Family Medicine Department Chairs’ Opinions Regarding Scope of Practice

physicians are trained broadly to provide the majority of health care for patients of all ages and conditions. While providing such a wide range of care remains at the heart of the specialty, the scope of practice of many family physicians has been shrinking. 1-4 Some believe that a broad scope of practice is becoming more of an ideal than a reality. 5 Despite these changes, family medicine residency training requirements specify that "teaching and role-modeling by family physician faculty" should cover the full spectrum of care. 6 Further, each program must have at least one family physician faculty member who is engaged in obstetrical practice and can supervise and role model this service to residents. However, no specific requirements exist for faculty to role model home visits, nursing home care, or many of the other services traditionally provided by family physicians.

Perry Dickinson, MD: We are proud to have as our presenters today a group of our most outstanding leaders in family practice research, each an active member of this organization. They include the director or codirector of the three family practice research centers funded by the American Academy of Family Physicians (AAFP) and the director of the AAFP Center for Policy Studies in Family Practice and Primary Care. Each presenter will add an interesting perspective to the topic for this morning's plenary.

Strong departments of family medicine in academic medical centers help assure the future scope and quality of family practice patient care, the ongoing evolution of family medicine as a scholarly discipline, and a continued flow of qualified medical school graduates into family practice residency programs and eventually into practice. This report presents key strategies of six successful departments of family medicine and describes the methods and skills considered important by the leaders of these departments. Common themes that emerge are (1) recruit and mentor the best faculty, (2) build a reputation for clinical excellence of faculty and residents, (3) become part of schoolwide curriculum activities, (4) establish a scholarly presence, and (5) develop networks of support.
