Background To characterize the diagnostic role of swept source anterior segment optical coherence tomography (SS-ASOCT) in strabismus re-operations with lost surgical details. Methods Patients presenting to our outpatient services for strabismus care from February 2020 to March 15th 2021 were systematically evaluated. Those willing to undergo re-operation were included in the study. A total of 30 patients were identified and out of this a total of 10 eyes of 10 patients with lost surgical details were finally included in the study. Demographic details, preoperative deviation, ocular findings, orthoptic measurements, detailed SS-ASOCT findings, intraoperative measurements, and post-operative outcomes were recorded. Results The mean age of 10 patients was 23.7±5.75 years. Seven were male and three were female. Evaluation of macular thickness change after inferior oblique muscle recession surgery

This study aimed to evaluate the changes in macular thickness following inferior oblique muscle recession surgery. Materials and Methods: Thirty-eight eyes from 21 patients undergoing ocular muscle surgery were included. Patients were grouped into three groups based on the type of surgical intervention: Group I (n = 12), inferior oblique recession surgery alone; Group II (n = 12), inferior oblique plus horizontal muscle surgery; Group III (n = 14), horizontal muscle surgery alone. Each eye was scanned using the optical coherence tomography (OCT) device preoperatively and on the first postoperative day to measure macular thickness. Results: Following surgery, a significant increase in foveal thickness occurred in Group I (P < 0.05) and Group II (P < 0.01). In addition, a statistically significant difference was observed between the groups with regard to the increase in foveal thickness (P = 0.016), with significantly lower changes in Group III. Conclusion: Our findings suggested that inferior oblique muscle recession surgery is associated with an increase in macular thickness.

To evaluate the accuracy of the anterior segment optical coherence tomography (AS-OCT) in measuring the distance from the limbus to the insertion of primary and previously operated rectus muscles in children. In this prospective, double masked, observational study the distance of the extraocular muscle insertion from the limbus measured by AS-OCT preoperatively was compared to intraoperative measurement using the surgical calipers. Consecutive patients 4-18 years of age undergoing primary or repeat strabismus surgery on horizontal or vertical rectus muscles between September 2013 and May 2014 were included. Patients with any condition that interfered with imaging were excluded. Participants were asked to look in the direction opposite to the muscle to ensure that the middle third of the muscle was being imaged and measured. Optical Coherence Tomography Findings in Highly Myopic Eyes following Cataract Surgery

To study optical coherence tomography (OCT) findings in high myopic patients with the history of uncomplicated cataract surgery Methods: The sample included 34 eyes of 24 highly myopic patients with an axial length (AL) of 26 mm or more and a spherical equivalent (SE) of 8.0 diopter (D) or more who had uncomplicated phacoemulsification cataract surgery. OCTs were done in multiple sections around optic disc and fovea. Results: The mean age of the 24 patients was 58.9±12.9 years, and the mean SE was 14.5±5.8 D. The mean time gap between cataract surgery and OCT studies was 15.6±18 months, and the mean AL was 29.1±2.1 mm. The most common OCT findings were vascular microfolds (67.6%) and paravascular retinal cysts (64.7%). Vascular microfolds were found significantly associated with age. Internal limiting membrane (ILM) detachment was significantly associated with gender (female group). Peripapillary choroidal cavitation had a significant direct association with postoperative elapsed time in all ages. Conclusion: Vascular microfolds and paravascular retinal cysts are the most common pathologies in myopic (aphakic and pseudophakic) persons. ILM detachment is more common in females. Peripapillay cavitation incidence is associated with postoperative elapsed time.
Evaluation of macula in high myopic fundus using optical coherence tomography

High myopia is a risk factor for a number of retinal pathologies. OCT is very useful in eyes with high myopia and allows the detection of subtle macular changes that are otherwise undetectable. Aim: To evaluate relationship between the axial length, refractive error and macular thickness in subjects with high myopia. Study Design: Cross sectional observational single centric study. Materials and Methods: 50 diagnosed patients of high myopia(refractive error>= -6 dioptres and/or axial length>=24 mm) along with a control of 50 emmetropic patients were included. Patient’s anterior segment evaluation was done by slit lamp. Detailed fundus evaluation was done with indirect ophthalmoscopy and with +78 Dioptre lens along with measuring BCVA (Best corrected Visual Acuity). Axial length was measured using contact biometry. OCT was performed with TOPCON SD-OCT. Average macular thickness, central foveal thickness and total macular volume measured using 3D macula scan of all the cases and controls. Statistical Analysis: Unpaired t test. Results: The mean axial length was 27.40 mm among cases (SD0.99) and 22.84 mm among controls (SD0.422). The mean macular volume in cases was 6.90 and in controls was 7.82. The macular volume decreased with increasing axial length (r=-0.673) and increasing spherical equivalent (r=- 0.035). The average macular thickness was inversely correlated with axial length (r=-0.605) and directly correlated with spherical equivalent (r=-0.049).The mean central subfield foveal thickness was 170.10 mm amongcases and 158.34mm among controls. The axial length and central foveal thickness were found to be positively correlated (r=0.727). Conclusion: Average macular thickness and total macular volume are inversely correlated to the degree of myopia, whereas central foveal thickness is positively correlated to the degree of myopia.
Macular surgery using intraoperative spectral domain optical coherence tomography

Purpose: To report the use of intraoperative spectral domain optical coherence tomography (SD-OCT) for detecting anatomical changes during macular surgery. Methods: In a consecutive case series, 32 eyes of 32 patients undergoing concurrent pars plana vitrectomy and intraoperative SD-OCT for macular hole (MH), epiretinal membrane (ERM) and vitreomacular traction (VMT) were enrolled. Intraoperative changes in retinal thickness and dimensions of the macular hole were measured in patients with ERM and VMT following surgical manipulation using a hand-held SD-OCT device (iVue, Optovue Inc., Fremont, CA, USA). Results: SD-OCT images of sixteen eyes with macular hole were subjected to quantitative and qualitative analysis. All MH dimensions remained stable during consecutive stages of surgery except for MH apex diameter, which showed a significant decrease after internal limiting membrane (ILM) peeling (P=0.025). Quantitative analysis of ten patients with ERM showed a significant decrease in retinal thickness after membrane removal (P=0.018) which did not remain significant until the end of the procedure (P=0.8). In three cases, subretinal fluid was formed after ILM peeling. Quantitative analysis of five patients with VMT showed a decrease in retinal thickness during consecutive steps of the surgery, although these changes were not significant. In two cases, subretinal fluid was formed after ILM peeling. Conclusion: Intraoperative SD-OCT is a useful imaging technique which provides vitreoretinal surgeons with rapid awareness of changes in macular anatomy during surgery and may therefore result in better anatomical and visual outcomes.
