The Importance of Oral Health during Pregnancy: A review

Pregnancy is a transient physiological state which brings about different hormonal changes in a woman’s body. These effects are generalized and there are various oral changes as well. There are a number of especially important alterations in the periodontal conditions within the oral cavity. These changes have important implications as they have been known to cause adverse pregnancy outcomes. Better knowledge about these scenarios among health care professionals and women would go a long way toward avoiding or minimizing these adverse outcomes. Health education is an important tool in creating awareness among pregnant women regarding improvement of their oral health. Awareness among the health professionals and good inter-departmental collaboration would help toward a more efficient treatment of these pregnancy related conditions.

MANAGEMENT OF ORAL HEALTH CHALLENGES IN PREGNANT WOMEN

Pregnancy is a dynamic state leading to several physiological transient changes in the body system including the oral cavity. In order to maintain good oral health, the dental treatment should not be avoided. The dental management of pregnant patients involves special consideration as otherwise a hypersensitive vomit reaction in the third trimester of pregnancy can influence the eating behavior of the patient as well as oral hygiene influencing the appearance of dental erosion and tooth decay. Once pregnant women suffer an unpleasant gag reflex in a dental office, they may become phobic, delaying or postponing their dental treatment. The dental management of pregnant patients involves special considerations.

Pregnancy is a dynamic physiological state evidenced by several transient changes and significant hormonal changes like increased production of estrogen and progesteron. Both periodontal disease and caries are highly prevalent among pregnant women. Also association of periodontal disease and preterm low-birth weight has been found. Thus, it is essential to educate pregnant women to promote oral health behaviours. Pregnant women should be emphasised on the relationship of maternal oral health with fetal health and specific preventive oral health care programme should be made an integral part of antenatal care.

How much do pregnant women know about the importance of oral health in pregnancy? Questionnaire-based survey

Background Although pregnancy is a physiological process it causes hormonal changes that can also affect the oral cavity. Pregnancy increases the risk of gum disease inflammation and tooth caries which could affect the health of the developing baby. Proper oral health is crucial both for mother and her babies and is related with mothers' awareness of this connection. The aim of this study was the self-assessment of women's both oral health and oral health literacy as well as mothers' awareness of the connection of oral health and pregnancy. Material and methods In the study anonymous questionnaire was prepared and provided to be filled in by 200 mothers at the age from 19 to 44 y.o. who gave birth in the gynecological clinic. The questionnaire included demographic, and concerning the areas of oral health before and during pregnancy and after the childbirth questions. Results Only 20% of the investigated women underwent the oral examination before the pregnancy and the next 38.5% underwent it intentionally when the pregnancy had been confirmed. As much as 24% of women pointed out lack of awareness of the importance of proper oral hygiene during pregnancy. 41.5% of investigated women declared complaints during the pregnancy concerning teeth or gums and 30.5% underwent dental treatment; 68%, brushed their teeth properly-twice a day; 32% of women observed deterioration of oral health state during the pregnancy. The knowledge of the importance of oral health during pregnancy presented by the majority of mothers was relatively proper, which was strongly connected with higher education status and living in big cities. A significant correlation between higher birth weight and more frequent daily tooth brushing was observed. Both higher frequency of problems concerning the oral cavity and dental treatment during pregnancy were significantly related to the younger age of mothers. Conclusions The knowledge of women concerning of oral health on the management of pregnancy and development of fetus is still insufficient. Gynecologists should inquire pregnant women if they have done dental examination, and provide wider education about importance of oral health in pregnancy.

The gestational period must be the focus of attention by dental professionals, with a view to promoting oral health and preventing diseases that affect the oral cavity. Several factors can promote the manifestations of oral alterations during pregnancy, especially hormonal changes and the presence of bacterial plaque due to inefficient oral hygiene. In view of this, developing new ways of clarifying the possibilities of treatment and the meaning of chronic conditions as factors of harm to oral health is fundamental for the promotion of oral health in pregnant women. OBJECTIVE: To discuss the importance of implementing strategies to promote oral health during pregnancy, as well as highlighting the dentist's role in achieving the maintenance of the mother's general health. METHODOLOGY: The present study is a review of the literature with a quantitative approach and descriptive character. The articles were obtained after searching the Database: VHL. Database: LILACS and BBO. With the use of keywords: Oral health, Pregnant women and Health Education. As inclusion criteria: full text available, articles in Portuguese, articles published between the years 2012-2022 and original articles. RESULT: The lack of knowledge about oral health makes many pregnant women continue to practice wrong habits, causing many oral pathologies. Therefore, it is necessary to disseminate correct information about dental treatment during pregnancy, in addition to reinforcing the training of dentists to serve this specific public. CONCLUSION: Based on strategies implemented by dentists in prenatal care, the pregnant woman can act as a multiplier agent of preventive information, thus contributing to the promotion of oral health.

Oral health in pregnancy: educational needs of dental professionals and office staff

Objective: Dental care during pregnancy is important for pregnant women and their children. Comprehensive guidelines for the provision of dental services for pregnant patients were published in 2006, but there is relatively little information about their use in actual practice. The aim of this study was to examine differences in knowledge and attitudes regarding dental care in pregnancy among dentists, dental hygienists, dental assistants, and nonclinical office staff. A secondary aim was to identify sources of influence on attitudes and knowledge regarding the guidelines. Methods: A survey was used to collect information from 766 employees of a Dental Care Organization based in Oregon; responses from 546 were included in the analyses reported here. Results: Statistically significant differences in knowledge were found among the professional-role groups. Dentists and hygienists consistently answered more items correctly than did other respondents. Within all professional-role groups, knowledge gaps existed and were most pronounced regarding provision of routine and emergency services. Positive perceptions of providing dental care during pregnancy were associated with higher knowledge scores (z = 4.16, P < 0.001). Conclusions: Updated dental education and continuing education for all dental office personnel are needed to promote the diffusion of current evidence-based guidelines for dental care during pregnancy.

Primary health care providers are in a unique position to treat the pregnant mothers for dental as well as general health for a healthy mother and child. Oral cavity is also subjected to reversible as well as irreversible changes due to hormonal changes during pregnancy. Perinatal mortality rates in Pakistan are more than 10-fold greater than in developed countries. It increased with the severity of periodontal disease. Patients, physicians, and dentists are cautious, often avoiding treatment of oral health issues during pregnancy.
