An attractive smile enhances the appearance and acceptance of an individual in society. Gum exposure more than 3 mm is generally considered unattractive and known as a gummy smile, which is usually considered an aesthetic problem. At present, patients have a greater desire for more aesthetic results that may influence the planning of dental treatments. This case report aimed to describe the surgical sequence of aesthetic crown lengthening to improve smile profile and eliminate gummy smile. We reported a 21-year-old non-smoking woman with no pertinent medical history who presented with a chief complaint of an unattractive smile due to excessive gingival display. The gingival display in the smile was 5 mm, and the width to height ratio of the central incisor was 121%. Neither periodontal problems nor teeth mobility was detected. Impact of orthognathic surgery on the treatment of gummy smile: an integrative review

The gingival smile is considered unpleasant and can be treated in different ways according to its etiological factor. When caused by maxillary vertical growth excess, orthognathic surgery may be indicated to correct the vertical excess. The aim of this integrative review was to evaluate the impact of orthognathic surgery on the treatment of gingival smile. An electronic search was performed of the PubMed/Medline, Cochrane Library, Scopus, and Google Scholar databases. The selection of studies was performed by two blinded reviewers. Firstly, studies were selected by reading the titles and the abstracts of articles. The references from each study selected were then searched to find articles that were not found in the electronic search. After reading the full-text articles, studies that met the inclusion criteria were selected. A total of 667 studies were identified, but only 19 were selected for the integrative review after applying the inclusion and exclusion criteria. The selected articles reported maxillary impaction from 2 to 10 mm, but this amount of superior repositioning of maxilla does not appear to be related to the initial gingival exposure described. Orthognathic surgery is not the first choice of treatment for gingival smile, but it appears to be a suitable therapy for other conditions in the same patient for improving a gummy smile by correction of gingival exposure.

They are the essential elements of nonverbal communication and play a significant part in establishing a person's first impression. The altered passive eruption, which results in the excessive gingival display (EGD) when the gingival edge is situated incisal to the cervical convexity of the crown, is one of the factors affecting aesthetics. It has an impact on the patient's appearance and grin. The management of EGD becomes crucial. The following case study covers the control of EGD with a crown lengthening operation.
Treatment of bilateral maxillary buccal exostoses on gummy smile

Introduction: Buccal exostoses are benign, broad-based surface masses of the outer or facial aspect of the maxilla and less commonly at the mandible, usually found in the premolar and molar region. A definitive female sex prediction is the characteristic of this condition. They are painless, self-limiting and may increase patient concern about poor esthetics. Gummy smile is an excessive gingival display during smiling which can occur in response to certain medications such as phenytoin or smaller than normal teeth due to developmental events, tooth eruption abnormalities, orthodontic treatments and even congenital defects. The objective of this treatment is to eliminate buccal exostoses and gingival overexposure with osseous resective surgery and crown lengthening to improve the esthetics of the gingiva, especially on anterior teeth. Case: A 30-year-old female, buccal exostoses with gummy smile on the buccal aspect from molar to canines region of the bilateral maxillary arch. This case was managed by a combination of osseous resection surgery to treat exostosis and crown lengthening to remove alveolar bone and excessive gingival. Treatment selected for this case is osseous resective surgery and crown lengthening that aimed for the removal of bone and to achieve a physiological bone architecture bilateral exostoses, functional of gingiva and aesthetic. Osseous resective surgery and crown lengthening were the treatment options to makes patient smile more proportional and achieving optimal aesthetics.
Decision-making process for esthetic treatment of gummy smile: A surgical perspective

Gummy smile is a quite frequently found esthetic alteration characterized by excessive display of gingiva during smiling. Several causes have been implicated in the literature, but a dearth of clinical decision-making process has been found in the surgical treatment of excessive gingival display. An external bevel gingivectomy with osseous correction was performed in anterior maxillary region in accordance with the proposed decision-making process. The clinical observation at 1 month postoperatively revealed restoration of natural smile with 1-2 mm of facial gingival display. The outcome seems to suggest that this proposed decision-making process can provide valid treatment options for gummy smile cases.
Orthosurgical Correction of Severe Vertical Maxillary Excess: Gummy Smile

Gummy smile which is commonly described as excessive gingival display in full smile has become a major esthetic concern for patients and healthcare providers in recent years. Because of the complex etiologic factors and severity of this orofacial deformity including skeletal, dentoalveolar, muscular, and gingival, it is essential that a differential diagnosis be established so that a relevant therapy can be selected from a wide range of treatment options. The complex etiologic factors of this dentofacial deformity necessitates coordination between a team of health care providers, especially between orthodontist and maxillofacial surgeon, to improve diagnosis and treatment planning, accurately predict the outcome, and minimize orthosurgical complications. In this chapter, the author will present several clinical cases and briefly review available treatment modalities for excessive gingival display in full smile. Emphasis will be put on orthosurgical treatment of severe vertical maxillary excess as the optimal solution to restore facial balance and harmony, attractive smile, and patients' satisfaction.
