EVALUATION OF PATIENT COUNSELING IN DIFFERENT HOSPITAL OF KARACHI, PAKISTAN; A NEGLECTED DOMAIN OF PHARMACY

Patient counseling is a critical component of dispensing process that provides information, advice and assistance to help patients using their medication properly. The objective of our study was to analyze whether patient counseling services are provided or not and how efficiently it is being conducted. For this purpose a questionnaire based cross sectional survey of pharmacists was done in different hospital set ups of the Karachi city with a sample size of 30. The results show that counseling services are provided by all the Hospitals in which survey was conducted, medication charts are used by 15 (50 %) samples while patient education program is not organized by 18 (60 %) samples. The results obtained from the survey conclude that patient counseling is conceptually present in all hospital setups but not done efficiently or in appropriate manner. Steps should be taken by the authorities, pharmacists and the patients to make counseling more effective.

Pharmacists can improve adherence to drug therapy in order to help patients participate in their own health care decision. The study aimed to observe the problems faced during dispensing and counseling. A cross-sectional study was carried out in two different hospitals of Nepal. ASHP (American Society of Health-System Pharmacy) 1997 and USP (The United States Pharmacopoeia) 1997 guidelines were used to ensure the completeness of the contents of the counseling. The extent of counseling fulfilled by the pharmacists was observed and recorded. Collected data was compiled, managed and analysed using SPSS and MS Excel. Counseling was observed in total 400 cases (200 from each hospital). It was found that complete information from the guidelines was not provided in both the hospitals. Objective: To evaluate the impact of patient counselling in terms of knowledge, attitudes and practices (KAP) outcomes among retail and hospital pharmacist in Satara and Sanagli district. Methods: A cross-sectional survey was conducted on knowledge, attitudes and practices of patient counselling adopted by retail and hospital pharmacists from different areas of Satara and Sangali district. A specially designed questionnaire was used for data collection. Total 190 respondents were analyzed for the study. Results: The highlighting result of the survey based project revealed that almost all retail as well as hospital pharmacists were practicing patient counselling without any charges and there is necessity of separate patient counselling cabinet. Pharmacist was lacking in adopting modern techniques, aids for counselling and 20% only attended patient counselling course (PCC) by Maharashtra State Pharmacy Council (MSPC). Conclusion: KAP study revealed the positive attitude, knowledge and existence of patient counselling practices but not done in an appropriate manner. Step should be taken by authorities, pharmacist and the patient to make counselling more effective.

Non compliance is one of the important factors that lead to failure of the drug therapy. Various documents regarding problem of non compliance have been documented even in the developed countries . There are several reasons for non compliance among them, the ABSTRACT Background and Objectives: Patient needs specific guidelines from their pharmacist in support of better compliance of their medicine used. Counseling of the patient and patient representative is important for improving the therapeutic out comes. Clinical pharmacists are in regular communication with patients and take part in making clinical interventions along with the countless technical tasks done behind the counter. Dispensing just a kit of medicine to the patient or their representative is not the only duty but also dispensing the right drug to the right person with right information. Clinical pharmacist has a major role in providing effective counseling being it a critical element in ensuring optimal therapeutic outcome. Pharmacist's role is important in improving the patient understanding in the management of their medical condition. They help in reducing medication errors, minimizing incidents of adverse events and drug interactions and improving patient compliance. This article provides information on the attitude of patient towards counseling, barriers faced by pharmacists and the emerging trends in providing an effective patient counseling.

Background: Chronic kidney disease is a public health problem with an increasing incidence and prevalence, poor outcomes, and high cost. Patient involvement forms the keystone in the management of chronic kidney disease. This study evaluated effects of pharmacist-provided counseling in dialysis patients in terms of their knowledge, attitude, and practice outcomes. Methods: A total of 64 patients with chronic kidney disease were enrolled into the prospective, pre-post study based on the inclusion and exclusion criteria. The knowledge, attitude, and practice of patients regarding chronic kidney disease were assessed and recorded via baseline questionnaire. Case group patients were counseled regarding chronic kidney disease, their medication, diet, and lifestyle, and they were also provided with informative leaflet, whereas in the control group patients, the pharmacist did not intervene. After 1-month intervention, knowledge, attitude, and practice scores of patients of both groups were measured using the same knowledge, attitude, and practice questionnaire. Effectiveness of counseling on case group patients was evaluated by comparing the mean knowledge, attitude, and practice scores before and after counseling by paired t-test. Results: Mean knowledge, attitude, and practice scores before intervention were 8. 16 ± 4.378, 38.19 ± 3.217, and 6.69 ± 0.896, respectively, and these scores were changed to 13.75 ± 3.510, 38.78 ± 3.035, and 6.91 ± 0.777, respectively, after the intervention (p < 0.05).

Patient counseling by pharmacist - a focus on chronic illness

The management of chronic illness needs lifestyle modifications and drug therapy for a long period. Patient understanding regarding the illness plays a very important role in management of chronic illness. Effective patient counseling makes the patient understand his/her illness, necessary lifestyle modifications and pharmacotherapy in a better way and thus enhance patient compliance. The pharmacist has immense responsibility in counseling the patients with chronic illness. The counseling pharmacist should possess adequate knowledge and should be an effective communicator, making use of the verbal and non-verbal communication skills.

Role of Pharm.D in Better Patient Care: Perspective of Indian Health Care Practitioners

Background: Clinical Pharmacists (CP) are less prized in direct patient care compared to both physicians and nurses even though most of the professionals identified that the CP is an inexorable part of the medical team. Objectives: To explore the perception of Health care Practitioners (HCPs) towards the Pharm.D and Services provided by the CPs from the Indian perspective. Methodology: A 10 items questionnaire-based cross-sectional survey was conducted through surveying HCPs including medical, nursing, pharmacy and the academician about their perceptions regarding the Pharm.D program and the clinical pharmacy services providing by the CPs in a tertiary care teaching hospital situated in the south Indian state Karnataka. Results: A total of 130 volunteers participated including 91 medical doctors or students, 6 pharmacists who work in the hospital, 17 nurses, 3 academicians and other 13 didn't mention their profession. In the study, 91.54% (119) accepted that CPs can play a vital role in improving the medication adherence through the patient counselling and 92.31% (120) HCPs were agreed that CPs can provide appropriate interventions for better patient care. Their efforts in drug interactions and ADRs are really appreciated and 82.31% (107) recommended CP as an integral part of the multidisciplinary health care team. Conclusion: Our research suggests that CPs can be an asset to the health care practice especially in India through partnering the HCPs in better patient care by providing the appropriate interventions. Still, there is an emergent need for more practical emphasised syllabus and curriculum to make the Pharm.D graduates more employable.
