The objective of this study was to characterize the activities of health education in the university extension projects in undergraduate nursing courses in the state of São Paulo, Brazil. We collected data on health education activities conducted by university extension projects within four Higher Education Institutions (HEIs) through documents and interviews with project coordinators. The projects found showed the coexistence of various forms of extension in the HEI surveyed, with practices that promote health education (health care, courses, educational groups in communities, workshops, among others). The health education activities carried out in the identified extension projects remitted to second place the objective of promoting reflection about the sociocultural context of the groups involved, giving priority to the internal demands of the academic community and delaying the use of strategies that enable identifying the health needs of social groups.
