Prevalence and clinical profile of glaucoma patients in rural Nigeria—A hospital based study

Purpose To determine the prevalence and clinical presentation of participants with glaucoma attending a public eye care facility in Nigeria. Method Hospital based retrospective study of glaucoma participants aged 50 years and above seen over a 5-year period. Descriptive statistics summarized the demographic, clinical characteristics and treatment of the participants and determined the association of variables with gender and age. Prevalence of the glaucoma by type, and their 95% confidence intervals (CI) were also calculated. Result Of the 5482 case files that were reviewed, 995 (18.15%, 95% Cl 17.15–19.19%) had glaucoma particularly primary open angle glaucoma (11.55%, 95%CI 10.73–12.42%) and were mostly females (564, 56.7%) aged 69 ± 12 years (range, 50–103 years). Nigeria Normative Data for Defining Glaucoma in Prevalence Surveys

Purpose: To determine normative values for defining glaucoma in cross-sectional surveys in Nigerian adults. Methods: Multistage stratified cluster random sampling with probability-proportional-to-size procedures to select a nationally representative sample of 15,027 persons aged 40 years in 305 clusters across Nigeria. Systematic sampling of 1 in 7 participants gave 1759 who were examined in detail to construct a normative database. The normative subset was used to determine values for vertical cup/disc ratio (VCDR) and intraocular pressure (IOP) for glaucoma diagnosis according to the International Society of Geographical and Epidemiological Ophthalmology (ISGEO) criteria. Examinations included visual field testing by frequency doubling technology (FDT), Goldmann applanation tonometry, and optic disc image grading by Moorfields Eye Hospital Reading Centre. Results: In the normative dataset, 1057/1759 persons (60.1%) had normal FDTs, and constituted the hypernormal. Of these, 851 had VCDR and 973 had IOP measurements taken in both eyes. For category 1 (structural and functional evidence of glaucoma), the 97.5th percentile VCDR was 0.7. For category 2 (advanced structural damage with unproven visual field loss), the 99.5th percentile VCDR was 0.75. In addition, asymmetry in VCDR was 0.1 difference at the 97.5th percentile and 0.2 difference at the 99.5th percentile. Category 3 criteria were used when the optic disc was not visible and field testing not possible; 99.5th percentile IOP is one criterion (28 mmHg). Conclusion: While these results do not differentiate between open-angle and angle-closure mechanisms, they can be applied to determine the prevalence of glaucoma in Nigeria and sub-Saharan African countries with similar sociodemographic characteristics.

To obtain epidemiological data on the prevalence and risk factors for open angle glaucoma in hospital workers of African origin, and investigate appropriate methods of a rapid, cost-effective screening procedure for glaucoma. A cross-sectional study of workers in the University College Hospital (UCH) Ibadan, using a structured questionnaire for data collection. The variables available for data analysis include workers demographic characteristics, visual acuity, pupil status, intraocular pressure, cup-disc ratio, central visual fields, family history of glaucoma, chronic diseases such as hypertension and diabetes. Purpose To investigate the current practice on glaucoma care with the aim of highlighting its poise to tackle this emerging sight-threatening disease in Nigeria. Methods This was a cross-sectional, descriptive, population-based survey involving 88 ophthalmologists. Information on their demographic characteristics, practice profile, challenges and prospects on glaucoma care was collected using a semi-structured, self-administered questionnaire in August 2016. Data were analysed using SPSS 20. Results The participants are comprised of 46 (52.3%) males and 42 (47.7%) females, with a mean age of 42.2 ± 8.7 SD years. They were 45 (51.1%) consultants, 31 (35.2%) residents and 12 (13.6%) diplomates. Their years of practice were 8.8 ± 6.7 SD years. They worked mainly in government hospitals located at the southern part of Nigeria. The current practice was mainly comprehensive ophthalmology, 63 (71.6%). Only 2 (2.3%) had strict subspecialty practice. Others, 23 (26.1%), had combined practice. Eleven (12.5%) were glaucoma specialists and had combined practice. The majority of the participants, 57 (64.8%), were routinely diagnosed glaucoma properly. Sixty-three (71.6%) participants underwent trabeculectomy, 48 (54.5%) combined cataract surgery with trabeculectomy, 7 (8.0%) drainage implants, 5 (5.7%) laser trabeculoplasty, and 2 (2.3%) minimally invasive glaucoma surgery. Poor patients' acceptance and satisfaction, fear of complications, lengthy post-operative care and cost were the main deterrents to surgeries. Advocacy, public awareness, training of glaucoma specialists, provision of equipment and health insurance were the major recommendations on improving glaucoma care. Conclusion Given the meagre number of specialists and lack of strict subspecialty practice, optimal glaucoma care in Nigeria is still far from reality.
Prevalence and Types of Glaucoma Among an Indigenous African Population in Southwestern Nigeria

PURPOSE. To determine the prevalence and identify the types of glaucoma in the Akinyele district of Oyo State in southwestern Nigeria. METHODS. Residents of Akinyele district of Oyo State in southwestern Nigeria aged 40 years and older were randomly selected in a stratified manner. All participants underwent comprehensive ophthalmic examination, including visual acuity assessment, anterior segment biomicroscopy, IOP measurement, gonioscopy, optic nerve head and disc evaluation, and central visual field assessment. Glaucoma was diagnosed using the International Society of Geographical and Epidemiological Ophthalmology (ISGEO) classification scheme. RESULTS. A sample of 811 subjects (90% response rate) was examined. The crude prevalence of all forms of glaucoma was 7.3% (95% confidence interval [CI] 5.5%-9.1%) with an age and sex standardized rate of 6.9% (95% CI 6.88%-6.92%). Primary open angle glaucoma was found in 6.2% (95% CI 4.5%-7.8%) and primary angle closure glaucoma in 0.2% (95% CI 0.0%-0.6%). Secondary glaucoma accounted for 0.9% of the cases, with couching and neovascular process being the main causes (0.2% each). Prevalence of glaucoma increased significantly with increasing age (P for trend < 0.05). CONCLUSIONS. The high prevalence of glaucoma (7.3%) in the Akinyele district in southwestern Nigeria is comparable with those in predominantly black populations in the Akwapim-South district of Ghana and Barbados. Primary open angle glaucoma remains the most prevalent form of glaucoma.

Introduction: Glaucoma remains the second leading cause of blindness worldwide. Early detection and treatment play a key role in glaucoma management. The aim of this study was to determine the prevalence of glaucoma among patients attending the eye clinic at Buluk Eye Centre from 1st January to 31st December 2017. Method: This study was a cross-sectional retrospective study using data extracted from the registration cards of patients at Buluk Eye Centre from 1st of January 2017 to 31st of December 2017. A datasheet was used to collect the relevant variables including demographic variables. Data were entered, organized and analysed using SPSS version 21 [IBM SPSS Statistics] and p<0.05 was considered statistically significant. Introduction: Glaucoma blindness is burdensome. Knowing glaucoma management challenges can reduce glaucoma blindness burden. The purpose of this study was to evaluate the epidemiological and clinical profile of patients living with glaucoma and receiving care in a tertiary eye center in Ghana. This was a hospital-based retrospective study of clinical records of glaucoma patients from January 2010 to December 2019. The study involved collating demographic information of patients, clinical presentation, and the management of glaucoma. A total of nineteen thousand (19,000) charts were retrieved from the eye center’s archives. Out of these, 660 (3.5%) records of patients qualified for the study and were analyzed. There were 398 (60.3%) males and 262 (39.7%) females. Their ages ranged from 9 to 86 years (mean age = 47.30; SD ± 16.86 years).
