phacoemulsification time (EFT) in healthy eyes. A prospective interventional, non-comparative study of 30 eyes of 28 patients who underwent uneventful cataract extraction at Research Institute of Ophthalmology, Giza Egypt between April 2015 and April 2016. The main outcome measures were EFT, pre and 1 month post-operative CMT, Best-corrected visual acuity. Results revealed that thirty eyes of 28 patients (16 males and 12 females) were included in the study with mean age of 58.57 ± 5.13 years (range: 48 – 70 years). Central macular thickness (CMT) changed from 260.40 ± 19.09 μm preoperatively (Range = 214 – 310 μm) to 279.00 ± 59.80 μm postoperatively (Range = 214 – 572 μm) with a statistically significant CMT change (p = 0.043). Purpose Cataract surgery is known to elicit postoperative macular oedema owing to its traumatic effect. The aim of the study was to measure the foveal and perifoveal thickness of the retina after uneventful phacoemulsification and posterior chamber intraocular lens (PC-IOL) implantation. Methods Retinal thickness values of the foveal and perifoveal (fovea þ 3.0 mm, fovea þ 6.0 mm) sectors and the minimum values were measured in a prospective study using optical coherence tomography (OCT) in 71 eyes of 71 patients (34 men, 37 women, mean age: 68.8 years) who underwent cataract surgery. Phacoemulsification and 'in the bag' implantation of a foldable PC-IOL was performed under topical anaesthesia. Data were collected 1 day before the operation, and postoperatively at 1, 7, 30, and 60 days. Contralateral eyes of each patient served as controls. 'Student's t-test' was used for statistical analysis, and Po0.05 value was considered significant.
Changes in central corneal thickness before and after phacoemulsification cataract surgery

Aim: To evaluate the effect of an uneventful phacoemulsification surgery on Central Corneal thickness (CCT). Methods: 77 consecutive patients who had an uneventful phacoemulsification surgery by the same experienced surgeon were prospectively evaluated for Intraocular Pressure (IOP) and CCT measurements 1 day preoperatively and 1 day, 1 week and 1 month postoperatively. Result: A marked rise was seen in the mean CCT measured within 24 hours after surgery (583.22 μm), which gradually decreased at one week (552.57 μm) and returned to normal baseline values (539.9 μm) in almost all operated eyes in 1 month time. There is no statistically significant difference in the mea n CCT before surgery (538.23 μm) and at 1 month (p=0.685). Conclusion: The mean CCT substantially increases after post-operative uneventful cataract surgery but returns to normal baseline values in almost all operated eyes in 1 month time. BACKGROUND Phacoemulsification for cataract is the standard surgery in recent times with very good results and least complications. Macular area is the most important part of the retina, which is very sensitive for any type of insult on the Eye. It takes different durations for the macula to return to normal condition after phacoemulsification. We wanted to evaluate the macular thickness change occurring after uncomplicated phacoemulsification cataract surgery in postoperative period by using spectral-domain optical coherence tomography. METHODS In this prospective study, a total of 100 patients who underwent uncomplicated phacoemulsification cataract surgery were evaluated. Complete ophthalmological examination was done preoperatively. Patients were followed on day 7, week 4 and week 8 postoperatively. At each visit a complete ophthalmological examination was done and optical coherence tomography of macula was performed. RESULTS In the current study, we found a significant subclinical increase in central macular thickness at postoperative week 1, week 4 and week 8. Cystoid macular oedema was not noted in any of the patients. A statistically significant increase in foveal volume was observed on day 7, week 4 and week 8 (p value 0.001). CONCLUSIONS There was increase in central macular thickness and foveal volume after the uncomplicated phacoemulsification cataract surgery on day 7, week 4, and week 8 postoperatively. The increase in macular thickness was sub-clinical and did not affect final visual outcome in any patient. Further evaluation is required to trace its return to preoperative value.

Aim:To determine the role of AS-OCT to detect posterior capsular dehiscent preoperative. Design:Prospective hospital based observational study. Material and Method: Patient collection in Government college Kannauj March 2018 to February 2019 .According to Helnsinki's declaration preoperative assessment and AS-OCT is also performed .Phacoemusification with PCIOL and follow up was done upto 6 week. Result: AS-OCT done for 26 eye out of which in 4 (15.3%) eyes posterior capsule was found dehiscent rest of all eyes posterior capsule was intact. Examination under microscope surgeon found posterior capsular dehiscent only in 3(11.5%).Sensivity of AS-OCT is 75% and specificity is 95.45%.Stastical analysis was done for AS-OCT by CHI SQUARE test (P=.0004). Conclusion: Significant role of detecting AS-OCT in posterior capsular dehiscent.

To determine the role of anterior segment optical coherence topography (AS-OCT) preoperatively to detect pre-existing weakness of the posterior capsule in patient of posterior polar cataract (PPC) effecting outcomes during phacoemulsification. Materials and Methods: A prospective observational study was done in 26 eyes with clinically diagnosed PPC who underwent phacoemulsification between November 2018 to December 2019. AS-OCT was done preoperatively to detect pre existing dehiscence of posterior capsule in patient of PPC. The images obtained by AS-OCT were divided into either intact or dehiscent. Integrity of posterior capsule was also observed during operation by surgeon. Results: In 26 eyes, dehiscence of posterior capsule was detected in 4 eyes (15.4%) by AS-OCT pre operatively, of posterior capsule in 3 eyes (11.5%) and intact posterior capsule in 23 eyes (88.5%). Rupture of posterior capsule occurred in 4 eyes during phacoemulsification. 3 out of 4 eyes who had rupture, had dehiscence of posterior capsule that was noted by AS-OCT and also intraoperatively while 1 eye was detected to have intact posterior capsule. The sensitivity of AS-OCT was 75% for detecting posterior capsule dehiscence and specificity was 95.65%. Conclusion: AS-OCT can be used preoperatively to identify posterior polar cataract that are at risk of posterior capsule rupture. This provides preoperative information to surgeon and helps in proper counselling of patients about the prognosis. Background: Among adults as cataract being the most common cause of the correctable diminution of vision. There is 12.5 million blind, in India and blindness due to cataract is estimated to be 50% to 80%. Due to cataract, an additional 3.8 million become blind each year in addition to backlog against which cataract surgeries are done every year are 2.7 million. Two main approaches are there for cataract extraction 1)manual small incision cataract extraction. 2)phacoemulsification. Corneal astigmatism is a type of refractive error where point focus is not formed leading to unequal refraction. This plays an important role in compliance with post-cataract extraction surgery. Since the cornea is responsible for 70% of eyes refractive power, its examination by corneal topography is of clinical importance in determining the quality of vision. Objectives: 1) To study visual acuity changes between sics and phacoemulsification, 2) To study mean astigmatism between phacoemulsification and SICS.3)To estimate the net induced astigmatism and its association with refraction between SICS and phacoemulsification. Methods: A observational interventional study with 120 patients will be conducted in a hospital based setting utilizing the data of patients eye. Preoperative visual acuity, astigmatism and postoperative visual acuity and corneal astigmatism are compared in two groups. Expected Results: As compared to other studies present we expect to find to have a better visual outcome, the reduction is astigmatism by 6 weeks and less surgically induced astigmatism in the phacoemulsification group than Sics.
