Spontaneous infarction in fibroadenoma of breast: a rare pathological finding in a common pathological entity

Fibroadenoma is one of the most prevalent benign tumour in adolescent women. It accounts for 20% of all benign breast tumours. Spontaneous infarction in fibroadenoma is very unwonted and mostly, it is associated with gravidity or lactation. Infarction can additionally occur as a complication of antecedent fine-needle aspiration biopsy. At the institution during 2-years period we incidentally found three such cases of spontaneous infarction of fibroadenoma over varied age groups which surprised us. So, reviewed the literature and presented our findings. Three fibroadenomas with features of central necrosis were diagnosed on excision and biopsy. Spontaneous infarction within fibroadenoma is a rare occurrence. It causes diagnostic difficulties, hence conscientious search and possibility of this entity should be must during the evaluation of breast lump.

Spontaneous infarction is an uncommon complication of fibroadenoma of the breast. Although infarction following fine needle aspiration (FNA) has been reported in the literature, infarction encountering on first time aspiration is very rare. Lack of knowledge of this entity may lead to an erroneous diagnosis of inflammatory lesion like mastitis and carcinoma on cytology smears.We report the cytological and histological findings in partial spontaneous infarction of fibroadenoma in an 18-year-old female and literature reviewed. In young patients with breast lumps, if the aspirated material is necrotic, the probable diagnosis of spontaneous infarction should always be kept in mind. Viable fibroadenomatous tissue should be searched for in the FNA smears.

Infarcted fibroadenoma of the breast: report of two new cases with review of the literature

Introduction: Fibroadenomas are the most common benign breast tumors in young women. Infarction is rarely observed in fibroadenomas and when present, it is usually associated with pregnancy or lactation. Infarction can exceptionally occur as a complication of previous fine-needle aspiration biopsy or during lactation and pregnancy. Materials and methods: Retrospective review of 650 cases of fibroadenomas diagnosed at our institution during the 8-years period identified two cases of fibroadenomas with infarction (rate~0.3%). Results: Two partially infarcted fibroadenomas were diagnosed on core biopsy and frozen section in an adolescent girl (13 years old) and in a young woman (25 years old), respectively. No preceding fine-needle aspiration biopsy was performed in these cases, nor were the patients pregnant or lactating at the time of the diagnosis. Conclusion: Spontaneous infarction within fibroadenoma is a rare phenomenon in younger patients. The presence of necrosis on core biopsy or frozen section should be cautiously interpreted and is not a sign of malignancy. Infarcted Fibroadenoma: An unusual case with its Cyto-histopathological Correlation

Fibroadenoma is a benign breast lesion of adolescents and young women.It constitute 20% of all benign breast tumours. Fibroadenoma is associated with many secondary changes like calcification, ossification, hyalinization, myxoid and apocrine squamous metaplasia. Spontaneous infarction in fibroadenoma is very rarely seen. We report one such case of spontaneous infarction of fibroadenoma in a 16 year old unmarried female with its cyto-histopathological correlation, that posed us with diagnostic dilemma in early stage. The possibility of this entity should be considered while diagnosing the breast lumps on cytology .The lesion was further confirmed on histology.

A SYSTEMATIC STUDY ON FIBROADENOMA OF THE BREAST

Background: Fibroadenomas are one of the main benign diseases of breast. Though considered as a risk factor for development of breast cancer its reporting has been over shadowed by that of breast cancer. Early diagnosis and treatment can relieve anxiety associated with non malignant conditions of breast. Analysis of pattern and prevalence should provide a valuable guideline for clinicians in India for comparison with other countries. Methods: A prospective study was undertaken during 6 year period and all cases with breast lesion underwent FNAC or excision biopsy. Confirmed cases of fibroadenoma were studied for clinicopathological features. A total of 210 cases were analyzed. Results: Of the 210 cases of fibroadenoma most were married urban women of age group16-30(66.1%) with average 12 months of symptoms (80%). Majority of fibroadenomas (51.9%) were 3-5 centimeters located in upper outer quadrant (43.8%).Most of the cases (78.1%) underwent excision and none presented with recurrence or carcinoma within the lesion. Conclusion: Fibroadenoma are the most common benign lesions of the breast with average age of presentation 27yrs. Excision is the best treatment for women over 35yrs to exclude malignancy.

