Sexual Functions in Male Patients with Traumatic Spinal Cord Injury

Objective: To explore sexual dysfunctions and expectations about sexuality in male patients with spinal cord injury (SCI) aged between 20-40. Material and Methods: A total of 75 traumatic SCI male patients were included in this study. The demographic and clinical characteristics were obtained by retrospective chart review. The differences between preinjury and post-injury sexuality status of the participants were assessed. The relationship between neurological level and erection/ejaculation was investigated. Results: The mean age of the participants was 26.16±4.35 years. The mean time since injury was 34.26±30.68 months. The number of the patients who had sexual interactions before injury was 63 (84%). Also, 34 patients (45%) had sex after injury, but 66 (88%) of the patients mentioned that their sexuality was affected after injury; 31 (41.3%) of the patients were not informed about sexuality in the post-injury period. Further, erection duration of 37 (60.6%) of the participants was shorter than 5 minutes, and 20 (32.8%) of the patients reported that they were satisfied with their erection. Moreover, 31 (41.3%) of the patients had reflex erection (RE), 14 (18.7%) of them had a psychogenic erection (PE), 16 (21.3%) of them got a mixed type of erection (RE+PE), and 14 (18.7%) of the patients did not have an erection. Ejaculation was present in 28 (37.3%) of the patients. The first three responses to questions about circumstances causing problems in post-injury sexual life and expectations for sexual life were erection, ejaculation, and orgasm, respectively. Conclusion: This study showed that sexual functions, like erection, ejaculation, and orgasm, in male patients were affected severely after SCI. The majority of the patients did not have enough sexual consulting, and the patients had great expectations for getting informed about sexuality in the post-injury period.

After the spinal shock period, suprasacral injuries classically result in detrusor hyperreflexia/overactive bladder and detrusor sphincter dyssynergia. Sacral cord injuries produce detrusor areflexia consistent with lower motor neuron injury and often increased bladder compliance. However, previous investigators have noted an inexact correlation between spinal cord injury level and urodynamic findings. The aim of this study was to evaluate the relationship between level of injury and urodynamic findings. Fifty-one patients with post-traumatic spinal cord injury were classified by the radiographically determined level of injury, clinical neurologic level and completeness of injury. Urodynamic studies were performed in all patients. Twenty-six of 36 patients with suprasacral injuries had hyperreflexia/overactive bladder (72.2%). Arab Spring, which started in 2010, affected directly or indirectly many countries in the Middle East region. Anti-regime demonstrations in the region started in Syria in March 2011. As of this date, the harsh stance of the Syrian regime against the demonstrators and the civil war that started in the wake of it led to the civilian population's migration. Due to the violence of civil war in Syria, millions of Syrians have migrated to Turkey. Having hosted Syrian refugees since April 2011, Turkey has been indirectly influenced by the Arab Spring since April 2011 because of the refugees who are lodged in the country. Turkey has not sent any Syrians back from the border without discrimination of religion, language and race within the framework of the "open door" policy and has given them a "temporary protection status". Besides this, refugees entering from the border were recorded and then they are placed to the camps. However, there are limited capacity in the camps and some of the refugees live in difficult conditions and also some of them are panhandling to survive. Therefore, there are perceptions in the society like that they are delinquent peoples. Syrian refugees have different languages, cultures and lifestyles and that situation prevents the integration of Turkish and Syrians, and also leads to mutual negative perceptions in the society. Therefore, there are senses and stigmatizing stereotypes that lead to the circulation of negative opinions in the language of the society. Discoursive exclusion come into existence in the shape of cultural exclusion through the identity of the Syrian. On the other hand, the political discourses of the ruling party are based on religion and civilization, and they createa positive perception about Syrian refugees.As the political discourses in Turkey are going to form positive perceptions, hateful discourses against to the Syrian refugees, discriminations and racism can be prevented.
Medical Treatment in Spinal Cord Injury

Spinal cord injuries cause psychological in humans and require expensive care and treatments. In recent years, various pharmacological agents have been tested in acute spinal cord injuries. Prospective randomized controlled clinical trials on a large scale have failed to demonstrate significant neurological progression, in contrast to their success in the laboratory. The search for an effective neuroprotective pharmacological agent to prevent secondary damage in acute spinal cord injuries remains primary goals for basic sciences and clinicians.
