Clinical Diagnosis from a Psychodramatic Perspective: From role theory to an operative diagnosis
This article focuses on the relationship between psychodrama and diagnostic action. It begins with general epistemological considerations regarding the relationship between the assessor and the observed subject. Aspects of existentialism, scientific knowledge, subjectivity and the search for universal truth are reflected upon. These are developed in order to define typologies, purposes, characteristics and the limits of possible diagnostic activities compatible with the Morenian philosophy and method. In this context the specific category of a working diagnosis is defined, from which principles and reference criteria are explained and described.

The concept of the internal objects and internal and external spaces has been further developed in this paper. These objects are an important part of patients seeking treatment for themselves for they are also looking unconsciously for 'treatment' of these objects. The objects are ' damaged' and the patient does not know how to repair them. He keeps them alive in the hope that help will come. The methods used to keep them alive, even when dying, have been described. The stages at which these objects have been, or are thought to have been, damaged is vital. For example, they can range from the sensori-motor level to higher levels of maturation, to produce psychosis, borderline or hypochondria. The concrete level is stressed for it leads to concrete repair as opposed to psychic reparation.

Introduction: Teaching Psychology in medical curriculum has been the subject of numerous dissertations that focus on the relevance of this knowledge for doctors, at a general level.Methods: A non-systematic review of the relevant literature, particularly from the last decade, as well as national and international recommendations addressing the need for integration of behavioural and social sciences in medical training, was performed.Results: The literature supports the existence of preconceptions and negative attitudes towards the role of psychology in medical education, demonstrated by research in various european and american universities. La preparación profesoral en la impartición de la asignatura Morfofisiología Humana IV

Identificar las necesidades de aprendizaje presentadas por los profesores para impartir la asignatura Morfofisiología Humana IV, y los criterios que tienen sobre la reunión de Preparación Metodológica en su preparación y desempeño docente. MÉTODOS: Se encuestaron 40 profesores del Nuevo Programa de Formación de Médicos (NPFM). RESULTADOS: Mostraron que el nivel de dificultad presentado en el desarrollo de la asignatura estuvo entre las categorías bajo y medio y que los criterios sobre la reunión de preparación metodológica son positivos. CONCLUSIONES: Se identificaron necesidades de aprendizaje en relación con la autopreparación docente, la impartición de la asignatura y su vinculación con los problemas de salud de la comunidad; de otra parte se reconoce como positiva la realización de la reunión de preparación metodológica como forma de trabajo metodológico.

Medical advances force practitioners to work in an increasingly standardized manner with their patients. Quantitative health psychology attempts to follow a similar path by adopting, for the same patients, methods that are equally systematized. In this article, the origin of such an attempt will first be positioned historically. The clinical method will then be used to establish that, while patients tend to accept the constraints imposed by the medical technique, they usually resist those resulting from quantitative psychology. Based on clinical observations, we will present several ways in which such resistance may manifest itself. This article aims to further the understanding of qualitative health in psychology.
Theory as metaphor: clinical knowledge and its communication

This paper investigates the relationship between clinical knowledge and psychological theory and considers the implications for clinical writing. I argue that clinical knowledge is a way of understanding rather than a body of facts and compare clinical material to 'texts' that generate multiple and indeterminate meanings. Analytic theories, which represent the crystallization of ways of understanding clinical phenomena, have an inherently metaphorical 'as if' quality since they are derived from and adapted to the clinical process of making meaning by representing psychic states in symbolic form. Thus good clinical writing demonstrates an integration of theory and clinical material into a unified network of symbolic meanings. Redfearn's paper, 'The captive, the treasure, the hero and the "anal" stage of development' (1979), is discussed as an exemplar of such integration. It is suggested that clinical knowledge is equivalent to the skill of making effective interpretations.
OLVIDO DE LOS FENÓMENOS PSICOLÓGICOS EN LA RELACIÓN MÉDICO-PACIENTE ANCIANO

Para evaluar las publicaciones sobre la relación médico-paciente anciano, se hizo una revisión sistemática de ese tema en el MEDLINE (1966MEDLINE ( -1995. Se buscaron los artículos de investigación que tenían que ver con los componentes de la relación médico-paciente anciano. De los 57 artículos que se incluyeron, 33% se enfocaban en las barreras actitudinales, 25% en los resultados, 23% en las capacidades de comunicación, 16% en asuntos éticos, y sólo 3% en los fenómenos psicológicos. Aspectos como la transferencia, la contratransferencia y la empatía son esenciales para entender la relación médico-paciente anciano. Se concluyó que los fenómenos psicológicos entre los médicos y sus pacientes ancianos a menudo no se incluyen en la investigación.
