Verbalization of an Informing Communication Strategy in Consulting Cancer Patients

The article is devoted to the problem of communicative interaction in the “doctor — patient” dyad. The relevance of the study is due to the need to return the humanistic principle to medical practice and to increase the efficiency of communication between medical personnel and patients, both in their native language and in a foreign language. The aim of the study is to identify the means of verbalization of the informing communicative strategy, which the doctor uses during a consultation of cancer patients. The empirical material was authentic consultative conversations in German, recorded in the GAT-2 transcription system. It has been proven that in the material under study, the informative communicative strategy is dominant. Purpose: to estimate the efficiency of the concurrent chemoradiotherapy (CRT) and radiotherapy (RT) alone for patients with the locally advanced cervical cancer (CC) taking into account prognostic factors. Material and methods: The research included 335 patients with morphologically verified CC of the П-Ш stage on FIGO: 150 patients underwent RT (group 1): 185 patients - CRT with cisplatin and 5-fluorouracil (group 2). Patients in the group 1 were significantly older than patients of the group 2. The disease of the Stage III, parametric variant of the spread of the tumor, as well as metastatic lesions of the pelvic lymph nodes occurred more frequently in the group 2. Results: In the group 1 complete response to treatment occurred in 98 cases (52.9%), while in the group 2 demonstrated - 64 (42.7%) patients (p < 0.01). Health care is based on patient data. The doctor, nurse or medical assistant are able by education, training, knowledge and experience to interpret patient data needed for diagnosis and treatment. Health care is dependent on a good education with respect to the management of data, information and knowledge. International activities point to ways to achieve a good level of education. These include criteria for curriculum development and accreditation, quality of information systems, further experience with a comprehensive training concept in England and the 10X10 approach in the United States.
Prospects and risks of telemedicine: survey results
Перспективы и риски телемедицины: результаты социологического опроса 261 Аннотация. Телемедицина-перспективное направление развития здравоохранения, но при её внедрении необходимо учитывать возможные риски. В 2019 г. был проведен опрос врачей и пациентов амбулаторных организаций (квотная выборка организаций, 601 человек). Телемедицина слабо распространена, пациенты скорее с ней не знакомы. Имеются «консервативные» группы, чье отношение к телемедицине негативное. Врачи больше принимают взаимодействие с помощью телемедицины с коллегами, чем с пациентами. Признавая плюсы улучшения доступности, специалисты опасаются повышения нагрузки и снижения качества осмотра, пациенты-снижения «живого общения». Показана необходимость просветительской работы. Даны рекомендации, обозначены возможные перспективы и формы внедрения телемедицины.

Basic principles of communication doctors with the family of cancer patient

Annotation. Cancer accompanied by significant impairment of the patient's physical and mental health, which necessitates the involvement of additional resources to help the patient. The most powerful is the psychosocial support provided by the patient’s family. The communication of the doctor with the patient's family should be carried out in accordance with the legal acts, should take into account the practical needs for family involvement in the treatment process and address the specific problems of the patient during therapy, and should be carried out according with the basic principles of effective communication.
