Epidemiological aspects of developmental disorders in school aged children
Multiple factors infl uence the discrepancy between the prevalence rates of developmental disorders worldwide. In addition to different prevalence rates of developmental disorders, there are differences in terminology and their classifi cation. The purpose of the research was to estimate the prevalence and gender distribution of developmental disorders and to calculate the number of children with developmental disorders and special educational needs in a convenience sample of school-age children. We also calculated the number of children with congenital malformations and diseases that affect vision and hearing. We processed the data from medical records of 1750 children (835 males and 915 females) who were examined in the Pediatric Dispensary, Military Hospital, Skopje, Republic of Macedonia during the period of June 30 1992 to June 30 2011. Mental retardation (MR) is a manifestation of a heterogeneous set of impairments and conditions that result in cognitive limitation. It is a condition of medical, educational, and social importance. Physicians identify profound, severe, and moderate MR but rarely diagnose mild MR unless it is associated with a genetic or medical syndrome. From a medical perspective, the quest for etiology and the possibility of medical or surgical intervention to minimize deterioration are paramount. Educators, on the other hand are less concerned with causation than with academic achievement and school success. The majority of cases of mild MR is identified in school settings. Finally, the public uses the label to describe poor adaptive skills. Adults with MR who hold jobs, live independently, and participate in society are not always described as having MR. Thus some individuals characterized in childhood or adolescence as having mild MR become indistinguishable from the general population in adulthood. There are numerous definitions of MR but the two most widely used in textbooks and research articles are the American Association on Mental Retardation [AAMR] (AAMR, 2002), recently revised and most often used in the United States, and the International Classification of Diseases (ICD-10) (World Health Organization [WHO], 1992) the most widely used classification in other countries of the world. The AAMR definition is a practical tool for the determination of service eligibility; the ICD-10 allows for levels of disability and is based on a medical model of classification.

The aim of this paper was to summarize the main results of the POMONA-ESP project, the first study to explore health status in a large representative, randomized and stratified sample of people with intellectual developmental disorders in Spain. The POMONA-ESP project collected information about the health of 953 individuals with intellectual developmental disorders. Diseases such as urinary incontinence, oral problems, epilepsy, constipation or obesity were highly prevalent among the participants; with gender-differentiated prevalences for certain conditions, and age and intellectual disability level as risk factors for disease. Overmedication was common in the sample, and drugs were often prescribed without any clinical indication or follow-up. Review of cluster analysis of phenotypic data in Autism Spectrum Disorders: distinct subtypes or a severity gradient model?, 2020
Background: Individuals with autism spectrum disorder (ASD) form a heterogeneous group, posing a challenge for clinical definition. Additional problems regarding the diverse clinical presentation arise from changes in diagnostic criteria according to the latest Diagnostic and Statistical Manual of Mental Disorders (DSM-5), with exclusion of individuals who met earlier criteria or inclusion of more than previously. Objectives: To investigate studies that have attempted to reduce the heterogeneity of ASD based on cluster analysis of phenotypic data and to clarify whether ASD should be interpreted as 'a unitary spectrum,' with a severity gradient, or defined by distinct subtypes. This will allow better understanding of the disorder with implications for its treatment and prognosis. Methods: A literature search was made through PubMed, Researchgate and Google Scholar for studies of ASD populations. In addition, reference lists from identified studies were reviewed. Results: Only 10 studies were found that dealt with the heterogeneity of ASD and its different subtypes, based on the review prerequisites. Most of the studies appear to support the existence of subtypes within ASD, but it remains unclear whether these are considered as different specific subtypes with characteristic profiles of symptoms or as a part of a severity gradient across symptom domains. Conclusions: Drawing definitive conclusions from the published studies about the nature of ASD is difficult, due to the fundamental methodological differences among the studies and their inconsistent findings. This review shed light on a number of discrepancies regarding the current classification of ASD. However, future research will be necessary to provide a more definite answer on the question of a definition based on separate diagnostic subtypes or on a severity gradient by including larger samples that are followed longitudinal and by applying better diagnostic system and choosing the appropriate variables.
