Caso Clínico Clinical Case Resumo Apesar da ampla cobertura vacinal, a infecção por Bordetella pertussis está longe de estar controlada. Os recém-nascidos e lactentes ainda sem imunização completa e filhos de mães com baixos títulos de anticorpos para a Bordetella pertussis são altamente susceptíveis à infecção e têm maior risco de doença grave e morte. A infecção por Bordetella pertussis associada a hipertensão pulmonar no recém-nascido é frequentemente fatal.
Infant pertussis deaths and the management of cardiovascular compromise

Infants infected with Bordetella pertussis in the first few weeks of life are at risk of death from 'overwhelming cardiovascular compromise despite intensive care support'. The mechanisms of this severe disease are not completely understood. Three case histories, including that of one infant who survived, are presented. Two of the patients died despite intensive therapy with pressors and, in one child, milrinone. The third child survived following treatment with nitric oxide and sildenafil. Hence, sildenafil in combination with nitric oxide shows promise as a therapy for the haemodynamic consequences of pertussis toxaemia and this should prompt clinical trials for their efficacy for this condition.

Pertussis, or whooping cough, a highly contagious disease caused by Bordetella pertussis, is making a comeback globally and nationally in spite of reasonable vaccination coverage. Worldwide, there have been increasing reports of Bordetella pertussis infection among adolescents and adults, but the peak incidence and highest mortality occur among infants. We report a 19-day-old female infant presenting with progressive respiratory failure. The mother was the only familial contact who complained of mild cough. However, occasional apneic episodes with cyanosis and peripheral lymphocytosis prompted us to examine the presence of Bordetella pertussis, which remains a significant cause of morbidity and mortality in unimmunized infants. Understanding the source of pertussis transmission to infants may provide new approaches to prevent pertussis in the most vulnerable infants. The known about this topic Pertussis is a vaccine-preventable disease. The causative agent is Bordetella pertussis. It frequently leads to pneumonia in infants. It may cause severe disease in unvaccinated infants. Contribution of the study Among patients admitted to intensive care unit, severe pertussis occurs more frequently in infants who are too young to be protected with active immunization. Maternal immunization and the cocoon strategy may provide success to protect this age group. Severe pulmonary hypertension on echocardiographic examination and presence of leukocytosis in laboratory tests are associated with poor prognosis.

Background. Bordetella pertussis infection can cause severe illness and death among young infants. Methods. We collected demographic and clinical information from the medical records of infants who were 90 days of age and hospitalized for pertussis in 5 Southern California pediatric intensive care units (PICUs) from September 1, 2009 to June 30, 2011. Infants who died or were diagnosed with pulmonary hypertension were considered to have more severe pertussis. Results. Thirty-one infants were admitted to a participating PICU. Eight infants had more severe infections, 6 infants had pulmonary hypertension, and 4 infants died. The 8 infants with more severe infections had white blood cell counts that exceeded 30 000, heart rates that exceeded 170, and respiratory rates that exceeded 70 more rapidly after cough onset than the 23 infants with less severe illness. Conclusions. Identifying higher-risk infants earlier might allow for more rapid implementation of interventions.

Encefalopatia - Complicação rara de infecção por Bordetella pertussis

Recem-nascido de 25 dias internado por tosse acessual, gemido, fervores crepitantes bilaterais com apneia e insuficiencia respiratoria aguda. A radiografia do torax apresentava hipotransparencia no hemitorax esquerdo. Analiticamente leucocitos 72170/mm3, linfocitos 33660/mm3 e PCR 12 mg/L. Iniciou cefuroxime e gentamicina e ventilacao mecânica. A PCR para Bordetella pertussis foi positiva, associou-se azitromicina mas nao realizou exsanguineotransfusao. Em D3 iniciou movimentos subtis clonicos dos membros. O EEG apresentava actividade epileptiforme nas regioes temporais posteriores, o LCR 40/uL celulas, glicorraquia normal, proteinorraquia 254mg/dL e a RMN hipersinal da substância branca frontoparietal e talamos. A evolucao foi favoravel sob fenobarbital e fenitoina. Na familia, simpatizante de movimentos antivacinas, nenhum elemento estava vacinado. A encefalopatia da pertussis e uma complicacao rara mas temida (0,5-1%). A exsanguineotransfusao pode melhorar o prognostico. A vacina...
