Traditionally, inflammatory bowel diseases (IBD) ulcerative colitis (UC) and Crohn’s disease (CD). At the same time, there are a number of lesions of the gastrointestinal tract, which can proceed for a long time under the guise of IBD, masking the true cause of the disease. This leads to late diagnosis and, quite often, fatal consequences. These diseases include autoimmune enteropathy (AIE). It is a fairly rare disease characterized by severe diarrhea and immune- mediated damage to the intestinal mucosa.The aim is to describe the criteria of diagnosis, etiology, pathogenesis, epidemiology, clinic and treatment of AIE in children based on an analysis of modern literature.Results. Diagnostic criteria for AIE include chronic diarrhea (lasting more than 6 weeks), malabsorption syndrome, specific histological findings from small bowel biopsy with the exclusion of other causes of villous atrophy. Objective of the work was to investigate the peculiarities of gastrointestinal tract disorders in children with coronavirus disease caused by SARS-CoV-2 virus.Materials and methods. We have analyzed the frequency, terms of emergence, duration and character of gastrointestinal disorders on the basis of medical records of 36 children receiving the treatment for community-acquired pneumonia of coronavirus etiology (positive for SARS-CoV-2 virus) in the children’s infectious disease department of Moscow State Clinical Hospital, Mytishchi. The majority of patients had moderate form of the disease (97 %), severe cases were not registered. The age range of the children – 8 months to17 years old. В статье представлен обзор исследований, посвященных изучению эффективности и безопасности пробиотиков в комплексной терапии нарушений желудочно-кишечного тракта у детей различного возраста. Отмечено, что ежедневное применение препаратов, содержащих Lactobacillus reuteri protectis в виде капель, растворенных в грудном молоке или смеси, существенно снижает длительность плача, увеличивает количество и качество дефекаций, снижает количество регургитаций. Кроме того, L. reuteri protectis обладают противовоспалительным, противомикробным и противовирусным действием, участвуют в цитомукопротекции за счет образования специфических биопленок на слизистой кишечника, а также продуцируют гамма-аминомасляную кислоту – основной нейрорегулятор моторной функции кишечника. Analysis of gastrointestinal tract pathology structure in children

Захворювання органів травлення посідають одне з провідних місць в структурі соматичної патології дитячого віку. Поширеність хронічних захворювань органів гастродуоденальної зони, кишечника, печінки та жовчовивідних шляхів у дітей зростає як в Україні, так і в країнах Західної Європи та Північної Америки. Структуру патологи травної системи складають аномалії та вади розвитку, функціональні та органічні захворювання, а також новоутворення. У дітей молодшого віку переважають функціональні порушення, тоді як у підлітків значно зростає кількість запальних, органічних захворювань шлунка, дванадцятипалої кишки та товстого кишечника. Зберігається залежність рівня захворюваності від пори року. Кількість випадків поєднаної патології та коморбідних захворювань так само має тенденцію до збільшення. Сучасні методи діагностики дозволяють виявляти та диференціювати захворювання на ранніх етапах, що значно покращує прогноз. State of Cell Energy Supply and Its and Correction in Intensive Care for Gastrointestinal Insufficiency Syndrome in Neonates

In order to improve the intensive care for newborns with gastrointestinal insufficiency syndrome by providing the grounds to use amber acid, we carried out a randomized clinical trial involving 110 newborns with gastrointestinal insufficiency syndrome. The main group included newborns who received infusion with sodium succinate (n = 66) as a part of integrated therapy; the control group included the newborns who were not administered the above mentioned medication (n = 44). At the beginning of the study we observed a tendency towards the formation of lactate acidosis in the examined newborns, while no significant correlation was established between the severity of the gastrointestinal insufficiency syndrome and the indicators of cell energy supply. Early vs. late refeeding in acute infantile diarrhea

A randomized ambulatory trial was performed to compare early (6-h) vs. late (24-h) refeeding in acute infantile diarrhea. Ninety infants with mild dehydration were enrolled in the study. Following an initial oral rehydration period (WHO formula), refeeding was introduced using a diet based on either breast milk or cow's milk. Early (n = 53) and late (n = 37) refeeding groups were similar in ethnic background, socioeconomic level, relevant past history, nutritional and clinical state, and stool pathogens. Infants were assessed upon their initial visit, at 24 and 48 h, and at 7 and 14 days thereafter for evaluation of weight, hydration state, stool frequency and need of hospitalization. No significant differences in the above parameters were observed between the two groups. Current understanding of the treatment of newborns with intestinal malrotation: a systematic review

This review aimed to analyze and organize information on the treatment of newborns with bowel malrotation. A systematic review was conducted using the databases PubMed, Web of Science, Google Scholar, Scopus, and eLibrary and journals of Pediatric Surgery, Russian Bulletin of Pediatric Surgery, Anesthesiology and Reanimatology, and Pediatric Surgery from 2010 to 2020. The search terms were as follows: malrotation, neonatal, infant, newborn, intestinal malrotation, malrotation, neonatal period, Ledds syndrome, and midgut volvulus. All journal issues were reviewed for the selected period. Following the screening of published articles, six full-text articles that met the selection criteria were included in the systematic review. Тактика лечения детских запоров должна сочетать немедикаментозные и лекарственные методы, максимально щадящие ребенка. К широко используемым ректальным способам лечения следует прибегать крайне ограниченно.
