Our notion of immunological self tolerance and autoimmune disease has been transformed in the 100 years since Ehrlich coined the term horror autotoxicus (avoidance of autoimmunization) and claimed that autoimmunity was impossible 1 . Burnet, based on his clonal selection theory, proposed in the 1950s that self-reactive clones are normally deleted at immature stages of their development and autoimmunity is therefore an abnormal and exceptional phenomenon 2 . It was also considered possible that self-reactive lymphocytes that escape deletion might simply ignore self antigen because of their weak self-reactivity or might be rendered anergic (inactivated) in the periphery 3 . Then, efforts during the 1980s propounded another mechanism of self tolerance, namely regulatory T cells that actively and dominantly suppress lymphocytes, in particular the self-reactive T cells that exist in the normal periphery despite the existence of deletional mechanisms operative in the thymus 4 . Autoimmune disease may therefore develop as a consequence of altered balance between T reg cells and self-reactive T cells.
