Urban and rural patterns of bicycle helmet use: factors predicting usage

Objectives-To document current bicycle helmet use in Winnipeg, Manitoba and nearby rural communities, and to identify target groups for a helmet promotion campaign. Methods-Cyclist helmet use was observed between 28 May and 20 August 1996 at a sample of urban and rural locations. Age, gender, helmet use, riding companion(s), location type, correct helmet use, and use of headphones were recorded. Univariate and multivariate analyses were performed. Adjusted odds ratios with 95% confidence intervals were calculated from the final models. Results-Altogether 2629 cyclists (70% male, 30% female) were observed: 2316 at 183 urban locations and 313 at 25 rural locations, with nearly equal numbers of children and adults observed. Overall helmet use was 21.3%, with lower use in males (18.9%) than females (26.3%), despite gender only being a significant variable on multivariate analysis for children under 8 years and adults. Urban helmet use was considerably higher (22.9%) than rural use (8.9%). Helmet use increased linearly as mean neighbourhood income increased, with a nearly fourfold diVerence in use between the highest and lowest income neighbourhoods. Children less than 8 years old and adults had the highest, and teenagers the lowest, use. Significant predictive variables were identified separately by age category to inform targeted programming. Conclusions-We documented low helmet use in our region, emphasizing the need for a regional helmet promotion campaign as well as future helmet legislation. A marked urban-rural diVerence in helmet use that has not been previously reported was also identified. Target groups for a future campaign include adolescents, males, rural cyclists, and those in lower income neighbourhoods. (Injury Prevention 1999;5:183-188)

Bicycle Helmet Wearing in a Sample of Urban Disadvantaged Primary School Children.

Bicycle helmet wearing is currently not legally enforced in Ireland and little is known about the self-reported practice amongst young children. The principal aim of this study was to assess self-reported helmet wearing amongst a sample (n=314) of primary school children (aged 8-13 years) attending disadvantaged schools in Dublin. Approximately 86% of the sample owned a bike and provided a response to the question on helmet use. The findings indicate that helmet wearing is not a widespread practice (50.4%; 136/270 report never wearing helmets). As children get older, reported practice is also less likely with 67% (27/40) of 12/13 year-olds compared to 38% (31/81) of 8/9 year-olds reporting never wearing protective headgear. Regardless of age, more girls (61%; 82/135) than boys (39%; 52/135) indicated always/sometimes using helmets when cycling. Conversely, the findings show that (mandatory) seatbelt wearing is standard practice for the majority (93%; 252/270). The findings relating to helmet wearing add further to the debate around the mandatory introduction of protective headgear for cyclists.

A descriptive study of bicycle helmet use in Montreal, 2011

The purpose of this study was to describe bicycle helmet use among Montreal cyclists as a step towards injury prevention programming. Using a cross-sectional study design, cyclists were observed during 60-minute periods at 22 locations on the island of Montreal. There were 1-3 observation periods per location. Observations took place between August 16 and October 31, 2011. Standard statistical methods were used, unadjusted and adjusted odds ratios and 95% confidence interval were calculated. A total of 4,789 cyclists were observed. The helmet-wearing proportion of all cyclists observed was 46% (95% CI 44-47). Women had a higher helmet-wearing proportion than men (50%, 95% CI 47-52 vs. 44%, 95% CI 42-45, respectively). Youth had the highest helmet-wearing proportion (73%, 95% CI 64-81), while young adults had the lowest (34%, 95% CI 30-37). Rates of bicycle helmet use in an affluent Michigan County

Bicycle helmet use in the United States has remained low despite clear demonstration of its beneficial effect on reducing the incidence of serious head injury. Several interventions have been reported, with variable results and costs. Much of the recent literature has focused on child cyclists and on demographic factors associated with helmet use. This paper reports on helmet use by children and adults in a sample of 652 riders in an affluent southeast Michigan region, chosen to minimize the effect of previously recognized socioeconomic negative predictors that are not readily changed by intervention. Subjects were classified by age, sex, location, riding surface, type of bicycle, child bicycle seat use, child bicycle trailer use, and helmet use by companions.

Psychosocial factors associated with the use of bicycle helmets among children in counties with and without helmet use laws

Studies have demonstrated that bicycle-related head injuries are a serious threat to children and that bicycle helmets are a readily available, effective preventive measure. 14 In the absence of specific promotional campaigns, estimates of helmet use range from 0% to 13% by self-report 5-8 and 0% to 19% by observational studies. 9"12 A community-wide bicycle helmet campaign in Seattle, Wash., 9 documented an increase in helmet use from 5% before the campaign to 16% afterward (compared with an increase from 1% to 3% in a comparison community). Prevalence and predictors of bicycle helmet use in a southeastern, US city

Fatalities and head injuries from bicycle-related crashes remain a concern in the United States. Despite legislation in many states, helmet use remains low. This observational study examined the helmet use and related factors in a North Carolina city. The sample consisted of 2088 observations of bicyclists. The objectives were to (1) determine helmet use; (2) describe other safe bicycling practices; and (3) examine the relationship of demographic variables and safe riding practices with helmet use. Helmet use was observed for 25% of the sample. Demographic factors related to helmet use were being female (OR = 1.32), 26 years old or older (OR = 4.94), and White (OR = 2.17). Bicyclists riding on the road with traffic were more likely to wear a helmet than bicyclists riding on the sidewalk (OR = 2.04). Findings indicate that helmet use remains low in the city. Research to monitor, better understand, and promote helmet use is needed.