Incidentally detected carcinoma in situ in a fibroadenoma of breast in a postmenopausal woman: A case report

Fibroadenomas are the most common benign tumor of the br but the peak incidence seen in young adolescent women during the second and third decades of life. Fibroadenoma is a biphasic tumor, composed of an epithelial and a stromal component. Even though they are solitary le sions, multiple fibroadenomas can also occur especially in the older age group. Breast fibroadenomas seemed to display a wide spectrum of proliferative and non proliferative histologic changes. Of all these changes, carcinoma arising from fibroadenoma is a occurrence. Here, we have present standing multiple fibroadenoma in a postmenopausal women. Clinical and radiological features are not helpful in identifying carcinoma arising from fibroad the excised specimen should be done for the confirmation of the diagnosis.

Incidental Detection of Carcinoma In-situ in Fibroadenoma of the Breast in a Young Woman: A Rare Finding

Fibroadenoma, being a very common benign tumor of the breast in young females, does not pose any threat and, thus, can be treated with lumpectomy. Breast cancer arising within a fibroadenoma is a rare phenomenon, but detecting these neoplasms is of utmost importance for complete treatment and follow-up. These lesions are an incidental finding in a lumpectomy specimen done for fibroadenoma breast. A 28-year-old female presented with multiple bilateral lumps for eight years. The lumps were mobile, non-tender, and slowly growing, with no nipple discharge, no axillary lymphadenopathy, and no family history. The diagnosis of a benign lesion suggestive of fibroadenoma was given on ultrasonography (USG) and fine-needle aspiration cytology (FNAC). The patient underwent lumpectomy and the excised tissues were subjected to histopathological examination. Grossly, multiple, well-circumscribed, encapsulated masses, with the largest measuring 4x2.5x2 cm were noted. All masses showed homogenous grey-white areas with slit-like spaces. On microscopy, predominant areas were consistent with fibroadenoma, with few foci showing the features of ductal carcinoma in situ (DCIS) with nests of cells having pleomorphic nuclei, prominent nucleoli with atypical mitosis, a cribriform pattern, and comedo necrosis. This highlights the rare association of fibroadenoma and carcinoma in situ, thus, a careful and extensive sampling of the tissue is required to prevent the false negative diagnosis by pathologists.

Frequency of Fibroadenoma in Patients Presenting with Breast Lump

Objective: To determine the frequency of fibroadenoma in patients presenting with breast lumps. Methodology: This descriptive cross sectional study was conducted in the De- partment of Surgery, Hayatabad Medical Complex, Peshawar (from 1st March 2016 to 30th May 2017) recruiting a total of 165 patients who presented with breast lump. These patients were subjected to ultrasonography, fine needle aspiration cytology and tru-cut biopsy to detect fibroadenoma. Data was ana- lyzed with SPSS 17 and presented as tables. Results: The mean age was 23.2 ±4.6 years. Maximum patients were between 15 to 25 years of age (66.7%). Left side was involved in 58.8% while right side breast lumps were present in 41.2% of patients. Breast lumps in the superi- or-medial quadrant were in 26.7% of patients, superolateral quadrant 32.7%, inferomedial quadrant 13.3% and inferolateral quadrant 27.3% patients. Incidental Detection of Carcinoma in Situ in Fibroadenoma of Breast in a Young Woman: A Rare Finding

Fibroadenoma is the most common benign tumor of the breast in young females. Breast cancer arising within a fibroadenoma is a rare phenomenon. The incidence of carcinoma within a fibroadenoma is reported to be between 0.1% and 0.3% in a screened population, with a peak age of occurrence between the 4 th and 5 th decade. We present a case of 29-year-old female with ductal carcinoma in situ in a background of fibroadenoma. There is a low percentage of fibroadenomas harboring carcinoma; however, all breast lumps should be seriously managed; extirpation and histological examination is recommended.
