Impaction of maxillary central incisors is frequent in dental practice. However its management is challenging because of its importance in aesthetics. Supernumerary teeth are the most common dental anomaly that can cause impaction of adjacent teeth, crowding, diastema, rotation and displacement of teeth. We report a case of 13 year old male with an impacted supernumerary tooth in the maxillary anterior region, which was interfering with the eruption of the permanent, left central incisor. Impaction of a maxillary central incisor is a rare dental condition which is easy noticed by the patient and the general practitioners because it has a strong impact against dental and facial esthetics. In many cases this is due to the presence of one or more supernumerary teeth, easy recognized on radiographs. The absolute indication in these situations is the early removal of the supernumerary teeth in order to minimalize the damage in the anterior area. Almost in all cases the surgical phase should be followed by the orthodontic phase in order to bring the impacted tooth in the dental alignment, to establish the proper occlusion and function. In our paper we present the case of a 9 years boy with left central maxillary incisor impaction and 2 supernumerary teeth in the anterior area. The case is presented step by step until the final result at the end of the treatment with the upper incisor in place.

The inclusion of both maxillary permanent central incisors is uncommon. This condition compromises face aesthetics, phonation and masticatory function. Therefore, early diagnosis is essential to avoid complications and failures. There are various reasons for inclusion, but supernumerary teeth are the leading cause. Early causes of removal and rapid expansion of the palate determine a high probability of success with the spontaneous eruption of the impacted elements. Hyperdontia is a disorder of odontogenesis characterised by excess teeth. Many complications can be associated with supernumerary teeth, such as crowding, tooth displacement, diastema, deep caries, retention or impaction, delayed eruption or ectopic eruption of adjacent teeth, aesthetic problems, neuralgic manifestations, root resorption of adjacent teeth, and dentigerous cyst formation with significant bone destruction. Impaction of maxillary permanent incisor is not a frequently case in dental practice, but its treatment is challenging because of its importance to facial esthetics. Supernumerary teeth are the main cause of impaction of upper incisor. Supernumerary teeth when present can cause both esthetic and pathologic problems. Supernumerary teeth in the maxillary midline are common. Early detection of such teeth is most important if complications are to be avoided.

Multiple impacted teeth is a condition which often requires a multidisciplinary management. This case report describes orthodontic treatment that required surgical exposure of multiple impacted permanent teeth as well as extraction of supernumerary teeth followed by ligation and traction of impacted teeth in to the oral cavity. The treatment was completed with a significantly improved functional and aesthetic result.

The aim of the retrospective study was to evaluate the influence of the initial vertical position of the impacted maxillary incisor on spontaneous eruption frequency and timing after surgical removal of the supernumerary tooth. Records of 33 patients with unilateral impaction of upper central incisor caused by supernumerary teeth with the average age 9.58+/-1.54 years were analyzed. Impacted teeth were distributed into 3 groups on the basis on their initial vertical position on the orthopantomograms. The results indicate that impacted maxillary central incisors spontaneously erupt in 63.6% of cases after removal of supernumerary teeth. The average time of the spontaneous eruption of impacted maxillary central incisors was 16.05+/-9.3 months (from 3 to 30 months).
